The Patient Hides the Truth, but the Computer Tomography Examination Restores It!

In this article, we present the case of a patient who hid the real reason for which he came to the emergency room. This patient in this case is a 61-year-old man, who presented to the emergency department for symptoms relevant to respiratory disease. However, the initial diagnosis turned out to be only the "tip of the iceberg" in the evolution and treatment of this patient. Fournier gangrene is a fulminant form of infectious necrotizing fasciitis of the perineal, genital, or perianal regions, which frequently affects men, but can also occur in women and children. It is a rare but life-threaten

Standardization of Epidemiological Surveillance of Invasive Group A Streptococcal Infections.

Invasive group A streptococcal (Strep A) infections occur when Streptococcus pyogenes, also known as beta-hemolytic group A Streptococcus, invades a normally sterile site in the body. This article provides guidelines for establishing surveillance for invasive Strep A infections. The primary objective of invasive Strep A surveillance is to monitor trends in rates of infection and determine the demographic and clinical characteristics of patients with laboratory-confirmed invasive Strep A infection, the age- and sex-specific incidence in the population of a defined geographic area, trends in ris

Fournier's gangrene: a review of 43 reconstructive cases.

BACKGROUND: Fournier's gangrene is a rare and potentially fatal infectious disease characterized by necrotic fasciitis of the perineum and abdominal wall, along with the scrotum and penis in men and the vulva in women. Fournier's gangrene is a true surgical emergency. Skin loss can be very incapacitating and difficult to repair. METHODS: The authors reviewed retrospectively the clinical records of a series of 43 patients with Fournier's gangrene between the years 1985 and 2003 who, after initial treatment by the Departments of Urology and Surgery, were referred to the Department of Plastic Su

Necrotizing myositis case report and brief literature study.

Necrotizing myositis is an extremely rare soft tissue infection, mainly caused by Group A Streptococci. Although its presentation is nonspecific and seems harmless, it quickly leads to death in almost all cases. Therefore, diagnosis and treatment of necrotizing myositis are considered as medical emergencies. The 27 years old patient we report benefited from early diagnosis and care. Necrotic tissues were surgically removed 24 hours after the appearance of the first clinical signs. Intravenous antibiotherapy as well as immunoglobulin therapy were also given on the first day. Starting from this

Life- and limb-threatening infections following the use of an external fixator.

External fixation is widely used in orthopaedic and trauma surgery. Infections around pin or wire sites, which are usually localised, non-invasive, and are easily managed, are common. Occasionally, more serious invasive complications such as necrotising fasciitis (NF) and toxic shock syndrome (TSS) may occur. We retrospectively reviewed all patients who underwent external fixation between 1997 and 2012 in our limb lengthening and reconstruction programme. A total of eight patients (seven female and one male) with a mean age of 20 years (5 to 45) in which pin/wire track infections became limb-

Hip disarticulation for severe lower extremity infections.

UNLABELLED: Hip disarticulation is rarely performed for infections and variable mortality rates have been reported. We determined the number of deaths following hip disarticulation for severe lower extremity infections in 15 patients. Indications for hip disarticulation were necrotizing soft tissue infections in seven patients and persistent infections of the proximal thigh in eight patients. The most common microorganism was Staphylococcus aureus, present in eight patients. Hip disarticulation was performed emergently in seven patients and electively in eight patients. All patients survived t

[Skin and soft tissue infections: classification and epidemiology].

In the present review, authors take into consideration the classification and the epidemiology of the skin and soft tissue infections (SSTIs), a set of commonly observed pathologies, which can present different features, relatively to site and localization, clinical characteristics, and aetiological agent, their severity being related to the depth of the interested sites. Given the variable presentation of SSTIs, an assessment of their incidence and prevalence is difficult. In general, the incidence of SSTIs has increased due to the ageing of the general population, the increased number of cri

Trends and Economic Burden of Odontogenic Infections in Peri-Urban Ghana.

BACKGROUND: Severe odontogenic infections (OIs) are caused by dental caries, which have a sequelae of periodontal disease and periapical abscess. In clinical practice, there are sometimes sudden surges in odontogenic infections. Seasonal surges of dental emergencies have been documented with studies attributing these to public holidays when dental offices do not operate. The seasonal patterns however of OIs, be it rainy or dry season peaks of odontogenic infections, remain unexplored in the study area. The reasons behind such trends are not well understood either. METHODOLOGY: The study utili

[Otorhinolaryngologic infections in the post-pandemic context: What can we learn from the literature?].

BACKGROUND: The epidemiology of various respiratory infections changed due to the impact of the COVID-19 pandemic. Lockdowns and other containment measures (nonpharmacologic interventions, NPI) led to a decline in virus circulation, resulting in fewer cases at hospitals and practices. Once restrictions were eased, a rebound effect was observed worldwide, resulting in an increase in viral and severe invasive bacterial infections. OBJECTIVE: Various case series on different ear, nose, and throat (ENT) infections have been conducted in the context of the pandemic in Germany. These studies should

A uniquely human sebum lipid drives streptococcal skin infection severity.

Free fatty acids (FAs) secreted by the sebaceous glands are critical components of the skin barrier. Here, we investigate the activity of the major FA species present in the skin against Streptococcus pyogenes (group A Streptococcus [GAS]), a major cause of skin infections, such as impetigo, erysipelas, cellulitis, and necrotizing fasciitis. Several relevant FAs had antimicrobial activity against GAS, and linoleic acid was found to be therapeutic when applied in a murine skin infection model. However, sapienic acid, a FA abundant only in human sebum, significantly worsened disease severity des

Immunoglobulin G4-related disease presenting with nephrotic syndrome due to minimal change disease: a case report.

BACKGROUND: Immunoglobulin G4-related disease is an inflammatory disease affecting multiple organs including the kidney. Immunoglobulin G4-related kidney disease most commonly manifests as a tubulointerstitial nephritis and is associated with glomerular disease in a proportion of cases. Membranous nephropathy is the most frequent glomerular lesion. Herein, we report the first documented case of immunoglobulin G4-related disease presenting with nephrotic syndrome owing to minimal change disease. CASE PRESENTATION: A 67-year-old South Asian male presented to our service with systemic upset and

Polymicrobial infections with specific Actinomyces and related organisms, using the current taxonomy.

Actinomyces organisms reside on mucosal surfaces of the oropharynx and the genitourinary tract. Polymicrobial infections with Actinomyces organisms are increasingly being reported in the literature. Since these infections differ from classical actinomycosis, lacking of specific clinical and imaging findings, slow-growing Actinomyces organisms can be regarded as contaminants or insignificant findings. In addition, only limited knowledge is available about novel Actinomyces species and their clinical relevance. The recent reclassifications have resulted in the transfer of several Actinomyces spe
