Differential distribution of adipokines between serum and synovial fluid in patients with osteoarthritis. Contribution of joint tissues to their articular production.

OBJECTIVE: To analyze the distribution of leptin, adiponectin and resistin between paired serum and synovial fluid (SF) samples of patients … also determined in conditioned media from cultured joint tissues (synovium, infrapatellar fat pad, meniscus, osteophyte, cartilage and bone). RESULTS: The adipokines exhibited different patterns of distribution between the joint and the circulating compartment. Serum levels of resistin and adiponectin exceeded those in the paired SF. Conversely, leptin S …

[Triangular fibrocartilage complex: functional anatomy and histology].

The functional anatomy and histology of triangular fibrocartilage complex (TFCC) were investigated using 20 wrists from fresh cadavers, and a functional model of TFCC was proposed. The dynamic changes in the TFCC during pronation and supination were examined from both the proximal and distal sides. The distal side of the TFCC consisted of the disc proper (DP), the meniscus homologue (MH), the ulnolunate ligament (UL), and the ulnotriquetral ligament (UT). The prestyloid recess was observed between the DP and MH. The ulnar collateral ligament (UCL) existed at the ulnar side of the TFCC and cons…

Preoperative laxity in ACL-deficient knees increases with posterior tibial slope and medial meniscal tears.

PURPOSE: The aim of this study was to determine patient and anatomic factors that influence anteroposterior and rotational laxity in knees with ACL tears. Based on the findings of biomechanical studies, we hypothesized that static and dynamic anterior tibial translation (ATT) as well as positive pivot shift would increase with female gender, tibial slope, and meniscal tears. METHODS: The authors prospectively collected preoperative data and intraoperative findings of 417 patients that underwent ACL reconstruction. The exclusion criteria were: revision ACL procedures (n = 53), other surgical a

Which determinants predict tibiofemoral and patellofemoral osteoarthritis after anterior cruciate ligament injury? A systematic review.

BACKGROUND: Anterior cruciate ligament (ACL) injury is an important risk factor for development of knee osteoarthritis (OA). To identify those ACL injured patients at increased risk for knee OA, it is necessary to understand risk factors for OA. AIM: To summarise the evidence for determinants of (1) tibiofemoral OA and (2) patellofemoral OA in ACL injured patients. METHODS: MEDLINE, EMBASE, Web of Science and CINAHL databases were searched up to 20 December 2013. Additionally, reference lists of eligible studies were manually and independently screened by two reviewers. 2348 studies were ass

Association between varus alignment and post-traumatic osteoarthritis after anterior cruciate ligament injury.

PURPOSE: To investigate the association between varus alignment and post-traumatic osteoarthritis (OA) after an anterior cruciate ligament (ACL) injury. METHODS: One hundred subjects with an acute complete ACL tear were followed for 15 years. Anterior-posterior radiographs of the tibiofemoral joint were obtained with a knee flexion of 20°, and the patellofemoral joint was examined with skyline view at 50° knee flexion. Joint space narrowing and osteophytes were graded in the tibiofemoral and patellofemoral joints in the injured (ACL) and uninjured knee according to the radiographic atlas of t

PCL tibial avulsion with an associated medial meniscal tear in a child: a case report on diagnosis and management.

Posterior cruciate ligament (PCL) injuries from tibial avulsions are rare in the paediatric setting. One would need a high index of suspicion as clinical examination may be difficult, especially in the early period. Magnetic resonance imaging is an excellent diagnostic modality for this condition and other associated injuries within the knee. We report a rare case in which the patient had a PCL avulsion off the tibial insertion site with an associated posterior horn medial meniscal tear off the posterior capsule. He was treated through open reduction and internal fixation of the avulsed fragme

[Glycosaminoglycans as markers of post-traumatic gonarthrosis?].

Osteoarthritis (OA) influences the levels of free intraarticular glucosaminoglycans (GAG). Little is known about the direction--decrease/increase--of these changes, and information on the correlation between GAG levels and the degree of OA is sparse. Objectives of this study were to investigate the correlation between intraarticular levels of sulphated and unsulphated GAG and the degree of experimental OA, the time course of these changes and whether GAG might be useful as a marker for OA. Twenty-one sheep were randomly assigned to three groups: (1) transsection of the posterolateral bundle of

The effects of "decelerated" rehabilitation following anterior cruciate ligament reconstruction on a hyperelastic female adolescent: a case study.

Current concepts in postoperative anterior cruciate ligament (ACL) reconstruction management include participation in an "accelerated" rehabilitation program. There are no published reports examining the effects of accelerated or conservative rehabilitation on subjects with generalized ligamentous hyperelasticity. The purpose of this case study was to examine the effects of a conservative or "decelerated" rehabilitation program on the functional outcome of a hyperelastic female adolescent athlete following ACL reconstruction. The subject was a 15-year-old female basketball player who sustained

[Arthroscopic partial double meniscectomy].

Arthroscopic treatment of meniscal pathology has become a routine procedure within the last 10 years. In our patients who underwent arthroscopic surgery for multiple reasons we found 226 cases with combined lesions of the medial and lateral menisci. Eliminating the patients with instabilities, chondral lesions grade IV and general diseases like rheumatoid arthritis and those who had had surgery previously, 71 patients were left. Forty-four (62%) were male and 27 (38%) female. The average age was 52 years. The lesion was located in the right knee in 41 cases (58%) and in 31 cases (42%) in the l

The long-term course of various meniscal treatments in anterior cruciate ligament deficient knees.

One hundred thirty-five patients were reviewed six to 16 years after anterior cruciate ligament repair or reconstruction. At follow-up evaluation, 60% of the patients demonstrated significant residual laxity on objective testing. Thirty percent of the patients reduced activity levels because of instability. Degenerative arthritis was noted to be more frequent in those patients who had been treated with total open meniscectomy than in those treated with arthroscopic partial meniscectomy, and it was more severe than in patients with intact menisci. Continued participation in high-stress sporting

The problem of chondromalacia patellae.

Daily activity subjects the human patella to forces often several times the individual's body weight. Healthy cartilage can adjust to these forces if they are not too excessive, concentrated, or repetitive. Such abnormal stresses most frequently occur with the disturbance of normal patellar mechanical function. Chondromalacia patellae is the result common to a wide variety of unusual traumata. Treatment must be directed primarily not toward the damaged patellar cartilage but toward a correction of the mechanical abnormality causing it. Until proven otherwise, a young female complaining of knee

Delayed primary isolated posterior cruciate ligament reconstruction is not associated with intraarticular injury prevalence: A study from the Scandinavian Knee Ligament Registries.

INTRODUCTION/OBJECTIVES: Biomechanical studies suggest that persistent posterior cruciate ligament (PCL) deficiency may lead to abnormal joint contact mechanics and subsequent secondary intraarticular injury. However, clinical evidence that delayed PCL reconstruction (PCL-R) directly impacts the prevalence of intraarticular injury remains limited. The aim of the study was to determine the impact of time from injury to surgery on the prevalence of intraarticular injury in patients with primary isolated PCL-R. METHODS: Patients with PCL-R registered in the Scandinavian Knee Ligament Registries

Risk factor for knee cartilage damage in patients with anterior cruciate ligament rupture.

Anterior cruciate ligament (ACL) rupture is frequently accompanied by additional intra-articular lesions, among which articular cartilage-damage is clinically relevant because it may contribute to persistent symptoms and subsequent post-traumatic osteoarthritis. This retrospective observational study consecutively enrolled adults with primary unilateral ACL rupture treated between June 2020 and June 2025. Knee cartilage status was assessed using magnetic resonance imaging (MRI) and/or arthroscopic evaluation and graded according to the International Cartilage Repair Society classification; car
