Death of a child associated with multiple overdoses of acetaminophen.

A case of long-term acetaminophen overdosage in a six-year-old child, which contributed to her death despite optimal medical management including oral acetylcysteine therapy, is reported. Acetaminophen 325 mg every six hours was prescribed for fever associated with measles. Believing that acetaminophen was nontoxic, the child's mother progressively increased the dose over three days, first in response to fever and subsequently for abdominal pain probably secondary to unrecognized acetaminophen toxicity. On admission to the hospital, the patient's serum acetaminophen concentration was 163 micro

Maxillary odontogenic sinusitis, complicated with cerebral abscess--case report.

Maxillary sinus inflammation, when untreated or incorrectly treated, may extend locoregionally, the remaining paranasal sinuses being the first affected anatomical structures. This is why the understanding of the inflammatory pathology of the maxillary sinus, and particularly of the complications it can generate, is extremely important. The purpose of this presentation is to point out that inflammations of the paranasal sinuses are susceptible to develop complications in certain conditions and threaten the patient's life due to the proximity of vital structures. This is the case of a 16 years

Disseminated nocardiosis in an advanced AIDS patient.

Nocardiosis is often misdiagnosed as tuberculosis in patients with HIV, as both diseases have similar manifestations. We describe the successful management of a case of advanced AIDS with disseminated Nocardial infection due to N. asteroides. Nocardial infection needs to be suspected in a patient with HIV infection when there is chest radiographic abnormality and when thrice sputum microscopy for acid fast bacilli is negative.

Cerebral abscess of odontogenic origin.

INTRODUCTION: Cerebral abscess is a rare but serious and life-threatening infection. Dental infections have occasionally been reported as the source of bacteria for such an abcess. PATIENT AND METHODS: A 54-year-old man was admitted with a right hemiparesis and epileptic fits. After clinical, laboratory and imaging examination, the diagnosis of a cerebral abscess of the left parietal lobe was made. The intraoral clinical examination as well as a panoramic radiograph confirmed the presence of generalized periodontal disease, multiple dental caries, and periapical pathology. The treatment inclu

Strategy for 3-D computed tomography diagnosis and treatment of small pulmonary arteriovenous fistula.

In a patient with left hemianopsia, computed tomography (CT) and magnetic resonance imaging revealed an intracranial space-occupying lesion in the right occipital region. A cerebral abscess was removed at craniotomy. Enhanced pulmonary CT showed a small coin lesion in the peripheral lingula. and at 3-D CT two nidi with feeding arteries and draining veins were seen, indicating pulmonary arteriovenous fistula. Lingular segmentectomy was performed.

[Otogenic intracranial complications: current assessment of some aspects of the problem].

Clinical cases and 245 case reports have been analyzed in terms of otogenic intracranial complications. Of late, there is a trend to lowering occurrence of otogenic complications and lethal outcomes compared to 1950-60. More favourable outcomes are attributed to introduction of new antibiotics and higher quality of ENT service.

Extradural dermoid tumours of the posterior fossa.

Dermoid tumours in children usually occur in two locations: at the anterior fontanelle and on the occipital squama. An exceptional site of origin for a posterior fossa dermoid cyst is the extradural space. There are only six previous cases of this situation reported in the literature. A series of 103 subscalp and calvarial masses in children were reviewed and three children are reported with extradural dermoids of the posterior fossa, which communicated with the skin through midline occipital dermal sinuses. All three children were seen after the rapid growth or the formation of an abscess in

Otogenic intracranial abscesses.

The commonest cause of the intracranial abscesses collected prospectively during the last two years was chronic middle ear infection (73%). The diagnosis was based on the clinical history, otological investigations, contrast enhanced computerized tomography and surgical findings. The clinical presentation was characterized by chronic otitis with an exacerbation of otorrhea, otalgia or pain in the temporal region or headache with high fever, vomiting and nausea. A review of our 14 patients with otogenic intracranial abscesses is reported to highlight that prompt diagnosis, appropriate therapy a

Miliary meningeal tuberculosis - an unusual imaging presentation for an early definitive diagnosis.

PURPOSE: Tuberculous meningitis (TBM) causes significant morbidity and mortality in young children. Early treatment can be initiated with magnetic resonance (MR) imaging diagnosis. We present MR-detectable miliary meningeal TB in two patients. CASE 1: A 9-year-old girl developed fevers, cough, lethargy, and seizures. Brain MRI demonstrated multiple, small, T2-dark, rim-enhancing lesions, associated with cranial nerve and leptomeningeal enhancement. CSF showed pleocytosis, low glucose, and high protein. Chest CT showed mediastinal lymphadenopathy, multiple small interstitial lung nodules, and a

Morphological Patterns of Intracranial Lesions in a Tertiary Care Hospital in North Karnataka: A Clinicopathological and Immunohistochemical Study.

INTRODUCTION: An "Intra-cranial space occupying lesion" (ICSOL)is defined as a mass lesion in the cranial cavity with a diverse aetiology like benign or malignant neoplasm, inflammatory or parasitic lesion, haematoma, or arterio-venous malformation. Central nervous system neoplasms represent a unique, heterogenous population of neoplasms constituting 1.9% of all malignant tumours in India. A broad spectrum of non-neoplastic conditions can mimic a brain tumour, both clinically and radiologically and these patients undergo biopsy. In such cases, the pathologist can readily differentiate between

Major complications from radiotherapy following treatment for atypical meningiomas.

OBJECTIVEComplications from radiotherapy (RT), in a primary or adjuvant setting, have overall been described as uncommon, with few detailed descriptions of major complications. The authors present two cases involving significant complications and their management in their review of patients undergoing RT for treatment of atypical meningioma.METHODSThe authors conducted a retrospective review of all patients with pathologically confirmed atypical meningioma (WHO grade II) treated with primary or adjuvant RT from February 2011 through February 2019. They identified two patients with long-term, g

Fatal Outcome of Widespread Cranial Basal Cell Carcinoma Due to Intraparenchymal Brain Extension in a Setting of Local Recurrences.

Basal cell carcinoma (BCC) is one of the most commonly encountered carcinomas; however, progression to invasive or locally advanced disease is extremely rare. The clinical course of locally advanced BCC is typically aggressive and has a poorer prognosis with variable survival. We present the case of a 58-year-old unhoused female with recurrent BCC of the left cranial vertex who underwent excision at an outside hospital. Intraoperative frozen section margins were negative for BCC; however, permanent sections demonstrated widespread residual tumor with morpheaform histology involving the parieta

Rodent Gut Bacteria Coexisting with an Insect Gut Virus in Tapeworm Parasitic Cysts: Metagenomic Evidence of Microbial Selection in Extra-Intestinal Clinical Niches.

In medicine, parasitic cysts (e.g., brain cysticerci) are believed to be sterile, and are primarily treated with antiparasitic medications, not antibiotics, which could prevent abscess formation and localized inflammation. This study quantified the microbial composition of parasitic cysts in a wild rodent, using multi-kingdom metagenomics to comprehensively assess if parasitic cysts are sterile, and further understand gut microbial translocation and adaptation in wildlife confined environments, outside the gut. Analysis was conducted on DNA from two hepatic parasitic cysts from a feline tapewo
