Association of serum vitamin D levels with clinical severity and recovery duration in cervical dizziness.

BACKGROUND: Cervical dizziness (CD) is associated with altered cervical musculoskeletal and proprioceptive input, leading to impaired sensorimotor integration and balance control. Vitamin D deficiency has been linked to musculoskeletal pain, muscle dysfunction, and impaired postural stability; however, its role in CD remains unclear. METHODS: This prospective observational study included 210 patients diagnosed with CD. Serum 25-hydroxyvitamin D [25(OH)D] levels were measured at baseline, and participants were categorized into deficiency (< 20 ng/mL), insufficiency (20-29 ng/mL), and sufficien

Reflex and functional responses to distal tibial/medial plantar nerve electrical stimulation in the treadmill-locomoting spinal cat.

Low-intensity electrical stimulation of the distal tibial nerve innervating the plantar foot with pulse trains promotes extensor activity in reduced animal preparations when delivered during the extensor phase. Since enhancing extensor activity could potentially improve weight support after spinal cord injury (SCI), we evaluated whether electrical stimulation of the distal tibial nerve or its medial plantar branch enhances/prolongs extensor activity in a locomotor-trained feline model of SCI. With stimulation trains at levels activating large-diameter afferents (low-threshold cutaneous/muscle

Effect of Weight-Shifting Practice Using Auditory Feedback on Postural Control in Patients With Body Lateropulsion: A Single-Case Experimental Design.

Body lateropulsion (BL) is a postural control disorder commonly associated with unilateral brainstem or cerebellar lesions. Patients with BL exhibit a tendency to lean toward one side and experience difficulty maintaining stable standing and walking. Although exercises focused on visual or somatosensory cues have been proposed, no standardized interventions have been established. Auditory feedback has emerged as a promising new approach, as it can complement visual and somatosensory inputs to improve balance. This single-case study investigated whether incorporating auditory feedback into stan

Kinesiotaping Is Not Better Than a Placebo: Kinesiotaping for Postural Control in Anterior Cruciate Ligament-Reconstructed Patients-A Randomized Controlled Trial.

OBJECTIVE: The primary aim of this study was to investigate the immediate and delayed effects of kinesiotape (KT) on postural control and patient-reported outcome measures under challenging conditions in individuals with anterior cruciate ligament reconstructions. METHODS: Thirty-two anterior cruciate ligament-reconstructed patients for whom 6 months had passed since their operation were randomly assigned to either the KT (n = 16, aged 21.8 [5.5] y) or the placebo KT (n = 16, aged 24.0 [5.1] y) groups. Initially, both groups stood barefoot on a force platform while performing postural tasks i

Repeated cold stress, an animal model for fibromyalgia, elicits proprioceptor-induced chronic pain with microglial activation in mice.

BACKGROUND: Fibromyalgia is characterized by chronic pain, fatigue, and other somatic symptoms. We have recently revealed that proprioceptor hyperactivation induces chronic pain in a rat model of myalgic encephalomyelitis. The present study explores whether similar proprioceptor-induced pain is elicited in a mouse model of fibromyalgia. METHODS: Repeated cold stress (RCS) was used as a fibromyalgia model. Pain behavior was examined using the von Frey test, and neuronal activation was examined immunohistochemically as activating transcription factor (ATF)3 expression. The Atf3:BAC transgenic m

Pelvic rotation correction combined with Schroth exercises for pelvic and spinal deformities in mild adolescent idiopathic scoliosis: A randomized controlled trial.

INTRODUCTION: Individualized treatment of spinal deformity is needed for adolescent idiopathic scoliosis (AIS), and the integration of pelvic rotation correction based on proprioceptive neuromuscular facilitation (PNF) into regular physiotherapy may be a promising approach. However, few high-quality studies have investigated its effects. This study aimed to evaluate the efficacy of pelvic rotation correction combined with Schroth exercises in the treatment of mild AIS. METHODS: This was a randomized controlled trial. Forty-two AIS patients were randomly divided into experimental and control g

Blind people can actively manipulate virtual objects with a novel tactile device.

Frequently in rehabilitation, visually impaired persons are passive agents of exercises with fixed environmental constraints. In fact, a printed tactile map, i.e. a particular picture with a specific spatial arrangement, can usually not be edited. Interaction with map content, instead, facilitates the learning of spatial skills because it exploits mental imagery, manipulation and strategic planning simultaneously. However, it has rarely been applied to maps, mainly because of technological limitations. This study aims to understand if visually impaired people can autonomously build objects tha

Dynamic changes of brain networks during standing balance control under visual conflict.

Stance balance control requires a very accurate tuning and combination of visual, vestibular, and proprioceptive inputs, and conflict among these sensory systems may induce posture instability and even falls. Although there are many human mechanics and psychophysical studies for this phenomenon, the effects of sensory conflict on brain networks and its underlying neural mechanisms are still unclear. Here, we combined a rotating platform and a virtual reality (VR) headset to control the participants' physical and visual motion states, presenting them with incongruous (sensory conflict) or congr

Osseoperception in transcutaneous osseointegrated prosthetic systems (TOPS) after transfemoral amputation: a prospective study.

BACKGROUND: Endo-exo prosthetics (EEP), which belongs to the transcutaneous osseointegrated prosthetic systems (TOPS), provides an alternative bone-anchored rehabilitation method for transfemoral amputees. It led to the question of whether transmitted forces from prosthetic feet are perceptible by osseoperception resulting in proprioceptive feedback of ground conditions. OBJECTIVES: The following hypotheses emerged for our trial with the null hypothesis: EEP fitting after transfemoral amputation does not influence osseoperception. Alternative hypothesis 1: EEP patients achieve better osseoper

Magnetic resonance imaging and neurological findings in dogs with disc-associated cervical spondylomyelopathy: a case series.

BACKGROUND: Canine cervical spondylomyelopathy can be separated into osseous and disc-associated (DA-CSM) forms. Our aim was to describe the magnetic resonance imaging (using a high-field scanner) and neurological findings in dogs with DA-CSM and investigate a relationship between these findings. RESULTS: Sixty-three dogs were included: 60/63 (95 %) were large breeds, with Doberman Pinschers and males over-represented (70 %). Mean and median age at the time of diagnosis was 7.25 and 7.2 years (range 0.41-12 years). Chronic signs were noted in 52/63 (83 %) dogs, with proprioceptive ataxia the

Assessing various sensorimotor and cognitive functions in people with epilepsy is feasible with robotics.

BACKGROUND: Epilepsy is a common neurological disorder characterized by recurrent seizures, along with comorbid cognitive and psychosocial impairment. Current gold standards of assessment can quantify cognitive and motor performance, but may not capture all subtleties of behavior. Here, we study the feasibility of assessing various upper limb sensorimotor and cognition functions in people with epilepsy using the Kinarm robotic assessment system. We quantify performance across multiple behavioral domains and additionally consider the possible effects of epilepsy subtype and medication. METHODS

Progressive Modular Rebalancing System and Visual Cueing for Gait Rehabilitation in Parkinson's Disease: A Pilot, Randomized, Controlled Trial With Crossover.

Introduction: The progressive modular rebalancing (PMR) system is a comprehensive rehabilitation approach derived from proprioceptive neuromuscular facilitation principles. PMR training encourages focus on trunk and proximal muscle function through direct perception, strength, and stretching exercises and emphasizes bi-articular muscle function in the improvement of gait performance. Sensory cueing, such as visual cues (VC), is one of the more established techniques for gait rehabilitation in PD. In this study, we propose PMR combined with VC for improving gait performance, balance, and trunk

Postural control during galvanic vestibular stimulation in patients with persistent perceptual-postural dizziness.

Over the past years galvanic vestibular stimulation (GVS) has been increasingly applied to stimulate the vestibular system in health and disease, but not in patients with persistent postural-perceptual dizziness (PPPD) yet. We functionally tested motion perception thresholds and postural responses to imperceptible noisy (nGVS) and perceptible bimastoidal GVS intensities in patients with PPPD with normal vestibulo-ocular reflexes. We hypothesized that GVS destabilizes PPPD patients under simple postural conditions stronger compared to healthy controls. They were compared to healthy subjects und

Postural Stability in Healthy Child and Youth Athletes: The Effect of Age, Sex, and Concussion-Related Factors on Performance.

BACKGROUND: Postural stability plays a key role in sport performance, especially after concussion. Specific to healthy child and youth athletes, little is known about the influence development and sex may have on postural stability while considering other subjective clinical measures used in baseline/preinjury concussion assessment. This study aims to describe age- and sex-based trends in postural stability in uninjured child and youth athletes at baseline while accounting for concussion-related factors. HYPOTHESES: (1) Postural stability performance will improve with age, (2) females will di

Brown-Séquard syndrome after a gun shot wound to the cervical spine: a case report.

BACKGROUND CONTEXT: Brown-Séquard syndrome is characterized by a hemisection of the spinal cord most commonly after spinal trauma or neoplastic disease. The injury causes ipsilateral hemiplegia and proprioceptive sensory disturbances with contralateral loss of pain and temperature sensation. Patients with Brown-Séquard syndrome have the best prognosis of all spinal cord injury patterns. At this time, the ideal management for Brown-Séquard syndrome after penetrating trauma has yet to be defined. PURPOSE: To report a case of a gun shot wound to the upper cervical spine that resulted in Brown-Sé

Muscle spindle traffic in functionally unstable ankles during ligamentous stress.

CONTEXT: Ankle sprains are common in athletes, with functional ankle instability (FAI) developing in approximately half of cases. The relationship between laxity and FAI has been inconclusive, suggesting that instability may be caused by insufficient sensorimotor function and dynamic restraint. Research has suggested that deafferentation of peripheral mechanoreceptors potentially causes FAI; however, direct evidence confirming peripheral sensory deficits has been elusive because previous investigators relied upon subjective proprioceptive tests. OBJECTIVE: To develop a method for simultaneous

Ankles back in randomized controlled trial (ABrCt): braces versus neuromuscular exercises for the secondary prevention of ankle sprains. Design of a randomised controlled trial.

BACKGROUND: Ankle sprains are the most common sports and physical activity related injury. There is extensive evidence that there is a twofold increased risk for injury recurrence for at least one year post injury. In up to 50% of all cases recurrences result in disability and lead to chronic pain or instability, requiring prolonged medical care. Therefore ankle sprain recurrence prevention in athletes is essential. This RCT evaluates the effect of the combined use of braces and neuromuscular training (e.g. proprioceptive training/sensorimotor training/balance training) against the individual
