Intracranial abscesses secondary to nasal, sinus, and orbital infections in adults and children.

Nineteen cases of intracranial abscesses secondary to infection of the midface are reported. The most common underlying cause was bacterial sinusitis. Other etiologic factors included mucormycosis following steroid therapy, Wegener's granulomatosis, nasal dermoid cyst and sinus tract, tooth abscesses, aspergillosis following chemotherapy for leukemia, squamous cell carcinoma of the frontal sinus, infected methylmethacrylate plate for a prior skull fracture, and a case of gauze packing left in the sinus following surgery. Anaerobic organisms were the predominant cause of the abscesses. The most

[High-dosage cotrimoxazole therapy of disseminated Nocardia brasiliensis infection].

After a scraping injury to the skin a 47-year-old man developed a generalized Nocardia brasiliensis infection with cutaneous, pulmonary and cerebral dissemination. There was no predisposing underlying illness. High-dosage cotrimoxazol and short-term intrathecal and systemic gentamycin administration achieved extensive healing. This form of treatment would thus seem capable of reversing the otherwise poor prognosis of generalized N. brasiliensis infection.

Deaths in children during an outbreak of hand, foot and mouth disease in Peninsular Malaysia--clinical and pathological characteristics.

From July through December 1997, 11 previously healthy children in Peninsular Malaysia succumbed to an illness clinically characterised by an acute severe refractory left-ventricular failure, following a brief prodromal illness, in the midst of an outbreak of hand, foot and mouth disease (HFMD), similar to the reported experience in Sarawak and Taiwan. Retrospective reviews of the clinical features and results of laboratory, pathological and virological investigations of cases were conducted. The median age of the 11 case-patients was 31 months (range, 13 to 49 months); 6 were males. A brief p

A Case of Fulminant Infective Endocarditis in Mitral Valve Prolapse With Septic Emboli and an Annular Abscess.

Mitral valve prolapse (MVP) is associated with an increased risk of infective endocarditis (IE), particularly in patients with mitral regurgitation and leaflet thickening. Historically, a subset of patients with mitral valve prolapse were considered for antibiotic prophylaxis before dental procedures. Conversely, current recommendations limit prophylaxis to the highest-risk cardiac conditions, such as congenital heart defects, mechanical valves, heart transplant with valvular issues, and a history of IE. We present a case of a 59-year-old man with a history of MVP with mildly thickened posteri

Epidemiology of diagnostic errors in pediatric emergency departments using electronic triggers.

OBJECTIVES: We applied three electronic triggers to study frequency and contributory factors of missed opportunities for improving diagnosis (MOIDs) in pediatric emergency departments (EDs): return visits within 10 days resulting in admission (Trigger 1), care escalation within 24 h of ED presentation (Trigger 2), and death within 24 h of ED visit (Trigger 3). METHODS: We created an electronic query and reporting template for the triggers and applied them to electronic health record systems of five pediatric EDs for visits from 2019. Clinician reviewers manually screened identified charts and

Unravelling a pediatric enigma: coexisting retroesophageal right subclavian artery and congenital colonic stenosis masquerading as cow's milk protein allergy and ileus in a neonate.

BACKGROUND: Double alimentary tract obstruction due to congenital anomalies is a rare clinical occurrence, with limited cases published in m … llergy and ileus associated with sepsis. Tragically, the patient succumbed at seven months due to a brain abscess stemming from an infected external ventricular drain. Ultimately, postmortem examination unraveled double alimentary tract obstruction, consisting of RRSA and CCS. The RRSA, originating from proximal thoracic aorta, caused notable esophageal compres …

Listeria monocytogenes infection in multiple myeloma: A case report about a rare but lethal complication requiring heightened clinical vigilance.

RATIONALE: Multiple myeloma (MM), ranking as the second most prevalent hematologic malignancy. Despite prolonged survival in MM due to novel therapies, infections remain a leading cause of mortality. While bacterial pneumonias and viral reactivations are well-documented, Listeria monocytogenes (L monocytogenes) (a rare but aggressive pathogen) is underrecognized in MM. PATIENT CONCERNS: In this report, we report a 60-year-old male patient with newly diagnosed IgA-λ MM (Durie-Salmon stage IIIA, ISS stage III with 1q21 amplification) who developed high-grade fever (39.8 °C) and expressive aphas

The Interplay Between Neuroinfections, the Immune System and Neurological Disorders: A Focus on Africa.

Neurological disorders related to neuroinfections are highly prevalent in Sub-Saharan Africa (SSA), constituting a major cause of disability … ign primary headaches, some secondary headaches are caused by infections (meningitis, encephalitis, brain abscess). HIV and neurosyphilis, both common in SSA, can trigger long-standing immune activation in the central nervous system (CNS) potentially resulting in dementia. Despite the progress achieved in preventing diseases from the poliovirus and retroviruses …

Management of CVJ tuberculosis: the changing paradigm.

BACKGROUND: The literature seems fractured for the management of craniovertebral junction (CVJ)-tuberculosis (TB). Presently, non-surgical management has been in vogue for neurologically intact patients. On the contrary, severely disabled cases of CVJ-TB continue to attract discussion, tilted towards surgical intervention. We present our experience with the non-surgical management of CVJ-TB tailored to their neurological status. METHODS: Authors managed 37 cases (2004-2019; age 1-57 years, mean 36 years) of CVJ-TB, of which eighteen (18/37, 48.6%) were severely disabled (Nurick grade ≥ 3) wit

Comparison of CSF and MRI Findings among Neonates and Infants with E coli or Group B Streptococcal Meningitis.

BACKGROUND AND PURPOSE: Group B Streptococcus and Escherichia coli (E coli) are the 2 most common causes of bacterial meningitis in neonates. The purpose of this study was to determine whether CSF and/or MR imaging findings differ between infants with group B streptococcal or E coli meningitis. MATERIALS AND METHODS: A retrospective review was performed among neonates (younger than 28 days) and infants (younger than 120 days) with proved group B streptococcal (n = 57) or E coli meningitis (n = 50). A CSF or blood culture positive for Streptococcus or E coli and an elevated CSF white blood cel

The impact of Bacillus subtilis 18 isolated from Tibetan yaks on growth performance and gut microbial community in mice.

The purpose of this study was to evaluate the effects of Bacillus subtilis 18 (BS-18) isolated from free-ranging Tibetan yaks in high altitu … ncreased after BS-18 intake, which is associated with immune system activity. Acinetobacter induced brain abscess and bronchopneumonia were reduced in the experimental group. In conclusion, this study demonstrated that BS-18 isolated from Tibetan yaks was safe, beneficial and had the potential to serve as a probiotic.

Frame-based stereotactic neurosurgery in children under the age of seven: Freiburg University's experience from 99 consecutive cases.

INTRODUCTION: Stereotactic frame-based procedures proved to be precise, safe and are of widespread use among adult patients. Regarding pediatric patients few data is available, therefore the use of the stereotactic frame remains controversial in this population. This motivated us to report our experience in stereotactic procedures in the youngest patients and review the literature concerning this subject. METHODS: All frame-based procedures performed in patients younger than seven years in the University of Freiburg during the last 10 years were retrospectively analyzed and discussed under th

[Diagnostic value of (18)F-FDG PET/CT in extrapulmonary tuberculosis].

OBJECTIVE: To explore the value of (18)F-FDG PET/CT in the diagnosis of extrapulmonary tuberculosis (ET). METHODS: The imaging data from 88 cases of ET undergoing (18)F-FDG PET/CT examinations between August, 2005 and May, 2011 were analyzed retrospectively. RESULTS: All the 88 cases showed high (18)F-FDG uptake on PET imaging with a SUVmax ranging from 1.3 to 23.2. In 19 of the 88 cases, the extrapulmonary lesions were misdiagnosed as malignant neoplasms. Twenty-two patients were found to have phthisis. According to locations, the lesions were classified into 4 basic types, namely membranou
