The test-retest reliability of the Form 90-DWI: an instrument for assessing intoxicated driving.

Although driving while intoxicated (DWI) is a pervasive problem, reliable measures of this behavior have been elusive. In the present study, the Form 90, a widely utilized alcohol and substance use instrument, was adapted for measurement of DWI and related behaviors. Levels of reliability for the adapted instrument, the Form 90-DWI, were tested among a university sample of 60 undergraduate students who had consumed alcohol during the past 90 days. The authors administered the instrument once during an intake interview and again, 7-30 days later, to determine levels of test-retest reliability.

Unintentional injuries in a twin study of preschool children: environmental, not genetic, risk factors.

OBJECTIVE: To analyze the relative contribution of latent genetic and environmental factors to differences in the injury liability of childr … injury variance in this sample. However, behavioral variables, especially the child's externalizing problem behaviors, are also important in explaining unintentional injuries.

Characteristics of long-term asymptomatic infection with human immunodeficiency virus type 1 in men with normal and low CD4+ cell counts.

From a cohort study of homosexual men in Amsterdam, 61 human immunodeficiency virus (HIV)-infected men who had remained asymptomatic for at least 7 years were identified. In a nested case control study, these men were compared with 142 men who progressed symptomatic HIV infection (CDC class IV) within 7 years, regarding laboratory markers, sexual behavior, psychologic coping, and drug use. Of the 61 long-term asymptomatic men, 13 had a CD4+ cell count > or = 500/mm3 after 7 years; in 2 of these 13, the CD4+ cell count had not declined during follow-up. Independent of CD4+ cell count, long-term

An overview of prevention research: issues, answers, and new agendas.

Efforts to curtail alcohol abuse and alcoholism can be divided into primary, secondary, and tertiary prevention. Primary prevention attempts to stop a problem or illness from occurring in the first place. Secondary prevention identifies persons in the early stages of problematic or illness behavior and refers them for counseling or treatment, which is considered tertiary prevention. Five research areas concerned with primary and secondary prevention are selected for discussion: youth, the mass media, the worksite, blacks and Hispanics, and alcohol-related behavior that increases the risk of AI

Approach to the patient with acquired hypothalamic syndrome.

The hypothalamus integrates signals from the body and the environment and coordinates homeostatic responses in multiple physiologic processes, including water balance, stress response, temperature regulation, energy balance, feeding behavior, sleep-wake cycles, and reproduction. Endocrinologists care for individuals for whom hypothalamic injury, whether from tumors, their treatment, or other causes, impairs pituitary hormone production and secretion. The extent of other symptoms in affected individuals is also influenced the extent of hypothalamic injury. "Acquired hypothalamic syndrome" refer

The role of psychology and neuropsychology in pediatric epilepsy surgery evaluation.

Epilepsy is one of the most common neurological problems affecting 470,000 children in the United States. While most youth will achieve seizure freedom using medication, up to a third will continue to have seizures and are therefore considered to have drug-resistant epilepsy (DRE). Children and adolescents with epilepsy are at higher risk of behavioral, cognitive, and emotional disorders. Youth with DRE are at even greater risk of behavioral and emotional problems impacting quality of life and may need to pursue surgical interventions, including resective surgery or device implantation. Due to

Fetal Rhabdomyoma Leads to Family Diagnosis of Tuberous Sclerosis Complex.

Tuberous sclerosis complex (TSC) is a multisystem genetic disorder characterized by the growth of numerous noncancerous tumors in many parts of the body. It is highly variable in clinical presentations, including a wide range of cognitive, behavioral, and psychiatric manifestations. Of all the possible manifestations, cognitive and behavioral problems are the greatest concern to parents and physicians. In this study, two fetuses were identified to have rhabdomyomas by prenatal ultrasound. Carefully inquired familial medical history revealed other symptoms of TSC such as skin lesions or psychia

College student beliefs about wagering: an evaluation of the adolescent gambling expectancies survey.

Expectancy theory posits that decisions to engage in a given behavior are closely tied to expectations of the outcome of that behavior. Gambling outcome expectancies have predicted adolescent gambling and gambling problems. When high school students' outcome expectancies were measured by Wickwire et al. (Psychol Addict Behav 24(1):75-88 2010), the Adolescent Gambling Expectancy Survey (AGES) revealed five categories of expectancies that were each predictive of gambling frequency and pathology. The present study aimed to explore if the AGES could be successfully replicated with college students

Factor structure and validity of the parenting scale.

Confirmatory factor analyses based on the scoring derived from 5 prior studies of the Parenting Scale were conducted using a representatively recruited sample of 453 couples parenting 3-to 7-year-old children. Comparative analyses favored the Reitman et al. (2001) 2-factor scoring system as well as a 3-factor solution, including Lax, Overreactive, and Hostile discipline. This 3-factor solution demonstrated good fit across parent gender and child age and gender. Mothers rated themselves as more overreactive than fathers. The factor scores correlated significantly with several validity measures,

Maternal depression and child internalizing: the moderating role of child emotion regulation.

This study tests a model of children's emotion regulation (ER) as a moderator of the link between maternal depression and child internalizing problems. Participants were 78 children (ages 4 to 7), including 45 children of mothers with a history of childhood-onset depression (COD) and 33 children of mothers who had never been depressed. ER was assessed observationally during a laboratory mood induction. ER behaviors were empirically reduced into 3 categories: (a) negative focus on delay, (b) positive reward anticipation, and (c) behavioral distraction. Linear mixed models indicated that positiv

Latina mothers' stances on stimulant medication: complexity, conflict, and compromise.

This study was undertaken to understand and describe Latina mothers' cognitions and attitudes toward the use of medication for their young children's behavior problems under the premise that these factors are determinants of noncompliance and inadequate adherence to medication treatments. Quantitative and qualitative data were extracted from a multimethod study of professional help seeking. Participants were 62 mothers from Cuba, Puerto Rico, and the Dominican Republic. Findings indicated that mothers overwhelmingly preferred treatment options other than medication primarily because they under

An overview of urinary incontinence in adults: assessments and behavioral interventions.

Urinary incontinence affects millions of Americans. Often the goal of treatment is to improve the condition, prevent complications and provide comfort. Behavioral interventions can improve the condition in 54-75% of patients with urge and/or stress incontinence and can cure 12-16% of patients. Advanced practice nurses (APNs) are in a unique position of both providing direct care to patients who experience these problems and educating other nurses about signs, symptoms and appropriate nursing interventions for urinary incontinence. The scope of the problem, costs of urinary incontinence and pot

Management of early postdischarge adjustment reactions following psychiatric hospitalization.

Psychiatric patients frequently experience serious symptoms and demonstrate disturbed behaviors in the very early postdischarge period. Based on 25 years of clinical experience, the author reviews symptoms and behaviors that can occur and notes that they should most often be viewed as adjustment reactions rather than as exacerbations of the primary illness. A team approach to management of early postdischarge reactions that uses a psychiatrist and psychiatric nurse is effective. Interventions include forewarning inpatients that problems may occur and helping them identify potential problems. S

Management of childhood sexual abuse.

Childhood sexual abuse is increasingly recognized as a major problem in the United States. These children are frequently seen by the primary care provider, and physicians must be able to evaluate them. Common presentations for sexually abused children include direct allegations, suspicions by the caretaker, unexplained vaginal trauma or bleeding, sexually transmitted disease, and a wide spectrum of behavioral problems including sexual promiscuity and runaway behavior. The interview and physical examination must be performed in a quiet, private area and sufficient time allocated to allow a thor

Intimate partner violence: childhood exposure to domestic violence.

Children who are exposed to domestic violence (DV) may experience many short- and long-term negative effects. They are up to 3.8 times more likely to become perpetrators or victims in adulthood than are children not exposed to DV. They also are at high risk of health problems, risky health behaviors, violence, and social functioning problems. Girls who witness intimate partner violence (IPV) are more likely to experience depression, anxiety, and trauma symptoms, and boys exposed to IPV are more likely to exhibit aggression and delinquent behaviors. To prepare the practice to identify and assis

Behavioral parent training as an adjunct to routine care in children with attention-deficit/hyperactivity disorder: moderators of treatment response.

OBJECTIVE: To investigate predictors and moderators of outcome of behavioral parent training (BPT) as adjunct to ongoing routine clinical care (RCC), versus RCC alone. METHODS: We randomly assigned 94 referred children (4-12 years) with attention-deficit/hyperactivity disorder (ADHD) to BPT plus RCC or RCC alone. Outcome was based on parent-reported behavioral problems and ADHD symptoms. Predictor/moderator variables included children's IQ, age, and comorbidity profile, and maternal ADHD, depression, and parenting self-efficacy. RESULTS: Superior BPT treatment effects on behavioral problems

A partial loss of function allele of methyl-CpG-binding protein 2 predicts a human neurodevelopmental syndrome.

Rett Syndrome, an X-linked dominant neurodevelopmental disorder characterized by regression of language and hand use, is primarily caused by mutations in methyl-CpG-binding protein 2 (MECP2). Loss of function mutations in MECP2 are also found in other neurodevelopmental disorders such as autism, Angelman-like syndrome and non-specific mental retardation. Furthermore, duplication of the MECP2 genomic region results in mental retardation with speech and social problems. The common features of human neurodevelopmental disorders caused by the loss or increase of MeCP2 function suggest that even mo

Treating children with early-onset conduct problems: intervention outcomes for parent, child, and teacher training.

Families of 159, 4- to 8-year-old children with oppositional defiant disorder (ODD) were randomly assigned to parent training (PT); parent plus teacher training (PT + TT); child training (CT); child plus teacher training (CT + TT); parent, child, plus teacher training (PT + CT + TT); or a waiting list control. Reports and independent observations were collected at home and school. Following the 6-month intervention, all treatments resulted in significantly fewer conduct problems with mothers, teachers, and peers compared to controls. Children's negative behavior with fathers was lower in the 3

Behavioral momentum in computer-presented discriminations in individuals with severe mental retardation.

Behavioral momentum was examined in 2 individuals with severe mental retardation via within-subject manipulations of obtained reinforcer rates. Subjects performed self-paced discrimination problems presented on a touch screen computer monitor. Two different problems, Tasks A and B, alternated in blocks of 15 trials on a multiple schedule. Reinforcers were snack foods. The reinforcement schedule for Task A was continuous (fixed-ratio 1) and the schedule for Task B was continuous in some conditions and variable ratio in other conditions. Behavioral momentum was assessed in test sessions by prefe
