Salvage of a femoral nonunion after primary non-Hodgkin's lymphoma of bone: a case report and literature review.

BACKGROUND: With the advent of superb microsurgery techniques and advanced stabilization instruments, recent decades have seen great progress in treating nonunions secondary to traumatic fractures. However, those nonunions that are secondary to primary non-Hodgkin's lymphoma of bone and often related to irradiation still remain a challenging problem. The condition could be more perplexing when bone healing abilities are greatly compromised and reliable stabilization is difficult. CASE REPORT: We performed an operation using free vascularized fibular graft in combination with a locking plate o

Osteosynthesis of supracondylar humerus fractures in children: a biomechanical comparison of four techniques.

BACKGROUND: Crossed k-wire osteosynthesis is a widely used procedure for displaced supracondylar humerus fractures in children, but the rate of secondary displacements is up to 31%. Alternative techniques including casts, elastic stable intramedullary nailing, and the fixateur extern, have been used, but there are no biomechanical data comparing these methods. We developed a biomechanical model to compare four osteosynthesis techniques for stabilizing supracondylar humerus fractures in children. METHODS: An osteotomy to simulate a fracture was made in a total of 32 adult cadaver humeri. The p

[Primary non-Hodgkin's lymphoma of bone].

Primary lymphoma of bone is a rare condition that has been described as a malignant neoplasm formed of lymphoid and myelopoetic tissues. Morphologic substrate is formed of lymphoid cells of different evolutional stages. Primary lymphoma of bone occurs predominantly in males; a male to female ratio is 1.8:1. It may occur at any age. The sites commonly affected are the long bones. In Lithuania 400 new cases of lymphoma are diagnosed every year. About 75% of them are non-Hodgkin's lymphoma. Many histological types and subtypes of lymphoma exist. This variation leads to difficulties when reviewing

CT-based micromotion analysis for tracking fracture displacement and fragment movement in trochanteric hip fractures: an agreement study.

BACKGROUND AND PURPOSE: 2-dimensional radiographs may underestimate 3-dimensional fracture displacement. CT-based micromotion analysis (CTMA) enables precise quantification of interfragmentary movement in 3 dimensions. We aimed to evaluate CTMA for quantifying fracture displacement, surgical reduction, and postoperative fragment movement in trochanteric hip fractures, and assessed measurement agreement from repeated follow-up scans. METHODS: We conducted a prospective cohort study including 15 patients with trochanteric fractures treated with a cephalomedullary nail at Uppsala University Hosp

The Legitimacy for the Use of Splint Immobilization After Elastic Intramedullary Nailing of Both-bone Forearm Shaft Fractures in Children.

BACKGROUND: Pediatric diaphyseal both-bone forearm fractures are increasingly treated with elastic stable intramedullary nailing (ESIN) using titanium nails. However, clear guidelines on the type and duration of splint immobilization are lacking. This study aims to assess the necessity of splint immobilization after ESIN for diaphyseal forearm fractures in children. METHODS: A prospective study was conducted on 38 patients with isolated radial and ulnar shaft fractures from 2018 to 2020. Patients were divided into two groups: 14 with splint immobilization for a mean of 3.93 weeks and 24 with

Risk of peri‑implant femoral fracture after cephalomedullary nailing in older patients with trochanteric fractures.

INTRODUCTION: Despite the recent increase in the use of cephalomedullary nails for trochanteric hip fractures, factors that may be associated with peri‑implant femoral fracture (PIFF) after cephalomedullary nailing for trochanteric fractures remain unknown. We investigated the factors associated with PIFF after cephalomedullary nailing of trochanteric hip fractures in older patients. METHODS: A nested case-control study was conducted using a database of patients aged ≥65 years who underwent surgery with cephalomedullary nails for trochanteric fractures caused by low-energy trauma during 2005-

Comparison of the clinical outcomes of intramedullary nailing between impending and completed pathological fractures caused by metastatic femoral tumors from solid cancers.

BACKGROUND: We examined the treatment outcomes following intramedullary nailing in patients with metastatic femoral tumors, excluding those from hematological malignancies. METHODS: We retrospectively evaluated treatment outcomes following intramedullary nailing between patients who underwent preventative surgery compared with those who had surgery following pathological fracture. Patients who underwent preventative surgery (Mirels' score ≥8) were allocated to the impending fracture group (n = 11) and those who underwent surgery after pathological fracture were allocated to the completed frac

Early weight bearing versus late weight bearing after intramedullary nailing for distal femoral fracture (AO/OTA 33) in elderly patients: A multicenter propensity-matched study.

BACKGROUND: This study aimed to assess differences in implant failure and bone union rates, amount of change in alignment of lower extremities, and walking ability between early weight-bearing (EWB) and late weight-bearing (LWB) groups following retrograde intramedullary nailing (RIMN) for distal femoral fracture (AO/OTA 33) (DFF) at multiple centers using propensity score matching. METHODS: The data of 213 patients who underwent RIMN from 2012 to 2019 in multiple tertiary hospitals were extracted from our database. Cases with the following factors were excluded: age <60, open fracture, AO/OT

[Suprapatellar Nailing of Tibial Fractures - Evaluation of Clinical and Radiological Results].

PURPOSE OF THE STUDY The locked nailing of diaphyseal fractures of the tibia currently represents a method of choice for treating the closed diaphyseal fractures, some of the tibial metaphyseal fractures and open tibial diaphyseal fractures classified as grade I and II according to Gustilo-Anderson (GA) classification. The suprapatellar (SP) approach is an alternative technique of insertion of the nail in semi-extension of the lower extremity with easier reduction, namely of multiple fractures and proximal diaphyseal fractures of the tibia in particular. This study aims to evaluate the group o

[Role of centro-medullary nailing in fractures of the distal quarter of the leg: about 30 cases].

The fractures of the distal quarter of the leg are characterized by fracture line located at the level of the lower quarter of the tibia, according to Gerard and Evrard definition [1]. They are serious and pose problems for consolidation, immobilization and stability. We here describe our experience in the Department of Orthopaedics and Traumatology at the Avicenne Military Hospital, Marrakech. We report 30 cases of closed fractures of the lower quarter of the leg treated with centro-medullary nailing over a period of 10 years (January 2001-December 2010). Locked nailing was performed in 80% o

Commentary on an article by Hannu T. Aro, MD, PhD, et al.: "Recombinant human bone morphogenetic protein-2: a randomized trial in open tibial fractures treated with reamed nail fixation".

This commentary was updated on April 15, 2011, because of a previous error. In the third paragraph, the sentence that had previously read “In the SPRINT, the two groups of open fractures were associated with statistically equivalent reoperation rates, but there was a trend toward better healing in the open fracture group treated with reamed intramedullary nailing.” now reads “In the SPRINT, the two groups of open fractures were associated with statistically equivalent reoperation rates, but there was a trend toward better healing in the open fracture group treated with unreamed intramedullary

Femoral shaft osteotomy for malunited femoral fractures presenting with varus OA of the knee.

Seven patients presenting between 1992 and 1999 with osteoarthritic knee symptoms in consequence of old malunited femoral shaft fractures, were treated by open femoral shaft osteotomy. The osteotomies were sited at the apex of the deformity and were fixed by locked intramedullary nailing. The patients were followed up until osteotomy union and reviewed clinically and radiologically with particular emphasis on symptoms and function. The mean age at presentation was 48 years and the mean time from fracture was 28 years. The mean time to osteotomy union was 28 months. Five of the seven patients r

Congenital pseudarthrosis of the ulna and radius in two cases of neurofibromatosis type 1.

Two children with neurofibromatosis type 1 who presented at birth with congenital pseudarthrosis of the ulna and radius are described. The patients were treated with broad resections. As a consequence, the forearms were reduced in length. The osteotomies were stabilized in one patient first with endomedullary nailing and then with a free vascularized fibular graft. In the second patient the osteotomy was stabilized by external fixation. Using these techniques, rapid and excellent healing and normal function were achieved. In contrast to the lower extremity, reduction of the length of the forea

Ipsilateral fractures of the femur and tibia: treatment with retrograde femoral nailing and unreamed tibial nailing.

Between 1989 and 1995 a total of 47 patients with 50 fractures of the ipsilateral femoral and tibial shafts without significant articular involvement were treated at Tampa General Hospital. Within this group were 24 patients with 26 fractures who were treated with intramedullary fixation of both bones using a technique of retrograde insertion of a femoral nail and unreamed insertion of an interlocking tibial nail. Information concerning the injuries, treatments, and much of the follow-up was gleaned from a trauma registry. Five of the femoral fractures (19%) and 14 of the tibial fractures (54%

[Does covered tibial intramedullary nailing promote formation of a compartment syndrome? Perioperative and intraoperative continuous monitoring of compartmental pressure in covered tibial intramedullary nailing].

Between October 1988 and October 1989 we performed a continuously pressure monitoring with the infusion technique in the tibialis anterior and deep posterior compartment of the lower leg during the nailing of the tibia in complete fractures of the lower leg in 16 patients. We set up 2 groups of patients because of different pathophysiological conditions: patients, who were operated on a few days after trauma (Group A) and patients operated on months after the trauma (Group B) because of non-union of the tibia. The effects of the preoperative, intraoperative, and postoperative manipulations wer

Tibiotarsal arthrodesis in a Moufflon sheep.

Restraint of a mature Moufflon sheep resulted in severe fracture and luxation of a previously osteoarthritic tibiotarsal joint. Arthrodesis was accomplished by means of internal pin fixation and an external fiberglass cast. After 3 months of hospitalization, the ram was returned to the original flock. Competition for dominance within the flock resulted in physical trauma to the ram. Seven months after surgery, the ram sustained a fractured ulna and died from exposure.

Use of intramedullary nailing in poor sanitary conditions: French Military Medical Service experience.

INTRODUCTION: Intramedullary (IM) nailing is the gold standard for treating long bone fractures in developed countries because of its minimally-invasive application and good biomechanical properties. In precarious conditions, external fixation or open plate fixation are often performed because surgeons do not have the means to carry out IM nailing. However, these procedures can lead to infection-related complications and mechanical failures. The aim of our study was to describe the outcomes and postoperative complications of IM nailing of closed, long bone fractures in patients operated in a F

Ipsilateral fracture of the femoral neck and shaft, treatment by reconstruction interlocking nail.

This study involved nine patients with ipsilateral fractures of the neck and shaft of the femur. They were all male with an average age of 28.5 years. All of the fractures resulted from high-energy trauma. The neck fracture was initially missed in one case. All fractures were fixed by a Russell-Taylor reconstruction femoral nail. Surgery was delayed for an average of 6.6 days (range 2-21 days). The patients were followed up for an average of 2.1 years. All fractures healed; the average time of union for the neck fracture was 4.2 months (range 3-6 months) and for the shaft fracture, 6.9 months
