Blood loss estimation during fronto-orbital advancement: implications for blood transfusion practice and hospital length of stay.

BACKGROUND: Reliable measurement of intraoperative blood loss remains a serious challenge during correction of craniosynostosis. This study analyzed the relationship between estimated blood loss (EBL) and calculated blood loss (CBL) in fronto-orbital advancement and its implications on blood transfusion practice and hospital length of stay (LOS). METHODS: The authors reviewed infants who underwent primary fronto-orbital advancement for craniosynostosis (1997-2009). Estimated blood loss was based on anesthesia records and CBL by preoperative/postoperative hemoglobin. Perioperative red blood ce

Classification of the inferior turbinate bones: a computed tomography study.

BACKGROUND: There are only few reports describing the texture of the inferior turbinate bone in normal and pathologic conditions. In this study, different types of human inferior turbinate bones were classified and radiological features of each type were defined. METHODS: The shape, structure and density of the inferior turbinate bones were evaluated using paranasal sinus computed tomography images of 283 patients. The cross-sectional areas of the bony part of the inferior turbinate were measured in bone windows. RESULTS: Human inferior turbinate bones were classified into four groups on the

Nasal defects and osseointegrated implants: UCLA experience.

A clinical study of 23 craniofacial implants placed in 11 nasal defects was conducted over a 7-year period. Implant-retained nasal prostheses were fabricated, implant success rate was determined, and the soft tissue responses were recorded at 6-month intervals. No data were gathered on two implants because of patient death. The implant success rate was 71.4% (15/21) but varied significantly by anatomic site. The implant success rate in the glabella was 0% (0/4), whereas the success rate in the anterior nasal floor was 88.1% (15/17). All implant failures occurred within the first year of loadin

Severe chemosis and treatment following fronto-orbital advancement surgery for Crouzon syndrome: A case report.

RATIONALE: Crouzon syndrome is a craniofacial malformation caused by premature fusion of fibrous sutures in infants. It is one of the most common craniosynostosis syndromes, and surgery is the only effective treatment for correcting it. Postoperative complications such as encephalocele, infections, hematoma have been reported. We herein report a case of a 62-month-old boy with Crouzon syndrome who underwent fronto-orbital advancing osteotomy, cranial vault remolding, and extensive osteotomy and subsequently developed left proptosis and severe chemosis, these complications are rare and we belie

The Relationships Between Craniofacial Structure and Frontal Sinus Morphology: Evaluation With Conventional Anthropometry and CT-Based Volumetry.

The purpose of this study was to investigate the effects of craniofacial structure and nasal septal deviation on frontal sinus morphology 3-dimensionally. This study of anatomy, anthropology, morphology, and radiology included 74 dry skulls as study sample. The craniofacial measurements were made through conventional anthropometric methods by the use of calipers. The nasal septal deviation measurements were done by computerized software on photographs taken from frontal view. Frontal sinus volumes were estimated by the computerized tomography-based volumetry. The relationships between craniofa

Frontobasal Fractures.

Frontobasal fractures occur in up to 24% of head injuries and often require a multidisciplinary approach. Besides the common bone fractures, the complex anatomy can cause damage to the sense of vision and smell. Further possibly lethal complications such as cerebrospinal fluid leak followed by meningitis or internal carotid bleeding can follow. Diagnostic and treatment options are reviewed with a focus on the endoscopic endonasal approach.

Frontal sinus repair through a frontalis rhytid approach.

Frontal sinus fractures have traditionally been repaired through a bicoronal approach. This incision provides a wide exposure, but is not without complications, particularly in the patient with or at risk for hairline recession. We present a series of 15 patients who underwent open reduction and internal fixation of anterior table frontal sinus fractures through a frontalis rhytid forehead incision and their results based on the scar appearance, forehead contour, frontalis function, sensation and fracture reduction. Paresthesias cranial to the incision in the supraorbital or supratrochlear dis

A clinical study of 31 individuals with midline facial defects with hypertelorism and a guideline for follow-up.

In order to contribute to clinical delineation of midline facial defects with hypertelorism (MFDH) and to etiologic diagnosis of the isolated form, 31 patients with MFDH unaffected by known syndromic associations were evaluated. Group A included patients personally examined by the authors, while Group B included those previously evaluated by other geneticists. Among the 14 patients from Group A, there were 7 with distinct pictures of multiple congenital anomalies. In Group B, 5 of the 17 patients also exhibited a distinct pattern of defects. Among isolated MFDH, there was association with anom

[Reconstruction of the frontal sinus with a calvarium split galea periosteum transplant after inflammatory complications].

After reconstruction of the frontal sinus with alloplastic material, with or without obliteration of the lumen, inflammatory complications may occur. The authors present a plastic-surgical functional reconstruction technique for revision surgery in cases of inflammatory complications following frontal sinus implantation of alloplastic material, by means of a modified autologous calvarian split graft. The inflammatory complications after primary reconstruction or obliteration of the frontal sinus occurred after a period of 1-10 years. The previously implanted material was removed from the front

The timing of physiologic closure of the metopic suture: a review of 159 patients using reconstructed 3D CT scans of the craniofacial region.

PURPOSE: The purpose of this study was to determine the normal physiologic timing of the closure of the metopic suture in non-craniosynostotic patients. METHODS: This clinical study involved a consecutive series of infants and young children who underwent 3D CT-scan evaluation for deformational plagiocephaly or suspected traumatic head injury. All patients with evidence of craniosynostosis were excluded from the study. Every infant and child referred to our Craniofacial Team for deformational plagiocephaly between 1997 and 2000 (n = 84) received a baseline pre-treatment 3D CT-scan of the head

MRI Allows Accurate Measurement of Glenoid Bone Loss.

BACKGROUND: Bony Bankart lesions larger than a certain size can lead to a high redislocation rate, despite treatment with Bankart repair. Detection and measurement of glenoid bone loss play key roles in selecting the appropriate surgical therapy in patients with shoulder instability. There is controversy about which diagnostic modalities, using different measurement methods, provide the best diagnostic validity. QUESTIONS/PURPOSES: (1) What are the diagnostic accuracies of true AP radiographs, West Point (WP) view radiographs, MRI, and CT to detect glenoid bone loss? (2) Are there differences

Two-generation Cleidocranial Dysplasia Involving Three Individuals of a Family: A Rare Case Report.

Cleidocranial dysplasia (CCD) is a rare autosomal dominant skeletal disorder characterized by multiple dental and skeletal abnormalities. While isolated case reports exist, this case is unique due to its clear familial inheritance pattern and the need for urgent surgical dental intervention in a symptomatic young adult, thereby contributing new clinical insights to the limited literature on CCD. A female patient in her early 20s presented with pain and pus discharge in the mandibular left posterior region for 3 months. Extraoral findings included short stature, frontal bossing, hypertelorism,
