Intraosseous ganglia are benign cystic and often multiloculated lesions located in the subchondral bone. Only 13 cases around the shoulder have been described in the literature. These lesions are often asymptomatic, but, in cases located close to neurovascular structures or articular surfaces, they can be symptomatic, causing pain, neurologic dysfunction, or articular fractures. This article reports the clinical, radiographic, and pathological findings of a 34-year-old man reporting shoulder pain during heavy working activities. At physical examination, his left shoulder showed complete active

We report the outcome of humeral head surface replacement hemiarthroplasty performed at our institution using the Copeland prosthesis. We followed 56 shoulders (52 patients) for a mean of 34.2 months (range, 24-63 months). Two were lost to follow-up, and there were six deaths unrelated to the shoulder surgery. Preoperative diagnoses in the remainder were osteoarthritis (20), rheumatoid arthritis (26), rotator cuff tear arthropathy (1), and post-traumatic arthrosis (1). The mean age was 68 years. Constant scores for the whole group improved from a mean preoperative score of 16.4 (range, 8-36) t

BACKGROUND: Rotator cuff tendinopathy (RCT) is a major contributor to over 30 million surgeries which are conducted to treat shoulder overuse injuries worldwide annually. Accumulating evidence indicates that neuropsychiatric disorders (ND) share pathogenic pathways with tendinopathy. However, unclarified causal relationships between these two disease spectra undermine the individualized design of treatment strategies benefiting patients with ND while minimizing the RCT risk. We aimed to unveil whether ND were genetically associated with increased RCT occurrence by conducting a two-sample Mende

INTRODUCTION: Blood flow restriction therapy (BFRT) has gained attention for its capacity to induce substantial muscle hypertrophy and strength gains even when employing relatively minimal loads. Strength training is of significant importance in the rehabilitation of patients experiencing shoulder pain, which may arise from a multitude of sources, including rotator cuff injuries, tendinopathies or postsurgical recovery. However, traditional resistance training can be challenging for these individuals due to the presence of pain and functional limitations. In this regard, BFRT in conjunction wi…

INTRODUCTION: A patient presented with traumatic posterior shoulder dislocation and underwent a diagnostic arthroscopy, where an acute subscapularis anterior leading edge tear was identified. This tear patient is exceptionally uncommon, having only been cited in the literature 5 times as of 2013, and to our knowledge, has no known documented cases since. This report seeks to summarize challenges associated with such a rare diagnosis by presenting a case where this rare presentation was timely diagnosed and treated with an optimal outcome, and to highlight pertinent details of the case that are

BACKGROUND: While glenoid bone loss (GBL) after anterior shoulder instability correlates with poor functional outcomes, the specific effects of GBL in posterior and combined-type shoulder instability remain poorly characterized, especially in a high-risk military population.
PURPOSE/HYPOTHESIS: The purpose of this study was to compare GBL between unidirectional anterior or posterior instability versus combined-type instability in active-duty servicemembers. It was hypothesized that total GBL and GBL in the direction of instability would be greater in those with combined-type instability compa

BACKGROUND: Fluoroquinolones (FQs) are associated with increased risk of tendon injury but comparative risk versus other antibiotic options for the same indication has yet to be evaluated.
OBJECTIVE: Describe the incidence (relative risk) of any tendon injury in patients receiving FQ compared with other (non-FQs) antibiotics for treatment of community-acquired pneumonia (CAP).
METHODS: A retrospective propensity score weighted cohort study was performed to evaluate the association between FQ antibiotics and tendon injury risk at 2 time points (within 1 month and within 6 months of use) compa

PURPOSE: Synthetic ligaments have been widely used in the knees for anterior cruciate ligament reconstruction and in the shoulder rotator cuff repair, but they have been rarely used in the hand. The only reported usage is in the Artelon space for carpometacarpal joint osteoarthritis. We describe our experience using the synthetic ligament or scaffold known as Orthotape (its predecessor was known as the Leeds-Keio ligament) in the hand and fingers.
METHODS: We retrospectively reviewed all patients in whom polyester synthetic ligament (Orthotape) was used to reconstruct absent tendons involving

BACKGROUND: Long head biceps tendon pathology is a common cause of anterior shoulder pain and is often associated with other shoulder conditions, such as rotator cuff tears and osteoarthritis. It is well accepted that older patients are at increased risk for major and minor peri- and postoperative complications. The purpose of this study is to investigate patients over 65 years old who underwent subpectoral biceps tenodesis and compare the complication rates of this group to those of patients younger than 65 years old. The hypothesis is, that there would be no difference in complication rates

BACKGROUND: Scapular dyskinesis (SD) is a common co-morbidity with shoulder complex injuries. However, the presence of shoulder pain (SP) with SD is inconsistent, suggesting a need for individualized assessment and treatment approaches. The purpose of this case report is to describe the differential diagnosis of an athlete with SD and SP and a rehabilitation strategy integrating physical therapy (PT) with a Stroke Efficiency Rating Analysis/Injury Risk Assessment (SER/IRA) to identify biomechanical faults during the tennis serve.

Greater trochanteric pain syndrome (GTPS) is a term used to describe disorders of the peritrochanteric region. This constellation of conditions includes greater trochanteric bursitis, gluteus medius (GM) tears, and external coxa saltans or snapping hip syndrome. Tears of the abductor mechanism, more specifically gluteus medius tears, have recently gained a considerable amount of interest in the orthopaedic literature. Abductor tears were first described by Bunker and Kagan in the late 1990s. They used the rotator cuff as an analogous structure to describe the pathological process associated wi

PURPOSE OF THE STUDY: Centromedullary nailing with a Seidel nail is a validated treatment for humeral shaft fractures which some teams have abandoned due to the injury caused to the rotator muscles. The purpose of this study is to assess shoulder function (Constant and DASH) after nailing procedures and to analyze sonographic findings.
MATERIAL AND METHODS: Twenty-nine patients who underwent Seidel nailing between 1996 and 2002 were reviewed by an independent operator at 36 months follow-up on average (range 11-84 months). The sex-ratio was 1.64. Mean age at surgery was 41.5 years (range 17-8

The efficiency of a pure carbon prosthesis to reconstruct intra-articular ligamentous loss of substance was evaluated on 14 sheep with an 18-month follow-up study of recent and old ACL experimental injuries. Functional results showed a correlation between a tightened implant, the stability of the knee, and the absence of severe arthritis. The rupture strength of the newly formed ligament amounts to about 300 Newtons (N). That of the normal ACL amounts to 250-550 N. The structure gives a viscoelastic behavior to the prosthesis. This tissue was made of collagen fibers surrounding the carbon fibe

BACKGROUND: Despite being a common orthopedic injury, the optimal management of proximal humeral fractures remains a topic of debate. Although intramedullary nails have been used to treat these fractures, several complications have been reported with older nail devices. We describe our surgical technique and aim to evaluate the early functional and radiological outcomes of displaced Neer's two- to four-part proximal humeral fractures undergoing fixation with a contemporary straight intramedullary nail.
METHODS: A prospective cohort study of 22 consecutive patients (mean age 55.7 ± 18.0) with

The unparalleled velocity achieved by overhead throwers subjects the shoulder to extreme forces, resulting in both adaptive changes and pathologic findings that can be detected at imaging. A key biomechanical principle of throwing is achieving maximum external rotation, which initially leads to adaptive changes that may result in a pathologic cascade of injuries. In addition to the well-established concepts of glenohumeral internal rotation deficit and internal impingement, osseous and soft-tissue injuries of the shoulder unique to overhead athletes are illustrated. The epidemiology and biomec

Faced to shoulder pathology, the way of investigating must be very systematic. Before doing sophisticated explorations (arthroCT, MRI), we first have to use the triad "interrogatory-clinical examination-standard X Rays". This first step is absolutely necessary; it must allow to make the diagnosis to which the therapy must be adapted. The rotator cuff pathology is the most frequent one. Its frequency depends on patient's age, precessive trauma or not and the pratice of activities using the arm up (sports, leasures, works). A stiff shoulder must never be neglected, the diagnosis is done by clini

The glenohumeral joint is the most dislocated articulation, accounting for more than 50% of all joint dislocations. The reason behind shoulder instability should be investigated in detail for successful management, and the treatment plan should be individualized for all patients. Several classification systems have been proposed for glenohumeral instability. A physical exam is mandatory no matter what classification system is used. When treating patients with anterior shoulder instability, surgeons need to be aware of the critical size of the bone loss, which is commonly seen. The glenoid trac

The aim of this prospective study was to assess the clinical outcomes of an unstable fracture of the lateral end of the clavicle treated with an arthroscopic-assisted locking compressive plate (LCP) clavicular hook plate. Twenty-three patients underwent arthroscopic assisted LCP clavicular hook plate fixation for these fractures. All patients achieved clinical and radiological union over a mean of 4.2 months (range, 3.4-5 months). Four patients (17%) showed some degree of acromial osteolysis. Three patients (13%) showed radiological signs of arthrosis of the acromioclavicular joint. In one pat
