First-bite syndrome: a review of 29 patients with parapharyngeal space tumor.

OBJECTIVE: The aim of this study is to clarify the etiology of first-bite syndrome. METHODS: A total of 29 consecutive patients treated with a surgical resection for parapharyngeal space tumor between April 1999 and April 2005 were retrospectively reviewed. RESULTS: Nine cases of first-bite syndrome were developed. Eight of those patients had undergone surgical ablation of the cervical sympathetic chain and/or external carotid artery. In two patients who underwent ablation of the sympathetic chain but in whom first-bite syndrome did not develop, the superior cervical ganglion was probably pr

[On the diagnosis and treatment of parotid gland tumors. Results of a nationwide survey of ENT hospitals in Germany].

BACKGROUND: It seems that the choice of the treatment modalities for parotid gland tumors frequently depends on personal experience more than on evidence-based criteria. A nationwide survey was conducted to obtain an overview of current practice in German ENT hospitals. METHODS: A standardised questionnaire comprising 19 questions on the treatment and diagnostic methods for parotid gland tumors was sent to all ENT hospitals in Germany. RESULTS: The overall return rate of the questionnaires was 128 of 170 (75%). The results confirmed highly variable strategies for the treatment of parotid gla

Xerostomia and its predictors following parotid-sparing irradiation of head-and-neck cancer.

PURPOSE: To assess long-term xerostomia in patients receiving parotid-sparing radiation therapy (RT) for head-and-neck cancer, and to find the patient and therapy-related factors that affect its severity. PATIENTS AND METHODS: From March 1994 through January 2000, 84 patients received comprehensive bilateral neck RT using conformal and multisegmental intensity-modulated RT (IMRT) aiming to spare the major salivary glands. Before RT and periodically through 2 years after the completion of RT, salivary flow rates from each of the major salivary glands were selectively measured. At the same time

Facial nerve paralysis. 2. 'All that palsies is not Bell's'.

Management of the patient with facial nerve paralysis challenges the diagnostic skills and clinical judgment of the attending physician. The clinician should not assume that all facial paralysis is Bell's palsy; the disease possibilities are broad and fascinating. Therapeutic options are still being evaluated, debated, and assessed. Appropriate management of the patient with facial nerve paralysis requires a complete history and precise examination.

[Squamous cell carcinomas of the external auditory canal, treatment of lymph node areas: A multicenter study].

INTRODUCTION: Squamous cell carcinomas of the external auditory canal are rare, with limited data available in the literature on patient survival in Europe. The low incidence of this condition complicates the development of a standard surgical management procedure, particularly given the lack of consensus regarding the value of neck dissection. The aim of this study was to evaluate the benefit of parotidectomy and neck dissection combined with tumor resection, and to assess the survival outcomes of patients treated for squamous cell carcinoma of the external auditory canal. METHODS: Data on p

The Clinical Significance of Incidental Parotid Uptake in a PET/CT Study: A Diagnostic Algorithm.

BACKGROUND: Patients diagnosed with cancer do not have sufficient clinical data for the management of incidental parotid lesions. We aimed to reveal the importance of randomized parotid lesions encountered during oncologic F-18 fluorodeoxyglucose positron emission tomography (FDG PET/CT) imaging in our clinical practice and the diagnostic algorithm of such lesions. METHODS: We performed a database search of PET/CT records generated from 2009 to 2015 for "parotid" in reports of patients who underwent PET/CT examination for a known malignancy elsewhere, or cancer screening. RESULTS: Incidental

Tumor-induced Osteomalacia Caused by a Parotid Tumor.

A 77-year-old man was suspected of having tumor-induced osteomalacia (TIO) because of hypophosphatemia (1.9 mg/dL) and elevated serum fibroblast growth factor 23 (FGF23) level (186.9 pg/mL). We detected a tumor in his left parotid gland, and the FGF23 level in the left external jugular vein indicated that the tumor overproduced FGF23. After the removal of the tumor, the serum FGF23 level rapidly decreased, and the serum phosphate normalized. This is the first case of TIO caused by a tumor in a parotid gland. This case indicates that the responsible tumors for TIO can be quite diverse.

Giant cell tumour of a temporomandibular joint presenting as a parotid mass: case report and analysis of the 19 cases in the literature.

Giant cell tumour (GCT) in the parotid region is a rare lesion whose origin can be divided into three compartments: giant cell tumour of the salivary gland (GCT-SG), giant cell tumour of the bone (GCT-bone) and giant cell tumour of the soft tissue (GCT-ST). A low risk of malignancy has been observed, in which all of them were GCT-SG. We present a case of GCT in the parotid region and review the features of GCT in origin, immunohistochemical characteristics, treatment of choice and disease outcome.

Warthin tumor with signet-ring cell features as a pitfall in salivary gland cytopathology.

BACKGROUND: Warthin tumor (WT) is a common parotid lesion reliably diagnosed by fine-needle aspiration (FNA). Worrisome metaplastic changes may occur in WT. Their interpretation as mucoepidermoid carcinoma represents a diagnostic pitfall. Moreover, WT and mucoepidermoid carcinoma may coexist, making this distinction difficult. So it is worthwhile to report unusual WT features. We describe a WT with signet-ring cells (SRCs). CASE: A 61-year-old male presenting with a 3.6-cm right parotid gland mass underwent FNA with rapid on-site evaluation. DiffQuik-stained smears showed groups of oncocytic

Where Birt-Hogg-Dubé meets Cowden syndrome: mirrored genetic defects in two cases of syndromic oncocytic tumours.

Birt-Hogg-Dubè (BHD) is an autosomal dominant syndrome characterised by skin fibrofolliculomas, lung cysts, spontaneous pneumothorax and renal cancer. The association of benign cutaneous lesions and increased cancer risk is also a feature of Cowden Syndrome (CS), an autosomal dominant disease caused by PTEN mutations. BHD and CS patients may develop oncocytomas, rare neoplasias that are phenotypically characterised by a prominent mitochondrial hyperplasia. We here describe the genetic analysis of a parotid and a thyroid oncocytoma, developed by a BHD and a CS patient, respectively. The BHD les

Reducing xerostomia after chemo-IMRT for head-and-neck cancer: beyond sparing the parotid glands.

PURPOSE: To assess whether, in addition to sparing the parotid glands (PGs), xerostomia after chemotherapy plus intensity-modulated radiotherapy (chemo-IMRT) for head-and-neck cancer is affected by reducing the dose to the other salivary glands. PATIENTS AND METHODS: In a prospective study, 78 patients with Stage III-IV oropharynx/nasopharynx cancer underwent chemo-IMRT, with the aim of sparing the parts of the bilateral PGs, oral cavity (OC) containing the minor salivary glands, and contralateral submandibular gland (SMG) outside the target (when contralateral level I was not a target). Tolerances on MLC leaf position accuracy for IMRT delivery with a dynamic MLC.

The objective determination of performance standards for radiation therapy equipment requires, ideally, establishing the quantitative relationship between performance deviations and clinical outcome or some acceptable surrogate. In this simulation study the authors analyzed the dosimetric impact of random (leaf by leaf) and systematic (entire leaf bank) errors in the position of the MLC leaves on seven clinical prostate and seven clinical head and neck IMRT plans delivered using a dynamic MLC. In-house software was developed to incorporate normally distributed errors of up to +/- 2 mm in indiv

Management and prognostic factors of recurrent pleomorphic adenoma of the parotid gland: personal experience and review of the literature.

The aim of this study was to investigate the management and prognostic determinants of recurrent pleomorphic adenoma (RPA). A retrospective analysis was performed to examine the clinical features, the prevalence of surgical complications, and new recurrences of RPA. Tumor recurrence rate was estimated by the Kaplan-Meier method, and the prognostic value of some of the variables was tested by univariate analysis using the log rank test. The study focused on 33 patients, 18 female (54.5%) and 15 male (45.5%), aged 12-71 years (median 41). A total or extended total parotidectomy was performed in

[Results of sialography (author's transl)].

A randomly selected group of patients which had been subjected to sialography during 1975/76, was analysed retrospectively. In a total of 386 patients, 395 contrast radiographs had been performed; of these, 243 sialographs were evaluated retrospectively, the definite diagnosis having been established by surgery, biopsy or clinical course of the disease. The accuracy of diagnosis was 95% when employing diagnosis by exclusion (sialographic diagnosis having yielded "normal findings"), the correct diagnosis being the presence of concrements and inflammatory diseases of the salivary gland. However,

A deep learning model for predicting radiation-induced xerostomia in patients with head and neck cancer based on multi-channel fusion.

OBJECTIVES: Radiation-induced xerostomia is a common sequela in patients who undergo head and neck radiation therapy. This study aims to develop a three-dimensional deep learning model to predict xerostomia by fusing data from the gross tumor volume primary (GTVp) channel and parotid glands (PGs) channel. METHODS: Retrospective data were collected from 180 head and neck cancer patients. Xerostomia was defined as xerostomia of grade ≥ 2 occurring in the 6th month of radiation therapy. The dataset was split into 137 cases (58.4% xerostomia, 41.6% non-xerostomia) for training and 43 (55.8% xeros

Analysis of the Parotid Glands on an Energy Spectrum CT Iodine Map to Evaluate Irradiation-Induced Acute Xerostomia in Patients With Nasopharyngeal Carcinoma.

Objective: This prospective study aims to evaluate acute irradiation-induced xerostomia during radiotherapy by utilizing the normalized iodine concentration (NIC) derived from energy spectrum computed tomography (CT) iodine maps. Methods: In this prospective study, we evaluated 28 patients diagnosed with nasopharyngeal carcinoma. At 4 distinct stages of radiotherapy (0, 10, 20, and 30 fractions), each patient underwent CT scans to generate iodine maps. The NIC of both the left and right parotid glands was obtained, with the NIC at the 0-fraction stage serving as the baseline measurement. After
