Implant cut-out following cephalomedullary nailing of intertrochanteric femur fractures: Are helical blades to blame?

INTRODUCTION: Implant cut-out remains a common cause of cephalomedullary nail (CMN) failure and patient morbidity following surgical treatment of intertrochanteric femur fractures. Recent studies have suggested an increased rate of CMN cut-out with helical blades as opposed to lag screws. We compared rates of overall cut-out between helical blades and lag screws and used bivariate and multivariate analysis to determine the role of proximal fixation method among other variables on risk for cut-out. Subgroup analysis was performed on the basis of failure mechanism; superior migration (Fig. 2) ve

Management of knee rheumatoid arthritis and tibia nonunion with one-stage total knee arthroplasty and intramedullary nailing: A report of two cases.

Total knee arthroplasty (TKA) is a surgical procedure which is widely used in the treatment of gonarthrosis secondary to rheumatoid arthritis (RA). The incidence of stress fractures in tibia in the patients with RA is higher compared to normal patients. In this study, we report two cases of TKA and intramedullary nailing in RA patients with severe knee arthritis and tibial nonunion. Both patients had a satisfactory clinical outcome with radiological healing of the tibial fracture.

A technique for extraction of the distal segment of a broken femoral nail using a flexible reamer.

INTRODUCTION: Extraction of broken femoral nails in peri-implant fractures is becoming an increasingly common problem faced by orthopaedic surgeons. Different closed and open techniques for removal of broken nails have been previously described but due to variations in equipment and fracture configurations these methods are not always easily reproducible. We describe an open surgical technique using simple equipment that can be utilised when other methods of extraction have failed. CASE PRESENTATION AND SURGICAL TECHNIQUE: We present a case of a peri-implant fracture secondary to non-union in

Use of a Nancy nail to remove a broken intramedullary nail: A technical note.

Intramedullary (IM) nailing is a standard surgical technique for treating long bone diaphyseal fractures. However, one complication is breakage of the IM nail. Many methods have been reported for removing broken nails. We devised another technique, using a Nancy nail, for removing a broken IM nail and report on the surgical technique and a case involving the use of our method.

[Intramedullary nailing of periprosthetic femoral fractures after revision for total knee endoprosthesis. Treatment of periprosthetic femoral fractures by inserted knee endoprostheis with intramedullary shaft by in situ lengthening of the prosthesis with specially prepared slotted hollow nails].

BACKGROUND: With the increasing number of revision operations after knee replacement a growing incidence of periprosthetic femoral fractures which are difficult to treat is observed. MATERIAL AND METHODS: This retrospective study describes the operating procedure for osteosynthetic treatment of periprosthetic femoral fractures using a specially made slotted hollow nail which is engrafted with the purpose of in-situ lengthening of the prosthesis and thus becomes stably clamped. From 1999 to 2008 our patients have included 9 who were treated by this method. There were 5 male and 4 female patien

Comparison of RIA and conventional reamed nailing for treatment of femur shaft fractures.

INTRODUCTION: The standard of care for femoral diaphysis fractures is sequentially reamed, locked, intramedullary nails. However, in the polytraumatized patient perioperative complications such as fat embolism syndrome (FES) and acute respiratory distress (ARDS) are well chronicled. The reamer irrigator aspirator (RIA)has been theorized to minimize such phenomena. METHODS: A retrospective study comparing conventional reamed nailing for femur fractures versus those treated with the RIA was conducted. From January 2005 to September 2006, 156 patients treated at our institution with an intramedu

Isolated interfragmentary compression for nonunion of humeral shaft fractures initially treated by nailing: A preliminary report of seven cases.

INTRODUCTION: Plating with bone grafting is considered the gold standard treatment for nonunion of humeral shaft fractures. However, this complex procedure involves multiple risks. The aim of this study is to evaluate an alternative treatment using isolated axial interfragmentary compression for the dynamisation of humeral shaft nonunion after retrograde locked nailing. MATERIALS AND METHODS: Between January 2000 and May 2009, 124 humeral shaft fractures were treated in our trauma department with retrograde locked nailing using the unreamed humeral nail (UHN(®), Synthes, Paoli, PA, USA). Delay of clinical manifestation and medical malpractice in a serious vascular complication of proximal locked tibial nailing.

We are reporting on a case of complex lesion of the popliteal neurovascular bundle in a young woman, resulting from an operation for reduction and synthesis of a closed spiroid fracture of the mid-distal third of the left tibia by means of a Grosse-Kempf nail, with significant delay of clinical manifestation. Determining whether this is an "unavoidable complication" or "medical malpractice" is the target of the forensic investigation. In this case, the investigation leads to the conclusion that medical malpractice was the only explanation of the damage.

Subtrochanteric fracture managed by intramedullary nail in a patient with pycnodysostosis.

A rare case of a 54-year-old man with pycnodysostosis having subtrochanteric fracture of right femur is being reported. This patient had this fracture after a trivial fall while walking. He had typical facial features, short stature, open fontenella, short stubby hands, high arched furrowed palate and radiological features including open cranial sutures, obtuse angle of mandible, dysplasia of lower end of radius and ulna, notching of lumbar vertebra and generalized skeletal sclerosis. The fracture was treated with an intramedullary nail; the surgery was quite a tedious procedure where we faced

The treatment of impending and existing pathological femoral fractures using the long gamma nail.

The subtrochanteric region of the femur accounts for one-third of all pathological fractures requiring surgical intervention. The large forces occurring in this region and the possible non-occurrence of bony consolidation constitute a difficult problem to the trauma surgeon. The medical records and X-rays of 25 consecutive patients treated with the long gamma nail (LGN) for pathological (impending or existing) fractures of the femur were analysed retrospectively. Our results in the use of the LGN for pathological fractures of the femur have been good. We recommend an aggressive approach to the

Pitfalls, errors, and complications in the use of locking Küntscher nails.

The addition of percutaneously inserted transfixion screws or internally deployed fins supplements the stabilization potential of the cloverleaf nail. The use of these systems increases the indications for reamed closed intramedullary nailing. Fractures with comminution or bone loss, malunions, and nonunions located outside of the isthmic region of the femur and the tibia can now be stabilized effectively with a Küntscher-type implant. These closed nailing techniques reduce blood loss, surgical stress, and complications associated with open reduction and offer the patient the advantages of ear

A Randomized Trial Comparing Outcome of Expert Tibial Nail versus Proximal Tibial Plate in the Management of Fractures of Proximal Tibia in Adults.

BACKGROUND: Extra-articular fracture of the proximal tibia frequently results in functional impairment of the knee and lower limb if not managed properly. Any treatment method must achieve the desired anatomical reduction and stable fixation to permit early weight-bearing and joint motion. This type of fracture can be treated using a variety of methods, from conservative casting to surgical management. It is still for debate that, which surgical procedure is best for treating extra-articular proximal tibia fractures. AIM: This study aims to compare the functional and radiological outcome of e

Internal fixation of nonunions.

We review the two major types of internal fixation of nonunions (plating and intramedullary nailing), and analyze nonunions in different locations (upper and lower extremities). Depending on the type and the location of the nonunion, plating or intramedullary nailing may be selected. Both have advantages and disadvantages: plating requires opening the nonunion site, which entails some damage to the soft tissues, and carries with it a risk of secondary infection. With plating, it often is impossible to do a real decortication because the periosteum may be thin and poorly adherent to bone, and t

Acute intra-articular calcaneal fractures: currents concepts.

Calcaneal fractures are predominantly intra-articular and most commonly involve the subtalar joint, with an estimated incidence of 11,5 per 100,000 population and a male predominance. They occur mainly after high-energy trauma and are associated with substantial functional impairment and major socio-occupational consequences. Anatomical assessment (loss of height, posterior facet collapse, lateral wall widening, and varus displacement with superior migration of the tuberosity) determines prognosis and guides management. Plain radiographs (Böhler's and Gissane's angles) remain useful, but compu

Intramedullary fixation with bioabsorbable and osteoconductive hydroxyapatite/poly-L-lactide threaded pin in digital replantation.

In digital replantation, Kirschner wire (K-wire) fixation has commonly been used for osteosynthesis. On the other hand, K-wires are often obtrusive because of protrusion from the replanted digit. We describe a case series treated using hydroxyapatite/poly-L-lactide (HPLLA) threaded pins, which are not only bioabsorbable, but also osteoconductive, for osteosynthesis in crushed amputation or comminuted fracture, including distal phalanx amputation. Using an HPLLA threaded pin, 10 digital replantations were performed between July 2016 and April 2018. The precisely cut pin is manually pushed into
