[The monorail system--bone segment transport over unreamed interlocking nails].

A treatment protocol is demonstrated, consisting of an osteotomy, either proximal or distal, of the bone defect with subsequent segmental transport via an anteromedially (tibia) or laterally (femur) mounted AO external fixation over an unreamed interlocking nail (monorail system). Twenty patients were treated by this method with indications as follows: 13 had a segmental bone defect of the tibia, 3 of the femur. Three patients showed post-traumatic and postinfectious leg-length discrepancies and one was treated for hypertrophic non-union of the femur. Defect distance varied between 5 and 18.5

Development of a radiation-free magnetic guidance device for distal screw placement in intramedullary nails.

This study developed a practical device for guiding distal interlocking screws during intramedullary nail fixation, aiming to address issues associated with the conventional free-hand approach using fluoroscopy. The objectives were to attain precision, reduce radiation exposure, and ease of operation. A small cylindrical magnet was embedded in the distal screw hole to emit signals for positioning. Due to the magnet's relatively weak magnetic force, especially in clinical settings with obese patients, the required long sensing distance is significant, making it susceptible to interference from

[Translated article] Biomechanical evaluation and analysis of a reused telescopic intramedullary femoral nail in a child with achondroplasia.

INTRODUCTION: Telescopic intramedullary nails (TIMN) have represented a significant advancement in limb lengthening procedures. However, their elongation capacity is limited to 5cm in the bones of patients with achondroplasia. Consequently, techniques involving TIMN reutilization have been developed. This reuse presents mechanical and safety challenges due to material fatigue and repetitive loading, which may compromise the structural integrity of the implant. This study evaluates the biomechanical performance and potential damage of a reused TIMN. METHODS: An experimental analysis was conduc

Reconstruction of Bony Defects With Motorized Intramedullary Nails After Tumor Resection.

Motorized intramedullary lengthening nails allow for transport of a bone segment for limb lengthening, deformity correction, healing of nonunion, and intercalary distraction osteogenesis. Resection of tumors involving the bone can result in substantial defects that require reconstruction. Use of these nails allows for a biologic reconstruction with the incorporation of allograft or by distraction osteogenesis. Limb lengthening after an internal hemipelvectomy where the hip joint is resected can be performed to improve gait, decrease pain, and prevent the need for a custom shoe or shoe lift. Us

Intramedullary fixation and bone grafting for congenital pseudarthrosis of the tibia.

Eleven consecutive patients with 12 congenital pseudarthroses of the tibia had intramedullary fixation and autogenous bone grafting as an initial treatment. None of these patients had previous surgeries or other concomitant procedures. The average age of the patients at the time of surgery was 2.5 years (range, 0.5-8.8 years). Four patients achieved initial union after the first index operation; two patients achieved union with no additional procedures, and three patients required an exchange intramedullary fixation for persistent nonunion after refractures that eventually healed. Seven patien

[Massive intraoperative lung embolism caused by the introduction of a telescope pin (Bailey-Dubow pin) in a child with osteogenesis imperfecta].

We report the case of a massive pulmonary embolism in a three year old boy with osteogenesis imperfecta during a closed insertion of a Bailey-Dubow-rod. An increased intramedullary pressure with consecutive entering of medullary particles or air into the blood stream due to the insertion of the Bailey-Dubow-rod is discussed as the cause of the pulmonary embolus.

Interventions for treating femoral shaft fractures in children and adolescents.

BACKGROUND: Fractures of the femoral shaft in children are relatively uncommon but serious injuries that disrupt the lives of children and their carers and can result in significant long-term disability. Treatment involves either surgical fixation, such as intramedullary nailing or external fixation, or conservative treatment involving prolonged immobilisation, often in hospital. OBJECTIVES: To assess the effects (benefits and harms) of interventions for treating femoral shaft fractures in children and adolescents. SEARCH METHODS: We searched the Cochrane Bone, Joint and Muscle Trauma Group

Characterization of human skeletal stem cells in closed and open tibia fractures: a single center pilot study.

INTRODUCTION: Tibial shaft fractures are the most common long bone fractures and carry a significant risk of nonunion, particularly in open injuries. This increased risk has been attributed to heightened activation of damage-associated molecular pathways, cellular senescence, and alterations in the fracture milieu. However, most supporting evidence arises from animal studies under controlled conditions, and the cellular and biochemical environment of human traumatic fractures remains poorly understood. While mechanical and patient-related factors have been associated with impaired healing, the

Long-term implant retention after impacted elastic stable intramedullary nailing in pediatric diaphyseal forearm fractures: a retrospective cohort study.

BACKGROUND AND PURPOSE: In elastic stable intramedullary nailing (ESIN) for forearm fractures, nail protrusion may cause irritation-related implant removal. The primary aim was to evaluate implant retention after nail impaction, and secondarily to assess secondary procedures, complications, and protrusion as a predictor of removal. METHODS: We analyzed a retrospective cohort of children with diaphyseal forearm fractures treated with impacted ESIN between 2017 and 2024. Implant retention was defined as no nail removal at record review. Secondary procedures were unplanned operations, while comp

Management of Open Ankle Fractures in Elderly Patients With a Fibula Nail is a Safe and Reliable Technique.

BACKGROUND: Ankle fractures are becoming more common in the elderly and their management is frequently challenging. There is increasing evidence of good outcomes following fibula nail fixation in the management of ankle fractures. The use of a fibula nail in open fractures comes with potential advantages such as minimal soft tissue dissection, early rehabilitation, and decreased wound complications. We aimed to assess their use in the management of open ankle fractures in the elderly. METHODS: A review of patients aged 60 years or older with an open ankle fracture treated with a fibular nail
