S100A8 and S100A9 Are Important for Postnatal Development of Gut Microbiota and Immune System in Mice and Infants.

BACKGROUND & AIMS: After birth, the immune system matures via interactions with microbes in the gut. The S100 calcium binding proteins S100A8 and S100A9, and their extracellular complex form, S100A8-A9, are found in high amounts in human breast milk. We studied levels of S100A8-A9 in fecal samples (also called fecal calprotectin) from newborns and during infancy, and their effects on development of the intestinal microbiota and mucosal immune system. METHODS: We collected stool samples (n = 517) from full-term (n = 72) and preterm infants (n = 49) at different timepoints over the first year o

Small for gestational age and extremely low birth weight infant outcomes.

Background Small for gestational age (SGA) infants are less likely to develop respiratory distress syndrome (RDS), but more likely to develop bronchopulmonary dysplasia (BPD) and have a higher mortality. Our aim was to focus on outcomes of those with a birth weight less than or equal to 750 g. Methods The mortality, BPD severity, necrotising enterocolitis (NEC), home oxygen requirement and length of hospital stay were determined according to SGA status of all eligible infants in a 5-year period admitted within the first 24 h after birth. Results The outcomes of 84 infants were assessed, and 35

Survival and early complications of preterm infants with birthweight less than 500 grams during a 10-year period in Hungary.

BACKGROUND: There are limited data available on the survival and early complications of preterm infants with less than 500 g birthweight. To estimate the outcomes for these infants, it is important for caregivers to be aware of perinatal factors that may affect survival. OBJECTIVES: We assessed the mortality and certain early complications of preterm infants born with less than 500 g in Hungary between 2006 and 2015. METHODS: We reviewed data of 486 infants from the database of the Hungarian Central Statistical Office and in parallel of 407 infants from the "NICU database." The study period

Management of the Preterm Infant with Congenital Heart Disease.

The premature neonate with congenital heart disease (CHD) represents a challenging population for clinicians and researchers. The interaction between prematurity and CHD is poorly understood; epidemiologic study suggests that premature newborns are more likely to have CHD and that fetuses with CHD are more likely to be born premature. Understanding the key physiologic features of this special patient population is paramount. Clinicians have debated optimal timing for referral for cardiac surgery, and management in the postoperative period has rapidly advanced. This article summarizes the key c

Impact of umbilical catheterization on morbidity and mortality in extremely premature newborns.

OBJECTIVE: Investigate the benefit of umbilical catheterization upon survival and selected morbidities in extremely premature newborns (<28 weeks gestation). Outcomes of successfully catheterized extremely premature newborns are compared with others who cannot be successfully catheterized, and we hypothesize that umbilical catheterization promotes survival and reduces morbidities. STUDY DESIGN: Utilizing a retrospective, cohort study design, survival and outcomes of catheterized and non-catheterized newborns (n = 722) were compared by univariate and multiple logistic regression analyses. Complications of the late preterm infant.

One of the goals of Healthy People 2010 (set in 1998) was to reduce preterm birthrates from 11.6% to 7.6%. However, in 2004, the preterm birthrate of 12.5% was actually higher than the rate in 1998. Approximately 65% of this increase in prematurity rate is attributed to the increasing birthrate of the late preterm infant. Care of the late preterm infant is far more complicated than many hospital policies and clinical guidelines imply. It cannot be stressed enough to frontline clinicians that late preterm infants are not full-term infants. Their care should not be defined by the same policies a

Attendance in a Neonatal Follow-Up Program before and in the Time of COVID-19 Pandemic: A Mixed Prospective-Retrospective Observational Study.

BACKGROUND: Attendance to neonatal follow-up programs presents a significant factor associated with positive long-term outcomes of high-risk infants. Strategies to maximize participation benefit not only future interventions' effectiveness but also healthcare systems and society. While a number of studies have focused on attrition or loss to follow-up, no studies have focused on the contributive risk factors to abstaining from neonatal follow-up programs specifically during the COVID-19 pandemic. This study aims to reveal the main factors linked to non-compliance in a neonatal follow-up progra

Surfactant replacement therapy with a single postventilatory dose of a reconstituted bovine surfactant in preterm neonates with respiratory distress syndrome: final analysis of a multicenter, double-blind, randomized trial and comparison with similar trials. The Surfactant-TA Study Group.

The effects of a single dose of surfactant TA were assessed in premature neonates (birth weight 750 to 1749 g) with respiratory distress syndrome (RDS) in a multicenter, double-blind, randomized clinical trial. Only neonates with surfactant deficiency and without ultrasonographic evidence of intracranial hemorrhage greater than or equal to grade II were enrolled. Fifty-four patients received surfactant (100 mg of phospholipid per kilogram of body weight) and 46 patients received an air placebo within 8 hours of life. Treatment with this surfactant resulted in a significant reduction in the sev

Early Oral Melatonin Supplementation Supports White Matter Maturation in Preterm Infants: Neuroprotective Insights of an Antioxidant Molecule.

Preterm birth is associated with increased oxidative stress and brain dysmaturation, contributing to adverse neurodevelopmental outcomes. Preterm infants are physiologically unable to produce melatonin during early postnatal life, potentially increasing their vulnerability to oxidative injury. We planned and conducted a prospective, multicentre, randomized, double-blind, placebo-controlled study to investigate whether 15 days of early oral melatonin (ME) supplementation after birth increases ME levels and reduces oxidative stress in preterm newborns at the end of administration period. The lev

A novel double-lumen tracheal tube-assisted InSurE versus LISA in preterm infants with RDS: a multicenter randomized trial.

BACKGROUND: Neonatal respiratory distress syndrome (NRDS) is a leading cause of mortality and long-term morbidity in preterm infants; however, safe and effective surfactant delivery to the neonatal lung remains challenging. We developed a novel dual-lumen tracheal tube (NDT) enabling surfactant administration without interrupting ventilation. This study aimed to evaluate the non-inferiority of the novel NDT-based technique compared with the Less Invasive Surfactant Administration (LISA) technique in reducing the need for early invasive mechanical ventilation. METHODS: The NDT, designed for a

Early Antibiotic Use and Retinopathy of Prematurity: A Single-Center Retrospective Cohort Study.

OBJECTIVE: Retinopathy of prematurity (ROP) has been linked to neonatal sepsis, with antibiotic use suggested as a connection. Given the role of antibiotics in gut dysbiosis and the gut-retina axis, we assessed whether exposure to different antibiotic classes is associated with the incidence of treatment-necessary ROP. DESIGN: Retrospective cohort study. SUBJECTS: Preterm infants born at the University of Chicago Medicine and screened for ROP between January 2012 and December 2023. METHODS: Retrospective analysis was performed to compare systemic antibiotic exposure within the first 2 month

Association of antenatal dexamethasone administration timing with outcomes in preterm infants in a low- and middle-income country.

BACKGROUND: Timely administration of antenatal corticosteroids improves preterm outcomes; however, in low- and middle-income countries (LMICs), multiple barriers often delay treatment. Whether exposure within 24 h before delivery confers benefits remains uncertain. We aimed to evaluate the association between antenatal dexamethasone-to-delivery intervals and preterm outcomes in an LMIC setting. METHODS: This single-center prospective cohort study was conducted at a tertiary neonatal intensive care unit in India from June 2023 to December 2024. The participants were preterm infants born at 24

Gut microbiota development in very preterm infants following fortification of human milk.

Very preterm infants (VPIs) are born with an immature gut and predisposed to gut microbiota dysbiosis-related diseases, for example, necrotizing enterocolitis. Although fortification of human milk is required for these infants, the optimal fortifier remains uncertain. Bovine colostrum (BC), rich in protein and bioactive components, could serve as an alternative to conventional fortifiers (CF). The gut microbiota (GM) of 225 VPIs fed human milk fortified with either BC or CF (FortiColos study, NCT03537365) was profiled by 16S rRNA gene amplicon sequencing of fecal samples collected before, and

Impact of a dedicated antenatal specialist service for women with a history of stillbirth: the rainbow clinic.

BACKGROUND: Stillbirth in a prior pregnancy represents a significant risk factor for a subsequent stillbirth and other adverse pregnancy outcomes. There is a developing body of evidence supporting the implementation of dedicated specialist antenatal service provision for women with a history of stillbirth in future pregnancies. We aimed to assess the impact of our specialist service, the Rainbow Clinic, on maternal-fetal outcomes. METHODS: A retrospective case-control study was conducted comparing women with a history of previous stillbirth prior to and following the inception of the Rainbow

Is there a role for oral misoprostol in labor induction for prelabor rupture of membranes at term?

BACKGROUND: Most patients with pre-labor rupture of membranes (PROM) enter labor spontaneously at term. However, maternal and neonatal complications increase with prolonged PROM. Induction of labor (IOL) is therefore often recommended, but the optimal agent is still debated. Low-dose oral misoprostol (OM) is safe, effective, cheap, and easy to administer. However, there is little data regarding its role in PROM at term. OBJECTIVE: To determine the safety and efficacy of OM compared to Intravenous Oxytocin for IOL for PROM at term. STUDY DESIGN: A cohort study compared the perinatal outcomes

Clinical features, risk factors, and outcomes of neonatal invasive candidiasis: a 20-case study series from a tertiary neonatal critical care unit in Oman.

BACKGROUND: One of the well-known causes of morbidity and mortality in the neonatal population is invasive candidiasis. In neonatal critical care units, candida sepsis is now the third most frequent cause of late-onset sepsis. The objective of this research was to determine the prevalence of related risk factors, clinical traits, and results related to invasive candidiasis in the neonatal intensive care unit (NICU) at the Royal Hospital in Oman. METHODS: A case-control retrospective analysis of 20 newborns admitted between January 2006 and December 2020 to the neonatal intensive care unit at

Individualized Prediction of Platelet Transfusion Outcomes in Preterm Infants With Severe Thrombocytopenia.

IMPORTANCE: Preterm infants with severe thrombocytopenia (platelet count <50 × 109/L) frequently receive platelet transfusions. However, it is unclear in what cases prophylactic transfusion truly reduces bleeding risk or whether it does more harm than good. OBJECTIVE: To develop and validate a dynamic prediction model for major bleeding or mortality if prophylactic platelet transfusion were or were not to be given to infants with severe thrombocytopenia. DESIGN, SETTING, AND PARTICIPANTS: The dynamic prediction model was developed in an international multicenter cohort (2017-2021) comprising

Optimal treatment for women with acute hypertension in pregnancy; a randomized trial comparing intravenous labetalol versus nicardipine.

OBJECTIVES: Blood pressure control in severe hypertension of pregnancy is crucial for mother and neonate. In absence of evidence, guidelines recommend either intravenous labetalol or nicardipine. We compared the effectiveness and safety of these two drugs in women with severe hypertension in pregnancy. STUDY DESIGN: We performed an open label randomized controlled trial. Women with a singleton pregnancy complicated by severe hypertension (systolic ≥ 160 mmHg and/or diastolic ≥ 110 mmHg) requiring intravenous antihypertensive treatment were randomized to intravenous labetalol or intravenous ni
