The auditory brain stem implant has been used effectively to provide hearing sensations to individuals deafened by bilateral auditory nerve tumors (neurofibromatosis type 2). During tumor removal, the auditory brain stem implant is implanted into the lateral recess of the fourth ventricle by a translabyrinthine approach and is intended to stimulate auditory neurons of the cochlear nucleus complex. A new eight-electrode multichannel auditory brain stem implant was developed and evaluated in 20 patients who had at least 3 months' experience with the device. Mild nonauditory sensations (primarily

The auditory brainstem implant (ABI) can provide hearing sensation to individuals where the auditory nerve is damaged. However, patient outcomes with the ABI are typically much poorer than those for cochlear implant recipients. A major limitation to ABI outcomes is the number of implanted electrodes that can produce auditory responses to electric stimulation. One of the greatest challenges in ABI surgery is the intraoperative positioning of the electrode paddle, which must fit snugly within the cochlear nucleus complex. While there presently is no optimal procedure for intraoperative electrode

As with other cranial nerves and many CNS neurons, primary auditory neurons degenerate as a consequence of loss of input from their target cells, the inner hair cells (IHCs). Electrical stimulation (ES) of spiral ganglion cells (SGCs) has been shown to enhance their survival. Glial cell line-derived neurotrophic factor (GDNF) has also been shown to increase survival of SGCs following IHC loss. In this study, the combined effects of the GDNF transgene delivered by adenoviral vectors (Ad-GDNF) and ES were tested on SGCs after first eliminating the IHCs. Animal groups received Ad-GDNF or ES or bo

There are no approved Food and Drug Administration indications for pediatric auditory brainstem implant (ABI) surgery in the United States. Our prospective case series aims to determine the safety and feasibility of ABI surgery in pediatric patients <5 years old with congenital deafness at a tertiary North American center. The inclusion criterion was pre- or postlinguistic deafness in children not eligible for cochlear implantation. Seventeen candidates were evaluated (mean ± SD: age, 2.52 ± 0.39 years). Four patients underwent ABI surgery (age, 19.2 ± 3.43 months), including 4 primary procedu…

In the developed world, there are currently more than 100 million people afflicted with hearing loss. In the United States and European Community alone, there are an estimated 21 million people with significant conductive hearing loss, whilst there are over 90 million suffering from moderate to severe sensorineural hearing loss. Of these, more than 65 million hearing-impaired persons are without treatment. This article presents a review of the latest technological advances and treatment options for the hearing-impaired, including external and middle ear devices, bone-anchored hearing aids, coc

HYPOTHESIS: In bone conduction implantation, the position of the implant influences the audiological benefit of the patient.
BACKGROUND: One way of treating hearing loss is the implantation of bone conduction implants (BCIs), which effectively transmit vibrations through the skull bone to the cochlea given that the implant transducer is securely fixated. Laboratory research on the efficacy of bone conduction sound transmission found that a closer proximity of the transducer to the ipsilateral cochlea yields significantly higher cochlear promontory vibrations and hence, higher stimulation effi

This study models induced electric fields, and their gradient, produced by pulsatile current stimulation of submillimeter inductors for cochlear implantation. Using finite-element analysis, the lower chamber of the cochlea, scala tympani, is modeled as a cylindrical structure filled with perilymph bounded by tissue, bone, and cochlear neural elements. Single inductors as well as an array of inductors are modeled. The coil strength (~100 nH) and excitation parameters (peak current of 1-5 A, voltages of 16-20 V) are based on a formative feasibility study conducted by our group. In that study, in…

Patients being considered for cochlear implantation undergo, of necessity, a wide-ranging selection process, of which imaging of the temporal bone to assess cochlear patency is an important part. We have studied the correlation between the results of computed tomography (CT) imaging of the cochlea and the cause of hearing loss in this group of patients. The CT scans of 105 patients were reviewed. Of these, 74 (70%) were normal, with the commonest abnormalities being labyrinthitis ossificans in 14 (13.3%) cases and capsular rarefaction in 10 (9.5%) cases. A congenital abnormality was seen in 4 …

The identification of congenital hearing loss using Transient Evoked Otoacoustic Emissions and Automatic Auditory Brainstem Response in a newborn hearing screening program is crucial for initiating early rehabilitation with hearing aids or cochlear implants. Specific genetic causes, such as Pendred syndrome, connexin-26, stereocilin, and otoferlin-associated deafness, can be identified today using gene panels. Specifically, for otoferlin-associated deafness, it may be possible to offer gene therapy as a novel treatment for this specific genetic type of hearing loss.

HYPOTHESIS/BACKGROUND: Bast's valve is a poorly understood inner ear structure located at the junction between pars superior and inferior in the membranous labyrinth. Anatomically precise three-dimensional reconstructions (3D-reconstructions) of Bast's valve can help illuminate the morphology of the valve, and point toward its role in normal physiology and pathological states such as endolymphatic hydrops. This is of particular relevance to the development of a vestibular implant, a device intended to rehabilitate deficits in the vestibular system.

Animals are frequently used for the development and testing of new hearing devices. Dimensions of the middle ear and cochlea differ significantly between humans and commonly used animals, such as rodents or cats. The sheep cochlea is anatomically more like the human cochlea in size and number of turns. This study investigated the middle-ear ossicular velocities and intracochlear sound pressure (ICSP) in sheep temporal bones, with the aim of characterizing the sheep as an experimental model for implantable hearing devices. Measurements were made on fresh sheep temporal bones. Velocity responses
