CT-based manual segmentation and evaluation of paranasal sinuses.

Manual segmentation of computed tomography (CT) datasets was performed for robot-assisted endoscope movement during functional endoscopic sinus surgery (FESS). Segmented 3D models are needed for the robots' workspace definition. A total of 50 preselected CT datasets were each segmented in 150-200 coronal slices with 24 landmarks being set. Three different colors for segmentation represent diverse risk areas. Extension and volumetric measurements were performed. Three-dimensional reconstruction was generated after segmentation. Manual segmentation took 8-10 h for each CT dataset. The mean volum

Inadvertent intracranial insertion of a soft rubber tube in a patient with Treacher-Collins syndrome: case report and review of literature.

CASE REPORT: An inadvertent insertion of a soft rubber tube into the intracranial compartment in a patient with Treacher-Collins syndrome is reported. The neonate was delivered vaginally at 34 weeks gestation, had bilateral choanal atresia and presented with apnea at birth. An emergency orotracheal intubation was performed, and ventilatory support was instituted. Computed tomography scan of the facial bones showed a bilateral bony choanal atresia, with a thick posterior nasal septum, and complete absence of maxillary sinuses, external auditory canal, middle ear and ossicles, and roof of ethmoi

Clinical Course and Autopsy Findings of a Patient with Clival Chordoma Who Underwent Multiple Surgeries and Radiation during a 10-Year Period.

The management of clival chordoma remains problematic. We present the case of a 48-year-old woman with clival chordoma who underwent multiple surgeries and radiation therapy, including gamma knife stereotactic radiosurgery (GK-SRS), during a 10-year clinical course. The tumor was initially removed by gross total resection via the trans-sphenoidal approach, followed by external linac radiation therapy. The tumor recurred at the clivus 5 years after the initial operation. After repeated trans-sphenoidal removal of recurrent tumors, she twice underwent GK-SRS for a tumor remnant adjacent to the b

Selective reconstructive options for the anterior skull base.

Carcinomas of the ethmoid, frontal, or maxillary sinuses sometimes invade the anterior skull base. It is necessary to perform en-bloc resection for this invasive carcinoma according to the concepts of surgical treatment for head and neck cancer. The anterior skull base consists of two parts, the orbital roof as the lateral portion and the roofs of the frontal sinus, ethmoid sinus, and/or sphenoid sinus as the central portion. Selective reconstructive options for the anterior skull base depend on the size of the defect of the skull base. A dural defect is repaired by a fascia lata or a pericran

Chronic rhinosinusitis and biofilms.

BACKGROUND AND HYPOTHESIS: Biofilms have been implicated in several head and neck infectious processes such as the following: dental and periodontal disease, otitis media, tympanostomy tube otorrhea, and chronic tonsillitis. We believe that biofilms also are associated with chronic rhinosinusitis. No information is known regarding the presence of biofilms in chronic rhinosinusitis. STUDY DESIGN AND SETTING: With institutional review board approval, tissue was obtained from consenting chronic rhinosinusitis patients who were undergoing functional endoscopic sinus surgery. Specimens were taken

Improving accuracy for inferior alveolar nerve segmentation with multi-label of anatomical adjacent structures using active learning in cone-beam computed tomography.

Recent advancements in deep learning have revolutionized digital dentistry, highlighting the importance of precise dental segmentation. This study leverages active learning with the three-dimensional (3D) nnU-net and multi-labels to improve segmentation accuracy of dental anatomies, including the maxillary sinuses, maxilla, mandible, and inferior alveolar nerves (IAN), which are important for implant planning, in 3D cone-beam computed tomography (CBCT) scans. Segmentation accuracy was compared using single-label, adjacent pair-label, and multi-label relevant anatomic structures with 60 CBCT sc

Ossifying fibroma of the nasal cavity: Case report and literature review.

INTRODUCTION AND IMPORTANCE: Ossifying fibroma is a benign, encapsulated, or demarcated neoplasm consisting of varying amounts of cementum-like tissue or bone within a fibrous tissue stroma. It commonly affects patients aged 20 to 40 years, with a predominance in females; however, it can also affect children and adolescents. In this case report, we will present a rare instance of ossifying fibroma in the nasal cavity. CASE PRESENTATION: A 25-year-old female, a known case of B-cell lymphoma diagnosed in 2022 and treated with chemotherapy, was referred to ENT for an abnormal CT finding. The CT

The sinus surgery completeness score: a radiological assessment of the extent of endoscopic sinus surgery.

PURPOSE: Chronic rhinosinusitis (CRS) often requires endoscopic sinus surgery (ESS) for symptom control. However, there is currently a high reported revision rate. The introduction of biologics offers an alternative treatment, but patient criteria are ambiguous, particularly regarding the definition of "previous ESS." This study aims to introduce the Sinus Surgery Completeness Score (SSCS) to help evaluate the extent of ESS as well as evaluate the extent of sinus surgery in the study cohort. METHODS: The SSCS was developed by expert rhinologists and anterior skull base surgeons and applied to

How to Reduce Inferomedial Orbital Wall Fracture Using a Navigation System: Tips and Pearls.

The orbit is a confined space with a defined bony structure. Bony protrusion into the ethmoid or maxillary sinuses by the blowout fracture can displace orbital tissues, including rectus muscles and adjacent fascial septae. Especially, reconstructing the orbit's floor and medial wall can be challenging when the inferomedial strut or posterior bony ledge is affected, leading to a loss of critical anatomic landmarks and support. Correctly positioning an implant in the precise anatomic location can be challenging. Recent updates to the navigation system have addressed this issue. Despite its early
