Fetal cerebrovascular response to maternal hyperoxygenation in congenital heart disease: effect of cardiac physiology.

OBJECTIVE: Fetal cerebrovascular resistance is influenced by several factors in the setting of intact autoregulation to allow for normal cerebral blood flow and oxygenation. Maternal hyperoxygenation (MH) allows for acute alterations in fetal physiology and can be a tool to test cerebrovascular reactivity in late-gestation fetuses. In this study, we utilized MH to evaluate cerebrovascular reactivity in fetuses with specific congenital heart disease (CHD). METHODS: This was a cross-sectional study of fetuses with complex CHD compared to controls without CHD. CHD cases were grouped according to

Longitudinal trends in fetoplacental biochemical markers, uterine artery pulsatility index and maternal blood pressure during the first trimester of pregnancy.

OBJECTIVE: To assess trends in levels of biochemical markers, uterine artery (UtA) pulsatility index (PI) and maternal blood pressure changes over time and study their relationships in uncomplicated first-trimester pregnancies. METHODS: The study population comprised 86 women with singleton pregnancies. In each woman, a blood sample was collected at 6-7, 8-9, 10-11 and 12-13 weeks' gestation. At the same visit blood pressure was measured and ultrasound examination was performed to measure the crown-rump length and Doppler flow velocity waveform patterns of both UtAs. Serum concentrations of p

Pulmonary artery smooth muscle activation attenuates arterial dysfunction during acute pulmonary hypertension.

Acute pulmonary hypertension (PH) may arise with or without an increase in vascular smooth muscle (VSM) tone. Our objective was to determine how VSM activation affects both the conduit (CF) and wall buffering (BF) functions of the pulmonary artery (PA) during acute PH states. PA instantaneous flow, pressure, and diameter of six sheep were recorded during normal pressure (CTL) and different states of acute PH: 1) passively induced by PA mechanical occlusion (PPH); 2) actively induced by intravenous administration of phenylephrine (APH); and 3) a combination of both (APPH). To evaluate the direc

Thyroid autoimmunity is associated with increased uterine artery blood flow resistance and predicts adverse pregnancy outcomes in women with unexplained recurrent pregnancy loss: a retrospective cohort study.

BACKGROUND: Uterine artery blood flow resistance is critical for endometrial receptivity and successful implantation. Thyroid autoimmunity (TAI) is prevalent in recurrent pregnancy loss (RPL), but whether TAI affects uterine hemodynamics before conception and whether this predicts subsequent pregnancy outcomes has not been directly investigated. OBJECTIVE: This study aimed to determine whether TAI is associated with increased uterine artery resistance in non-pregnant women with unexplained RPL (URPL) and whether elevated resistance indices predict adverse subsequent pregnancy outcomes, thereb

Role of Ultrasonography and Color Doppler in the Assessment of High-Risk Pregnancies and Their Accuracy in Predicting Fetal Outcome.

Introduction Various methods are employed to evaluate the well-being of the fetus in high-risk pregnancies which consists of a biophysical profile (BPP), a non-stress test (NST), and daily fetal movements. Detection of aberrant blood flow in fetoplacental beds has been revolutionized by recent developments in ultrasound technology, such as color Doppler flow velocimetry. The cornerstone of maternal and fetal care is lowering maternal and perinatal mortality and morbidity is antepartum fetal surveillance. Doppler ultrasound is a non-invasive way of obtaining a qualitative and quantitative evalu

Distal radial access in acute coronary syndrome patients with good arterial pulsation: a subgroup analysis from the KODRA registry.

BACKGROUND/AIMS: Distal radial access (DRA) has been associated with fewer access-site complications, but evidence in acute coronary syndrome (ACS) remains limited. This study evaluated the feasibility and safety of DRA in ACS patients with good arterial pulsation. METHODS: Patients with good arterial pulsation from the prospective, multicenter KODRA registry were analyzed, comparing those with ACS (n = 1,618) and non-ACS (n = 2,588). The primary efficacy endpoint was successful coronary angiography (CAG) without access-site crossover. The primary safety endpoint was DRA-related bleeding, and

Placental function and fetal weight are associated with maternal hemodynamic indices in uncomplicated pregnancies at 35-37 weeks of gestation.

BACKGROUND: Over the years, there has been an increasing interest in the assessment of maternal hemodynamic responses during pregnancy. With the use of both noninvasive devices and/or maternal echocardiography, it has been shown that mothers who have pregnancy complications have altered hemodynamics compared with those who have uncomplicated pregnancies. It also has been suggested that preexisting maternal cardiac changes might drive the development of complications in pregnancy that are associated with impaired placentation. To understand, however, this potential link in complicated pregnanci

Characterisation of the intracranial pressure waveform during infusion studies by means of central tendency measure.

OBJECTIVE: In the present study an attempt was made to quantify and characterise the changes in the intracranial pressure (ICP) waveform over the wide pressure range covered during infusion studies by means of the central tendency measure (CTM). CTM is a non-linear approach using continuous chaotic modelling that summarises the degree of variability in a signal. METHODS: CTM of the ICP wave in the lumbar subarachnoid space was analysed in 77 infusion studies performed in patients with idiopathic and secondary forms of normal pressure hydrocephalus (median age 74 years, range 22-88). Increased arterial stiffness in healthy subjects with high-normal glucose levels and in subjects with pre-diabetes.

BACKGROUND: Increased fasting plasma glucose (FPG), which includes impaired fasting glucose (IFG), impaired glucose tolerance (IGT), and diabetes, is a risk factor for arterial stiffness. While IFG is widely accepted as a cardiovascular risk factor, recent studies have argued that subjects with high-normal glucose level were characterized by a high incidence of cardiovascular disease. The purpose of this study is to investigate the relationship between FPG and arterial stiffness in non-diabetic healthy subjects. METHODS: We recruited 697 subjects who visited the health promotion center of a u

Increased carotid wall stress in vascular Ehlers-Danlos syndrome.

BACKGROUND: Vascular Ehlers-Danlos syndrome (vEDS), also known as EDS type IV, an inherited disorder of connective tissue, results from mutations in the gene encoding type III procollagen (COL3A1). Affected patients are at risk for arterial dissection or rupture, the main cause of death. To understand the pathogenesis of the vascular lesions, we used a biomechanical approach and determined steady and pulsatile wall stress. METHODS AND RESULTS: Sixteen patients with vEDS and 16 age-, gender-, and blood pressure-matched control subjects were included in this cross-sectional noninvasive study. C

Noninvasive surrogate of main pulmonary artery stiffness is associated with right ventricular function in experimental pulmonary arterial hypertension.

Main pulmonary artery (MPA) stiffening increases right ventricular (RV) afterload and is associated with worse outcomes in pulmonary arterial hypertension (PAH). Measures of pulmonary artery compliance (PAC) and vascular resistance (PVR) require invasive catheterization, limiting clinical application. Echocardiographic assessment of MPA pulsatility, the relative change in MPA diameter, may be useful but not well validated. We investigated the relationship between MPA pulsatility and passive stiffness, PAC, PVR, RV function, and RV-PA coupling in sugen-hypoxia (SuHx) rats and children with idio
