Guillain-Barré syndrome following severe head trauma and spine surgery.

Guillain-Barré syndrome (GBS) is an acute-onset inflammatory polyradiculoneuropathy usually triggered by an infectious disease. In some cases, GBS can occur without any preceding infectious episode, like after vaccination, epidural anaesthesia or surgery. A 73 years old woman had head and spine trauma. Body-TDM showed bilateral temporal and right frontal haematomas and fracture of the first lumbar vertebrae. Sextant and kyphoplasty were performed. She presented 14 days after surgery tetraparesis, swallowing difficulties and bilateral facial palsy. Electromyography was consistent with demyelina

Aortic valve tear with severe aortic regurgitation following blunt chest trauma.

An aortic valve tear associated with aortic regurgitation following blunt chest trauma is seldom seen. In this case, a 55-year-old man sustained a non-penetrating chest injury caused by a sudden fall from 10 meters. This led to a sizable tear in the left coronary cusp associated with severe aortic insufficiency. The case was treated successfully by surgical replacement of the aortic valve with a mechanical prosthesis.

The epidemiology of fractures of the proximal ulna.

INTRODUCTION: The aim of our study was to report the epidemiological characteristics of fractures of the proximal ulna. METHODS: From our prospective trauma database of 6872 fractures, we identified all acute fractures of the proximal ulna from a 1-year period between July 2007 and June 2008. Age, gender, mode of injury, fracture classifications, associated injuries and treatment were the factors documented and analysed. RESULTS: There were 78 fractures of the proximal ulna with a mean age of 57 years (15-97). Males (n=35) sustained their fracture at a significantly younger age than females

Talar neck fracture associated with talonavicular dislocation and fracture of posteromedial process of talus.

Canale and Kelly modified the Hawkins classification of talar neck fractures by adding a type IV fracture dislocation to the original 3 types described. Type IV injury occurs rarely and is the only type involving dislocation of the talonavicular joint. To our knowledge, only 2 unusual cases have been described in published studies in which a fracture of neck of the talus occurred in association with talonavicular dislocation but the ankle and subtalar joints remained intact. We describe a very unusual case of an adult male with a fracture of the neck of the talus with talonavicular dislocation

Arthroscopic treatment of an osteochondral lesion in a child with chronic wrist pain: case report.

Osteochondral lesions of the wrist are rare in the pediatric and adolescent population. We report a case of an osteochondral lesion of the lunate facet of the radiocarpal joint in a 6-year-old girl with chronic wrist pain after falling on her hand. Arthroscopic debridement of the osteochondral fragment relieved the symptoms and improved wrist function. Osteochondral lesions should be considered in the differential diagnosis of chronic wrist pain in children with a history of trauma.

Unintentional falls at home among young and middle-aged New Zealanders resulting in hospital admission or death: context and characteristics.

AIM: This study investigates the characteristics and contexts of unintentional falls at home among young and middle-aged adults. METHOD: We conducted a population-based study of individuals aged 25-59 years resident in Auckland who were admitted to hospital or died following a non-occupational fall at home (July 2005-June 2006). Information was obtained from participant or proxy interviews, and reviews of inpatient records. RESULTS: 344 patients (including 1 death) met the study eligibility criteria representing an overall age-specific incidence rate of 54.0/100,000 (95% CI 48.6-60.1) for th

Modified stoppa approach for acetabular fractures with anterior and posterior column displacement: quantification of radiographic reduction and analysis of interobserver variability.

OBJECTIVES: To quantify the reduction of acetabular fractures with displacement of the anterior and posterior column by using only a single anterior approach that combines the modified Stoppa approach with the lateral window of the classic ilioinguinal approach. The second objective was to evaluate the interobserver variability of our technique for radiographic evaluation of the reduction. SETTING: Level I trauma referral center. PATIENTS: A consecutive cohort of 17 patients with displaced acetabular fractures treated operatively with use of only an anterior approach that combined the modifi

Spinal cord injury without radiological abnormality in preschool-aged children: correlation of magnetic resonance imaging findings with neurological outcomes.

OBJECT: Spinal cord injury without radiological abnormality (SCIWORA) was defined in the era when magnetic resonance (MR) images were not popularly used as diagnostic tools. Although it is generally accepted that MR imaging can effectively illustrate the level and severity of spinal cord injury in the acute phase of trauma, only a few reports of MR imaging studies of SCIWORA have been published. The authors retrospectively reviewed nine preschool-aged patients with SCIWORA to study the correlation between MR imaging findings and the outcomes of neurological deficits, with an elimination of the

Follow-up treatment for osteoporosis after fracture.

Studies of the management of osteoporosis in older women who have had hip or wrist fractures have found underdiagnosis and undertreatment of the disease. Few such studies have been conducted in the United States, however, and most studies have been confined to a subset of the treatments currently available to treat osteoporosis. Mail surveys were sent to 381 women between 50 and 84 years of age who had been treated for a hip or wrist fracture at a large northeast US teaching hospital between October 1, 1998, and September 30, 2000. These surveys included questions about osteoporosis risk facto

Fracture penis--a study of twenty cases.

During the period from June 1985 to December 1999 a total of 20 male patients of 17-52 years age group were managed for traumatic fracture of the penile shaft. Diagnosis in all cases was established by history and careful physical examination. All but one underwent early surgical intervention and repair of the fractured part. Twelve patients, who reported for treatment within 36 hrs. of injury had good results. Of the 7 patients, who reported after 36 hrs. of injury 4 had good and 3 had fair results. One patient who presented on the 7th day after injury didn't undergo surgery and developed fib

Posttraumatic carotid-cavernous sinus fistula.

BACKGROUND AND OBJECTIVES: Posttraumatic carotid-cavernous sinus fistula is a rare complication of maxillofacial trauma and is seldom discussed in the literature. Motor vehicle accidents, falls, and other crush injuries contribute to the incidence of basilar skull fractures and the formation of fistulae. When injuries occur in the vessel wall, the carotid artery has the potential to fill the low-pressure cavernous sinus. The symptoms include chemosis, proptosis, pulsating exophthalmos, diplopia, ophthalmoplegia, orbital pain, audible bruits, and blindness. METHODS AND MATERIALS: The conventio

Sex and age-related implications for preventive measures of intensive care admitted traumatic brain injury patients in Switzerland: an observational study.

PURPOSE: Epidemiological studies of traumatic brain injury (TBI) in Switzerland have, to date, poorly investigated sex-related differences in causality and predisposing factors. This study examines differences in sex and age related TBI epidemiology in a high-volume trauma centre intensive care unit (ICU) cohort, aiming to identify potential targets for prevention. METHODS: This retrospective, single centre study includes all consecutive TBI patients admitted to the ICU in a 4-year study period. Patient demographics, comorbidities, co-medication, trauma setting and associated risk behaviour w

Twelve-year review of horse-related trauma registry presentations to a regional Australian Emergency Department.

BACKGROUND: The size, strength, and unpredictability of horses, makes any interaction a potential situation for minor to severe injury. Despite this, horses continue to be popular for work and recreational activities. There have been five recent Australian publications that have examined various aspects of horse-related trauma, but none have been conducted in regional New South Wales (NSW). This study aims to report on the pattern of horse-related trauma that met the Institute of Trauma Injury Management's (ITIM) Trauma Registry criteria for people who presented to the emergency department at

Pattern of fall and its determinants among elderly patients attending the general outpatient clinic of a tertiary hospital in northern Nigeria: a cross-sectional study.

INTRODUCTION: falls in the elderly are a neglected health problem in many societies, particularly in the developing world. Many health and social service providers are unprepared to prevent and manage falls and related injuries as they lack sufficient knowledge to identify their predisposing factors. For this reason, this study aims to identify the pattern of falls and its determinants among the elderly in northern Nigeria. METHODS: a cross-sectional study was conducted among 300 elderly patients, selected by systematic random sampling. An interviewer-administered questionnaire was used. Data

Risk of falls and injuries requiring hospitalisation after first-eye cataract surgery in elderly Australians.

PURPOSE: To investigate the impact of the type of the intraocular lenses (IOLs) in first-eye cataract surgery in elderly people on the risk of hospitalisation due to falls and injuries. METHODS: A retrospective cohort study was conducted using the Australian Government Department Veterans' Affairs claims data. All people aged 65 years and above who had first cataract surgery between January 2007 and July 2017 were identified. Two cohorts were established depending on the type of IOL-monofocal and multifocal. The risk of injuries and falls requiring hospitalisation in the first 3 months post t

Characteristics of single-vehicle crashes with e-bikes in Switzerland.

In Switzerland, the usage and accident numbers of e-bikes have strongly increased in recent years. According to official statistics, single-vehicle accidents constitute an important crash type. Up to date, very little is known about the mechanisms and causes of these crashes. To gain more insight, a survey was conducted among 3658 e-cyclists in 2016. The crash risk and injury severity were analysed using logistic regression models. 638 (17%) e-cyclists had experienced a single-vehicle accident in road traffic since the beginning of their e-bike use. Risk factors were high riding exposure, male

Risk Factors for Intracranial Haemorrhage in Accidents Associated with the Shower or Bathtub.

BACKGROUND: There has been little research on bathroom accidents. It is unknown whether the shower or bathtub are connected with special dangers in different age groups or whether there are specific risk factors for adverse outcomes. METHODS: This cross-sectional analysis included all direct admissions to the Emergency Department at the Inselspital Bern, Switzerland from 1 January 2000 to 28 February 2014 after accidents associated with the bathtub or shower. Time, age, location, mechanism and diagnosis were assessed and special risk factors were examined. Patient groups with and without intr

Head injury from falls in children younger than 6 years of age.

UNLABELLED: The risk of serious head injury (HI) from a fall in a young child is ill defined. The relationship between the object fallen from and prevalence of intracranial injury (ICI) or skull fracture is described. METHOD: Cross-sectional study of HIs from falls in children (<6 years) admitted to UK hospitals, analysed according to the object fallen from and associated Glasgow Coma Score (GCS) or alert, voice, pain, unresponsive (AVPU) and CT scan results. RESULTS: Of 1775 cases ascertained (median age 18 months, 54.7% boys), 87% (1552) had a GCS=15/AVPU=alert. 19.3% (342) had a CT scan:
