Gender differences in clinical features and outcomes of Fournier's gangrene among emergency department patients.

BACKGROUND: Fournier's gangrene (FG) is a fulminant necrotizing fasciitis of the perineal, genital, and perianal regions that predominantly affects men and carries substantial morbidity and mortality. The presentation and outcomes of FG in women remain less well characterized. Our objective was to compare clinical characteristics, treatment, and short-term outcomes between male and female emergency department patients with FG. METHODS: This retrospective cohort study included adult patients presenting to the EDs of 11 affiliated hospitals across 19 counties in Michigan from 2012 to 2024 with

A Striking Increase in Carriage Among Young Children in Iceland Paralleled the Unprecedented Increase of Invasive Group A Streptococcal Infection From 2022 to 2023.

BACKGROUND: An unprecedented increase in pediatric invasive group A streptococcal (iGAS) infections was observed in most countries, including Iceland, in early 2023. The reasons for this rise are largely unknown. The aim of the study was to describe the parallel between nasopharyngeal (NP) carriage of group A streptococcal (GAS) in Icelandic children and the incidence of invasive disease. METHODS: Electronic health records were used to identify children admitted to the Children's Hospital, Iceland, with iGAS. NP samples from asymptomatic children were collected for culture in February and Mar

Analysis of Antimicrobial Resistance and Virulence Factors in Multidrug-Resistant Streptococcus suis Serotype 2 Isolates Using Whole-Genome Sequencing.

Multidrug-resistant (MDR) Streptococcus suis (S. suis) is a zoonotic pathogen capable of infecting pigs across all age groups, leading to conditions such as meningitis, arthritis, and endocarditis. In humans, infections can result in septic arthritis, meningitis, necrotizing fasciitis, and septicemia, which may be fatal. The absence of a complete genome sequence hinders comprehensive bioinformatic studies of MDR S. suis derived from pigs. In this study, we present the whole-genome sequence of MDR S. suis serotype 2 ST01 isolated from joint fluid samples obtained from pigs. Whole-genome analysi

Differential expression profile of streptolysin S in the covR and covS mutants of group A Streptococcus is a mediator of keratinocyte death and aggressive local infection.

Group A Streptococcus (GAS) isolates with spontaneous mutations in covR and covS are frequently identified in patients with necrotizing fasciitis and toxic shock syndrome. CovR/CovS is a two-component regulator system, and the phosphorylation of CovR is solely modulated by CovS. Nonetheless, the phenotype of the ∆covS mutant and the ∆covR mutant is not entirely identical. It has been shown that the expression of SpeB protease is only detected in the ∆covR mutant, and the ∆covR mutant causes more severe local lesions than the ∆covS mutant in the mouse infection model. SpeB-mediated gasdermin A

Reconstruction of Noma Sequelae: A Surgical Treatment Algorithm Developed from Lessons from 210 Cases in Ethiopia.

UNLABELLED: Noma is an infectious disease affecting mostly children aged 0-10. Although it has almost completely disappeared from the Western world, it is still prevalent in many developing regions, mainly Africa's Sahel region. The infection behaves like a necrotizing fasciitis of the face, originating from the gums and progressively expanding into the cheek, nose, or eye regions. In an estimated 90% of cases, the disease is lethal as a result of systemic sepsis. For survivors, typical results are extensive defects of the cheek, nose, and periorbital and perioral regions. Due to the defects,

Understanding antimicrobial susceptibility profile of Finegoldia magna: an insight to an untrodden path.

BACKGROUND: Finegoldia magna (formerly known as Peptococcus magnus or Peptostreptococcus magnus) belonging to phylum Firmicutes, class Clostridia and genus Finegoldia, is the only species known to cause infections in human beings. Amongst Gram positive anaerobic cocci, F. magna is known to be the most virulent with a high pathogenic potential. Significant upsurge in antimicrobial resistance among anaerobes has been documented by various studies. F. magna is known to be susceptible to most of the anti-anaerobic antimicrobials, however, multidrug resistant strains are being reported in literatur

Fournier's gangrene and fecal diversion. When, in which patients, and what type should I perform?

INTRODUCTION: Fournier's gangrene (FG) is a necrotizing fasciitis affecting the perineum and urogenital tissue. The mortality rate is high although early detection and aggressive debridement can reduce mortality by up to 16%. The prevalence of sequelae is very high and a colostomy is often necessary to control the perineal wound. MATERIAL AND METHODS: A retrospective study was carried out to recruit all patients operated on by the General Surgery and Urology Departments with a diagnosis of GF at the University Hospital over 22 years. Mortality, the Fournier gangrene severity index (FGSI), and
