[Aseptic non-union of humeral shaft fractures treated by plating and bone grafting].

PURPOSE OF THE STUDY: Although aseptic non-union of humeral shaft fractures is generally considered to be an exceptional complication, rates in the literature have varied from 1 to 10%. Factors favoring non-union are often related to technical error or inappropriate therapeutic indication. Several types of treatment (orthopedic, locked centromedullary nailing, ascending pinning, plating, external fixation) can be proposed for humeral shaft fractures. In all cases, a precise technique and proper indication are essential for success. We reviewed the cases of 30 patients who underwent surgery for

Whether the Indications for Reverse Shoulder Arthroplasty Should Continue to Be Expanded? A Systematic Review and Meta-Analysis.

BACKGROUND: It is still unclear whether reverse total shoulder arthroplasty (RTSA) has advantages over traditional hemiarthroplasty (HA) and anatomic total shoulder arthroplasty (ATSA) in the treatment of complex shoulder joint diseases. Therefore, this study aims to evaluate the clinical effectiveness of RTSA in the treatment of complex shoulder joint diseases and further determine whether it is necessary to expand the indications of RTSA. METHOD: We conducted a systematic search of studies published between January 1, 2012 and May 31, 2023 in PubMed, Embase, and Cochrane databases. The expe

Confidence of graduating family practice residents in their management of musculoskeletal conditions.

A study was undertaken to determine the confidence of graduating family practice residents in the management of musculoskeletal conditions and to determine the level of exposure of graduating family practice residents to fracture care. A 2-page questionnaire consisting of 50 items was sent to 680 graduating family practice residents at 100 randomly selected residency training programs throughout the United States. Family practice residents were questioned about their fracture care experience, including the number of fractures diagnosed, the number of fractures reduced, the number of fractures

Shoulder arthroplasty for post-traumatic osteonecrosis of the humeral head.

BACKGROUND: Osteonecrosis (ON) of the humeral head is a known complication of proximal humeral trauma. Prosthetic replacement may be the last option to treat the associated pain. Depending on the condition of the glenoid, hemiarthroplasty (HA) or total shoulder arthroplasty (TSA) can be considered. To date, the peer reviewed literature offers limited direction on the better treatment for this population. METHODS: Between 1973 and 2010, 93 arthroplasties were performed for post-traumatic ON of the humeral head after conservative treatments failed. Of these, 37 HAs and 46 TSAs were monitored fo

Structural severity outweighs laboratory biomarkers in predicting short-term failure under nonoperative management for partial-thickness supraspinatus tears: a prospective cohort study.

BACKGROUND: Nonoperative treatment, including platelet-rich plasma injection is increasingly used for partial-thickness rotator cuff tears, yet predictors of treatment failure and the incremental prognostic value of laboratory biomarkers remain unclear. We aimed to identify predictors of short-term treatment failure under nonoperative management and to evaluate whether inflammatory and metabolic biomarkers improve risk stratification beyond clinical and imaging variables. METHODS: We conducted a prospective cohort study at Hai Phong International Hospital from 2020 to 2025 including patients w

Energy-responsive multi-catalytic nanogenerator hydrogels synergistically enhance diabetic tendon-to-bone healing via the ages/RAGE/PIEZO2 axis.

Rotator cuff tears (RCTs) represent a significant clinical challenge with high post-operative re-tear rates, especially in diabetic patients. Through comprehensive bioinformatic analysis of a publicly available transcriptomic dataset (GSE236746), we identified the significant downregulation of the mechanosensitive hub gene PIEZO2 and the aberrant activation of oxidative stress/inflammatory pathways as primary pathological barriers in diabetic patients. This metabolic dysfunction triggers the advanced glycation end products (AGEs)/receptor for AGEs (RAGE) axis, creating a self-amplifying oxidat

Three-week immobilization vs. no immobilization in primary reverse total shoulder arthroplasty: a randomized controlled trial.

BACKGROUND: There is no consensus about the length of the immobilization period after receiving a reverse shoulder arthroplasty (rTSA) for a cuff-deficient shoulder. The objective of this study was to determine whether patients receiving an rTSA for a cuff-deficient shoulder could be managed without any immobilization after surgery. METHODS: Prospective randomized study that included a primary rTSA implanted for a cuff-deficient shoulder. Patients were randomized into group 1 (3-week immobilization) or group 2 (without immobilization). Pain was assessed with the visual analog scale (VAS) the

Comparative proteome analysis of the capsule from patients with frozen shoulder.

BACKGROUND: The etiology of frozen shoulder (FS) is unclear. Accordingly, this study used a label-free quantitative shotgun proteomic approach to elucidate the pathogenesis of FS based on protein expression levels. METHODS: Tissue samples from the rotator interval (RI), middle glenohumeral ligament (MGHL), and anterior-inferior glenohumeral ligament (IGHL) were collected from 12 FSs with severe stiffness and 7 shoulders with a rotator cuff tear (RCT) as controls. Protein mixtures were digested and analyzed by nano-liquid chromatography/electrospray ionization-tandem mass spectrometry. Relativ

The effect of Medicaid payer status on patient outcomes following repair of massive rotator cuff tears.

BACKGROUND: The influence of socioeconomic status and insurance type has not been studied extensively for RCR, particularly not in the high risk massive RCT population. The purpose of this study is to identify relationships between Medicaid payer status and patient outcomes following massive RCR. METHODS: A retrospective review of shoulder surgery database identified 29 patients undergoing massive rotator cuff repair. Patients were stratified based on insurance type into two cohorts, Medicaid (14 patients) and non-Medicaid (15 patients). Missed routine follow-up appointments and comorbidities

Humeral bone resorption after anatomic shoulder arthroplasty using an uncemented stem.

BACKGROUND: Bone resorption around the femoral stem after total hip arthroplasty is a well-known phenomenon. However, only a few studies have evaluated bone resorption after shoulder arthroplasty. This study investigated the prevalence of humeral bone resorption after different shoulder arthroplasty procedures. METHODS: The study included 147 shoulders that underwent total shoulder arthroplasty (TSA) or humeral head replacement (HHR) with an uncemented humeral stem from November 2008 to May 2015 and were monitored for more than 1 year. The prevalence of humeral bone resorption and risk factor

Effects of kinesiotaping added to a rehabilitation programme for patients with rotator cuff tendinopathy: protocol for a single-blind, randomised controlled trial addressing symptoms, functional limitations and underlying deficits.

INTRODUCTION: Rotator cuff tendinopathy (RCTe) is the most frequent cause of shoulder pain, resulting in considerable losses to society and public resources. Muscle imbalance and inadequate sensorimotor control are deficits often associated with RCTe. Kinesiotaping (KT) is widely used by clinicians for rehabilitation of RCTe. While previous studies have examined the immediate effects of KT on shoulder injuries or the effects of KT as an isolated method of treatment, no published study has addressed its mid-term and long-term effects when combined with a rehabilitation programme for patients wi

The intra-rater reliability of measured thoracic spine mobility in chronic rotator cuff pathology.

OBJECTIVES: The relationship between thoracic spine dysfunction and painful shoulder pathologies is poorly understood. This study sought to determine the intra-rater reliability of a protocol aimed at assessing the active mobility of the thoracic spine in individuals with chronic rotator cuff pathology. METHODS: Ten individuals (6 men and 4 women) with chronic rotator cuff pathology were recruited and screened according to strict criteria. Voluntary active thoracic spine motion was recorded in two planes using a multidimensional motion analysis system. Each assessment was undertaken on two oc

Proximal humerus osteolysis after revision rotator cuff repair with bioabsorbable suture anchors.

Biodegradable anchors were designed to provide secure fixation while allowing for later resorption and replacement by host tissue. First-generation implants degraded relatively rapidly and caused foreign-body reactions, synovitis, fragmentation, and osteolysis. Newer implants have similar complications. It is not known if the primary cause of the osteolysis is biological (precipitated by breakdown products of the polymer) or mechanical (caused by initial loss of implant stability). Case reports have described glenoid osteolysis around biodegradable suture anchor placement for shoulder stabiliz

Arthroscopically assisted fixation of glenoid fractures: a cadaver study to show potential applications of percutaneous screw insertion and anatomic risks.

BACKGROUND: Displaced glenoid fractures require reduction and internal fixation to avoid chronic instability or degenerative changes. Arthroscopically assisted percutaneous fixation has been performed successfully for such fractures, avoiding extensive surgical dissection. To assess the application of this new technique, our cadaveric study was designed to show 1) the safe zones of percutaneous screw insertion and 2) the position of bony obstructions to the glenoid. METHODS: In 18 cadaver shoulders, we performed arthroscopically assisted percutaneous wire insertion into the glenoid using ante

Correlations between biochemical markers in the synovial fluid and severity of rotator cuff disease.

The role of biochemical factors in the onset and natural history of rotator cuff disease is not fully understood, but it is generally recognised that they could induce tendon damage in association with mechanical and vascular factors. In this study, 5 biochemical parameters were analysed (total protein concentration, matrix metalloproteinase (MMP)-2 or gelatinase A, MMP-9 or gelatinase B, type I collagen telopeptides, hyaluronic acid) in the synovial fluid (SF) aspirated from the gleno-humeral joint of 29 patients undergoing surgical therapy for rotator cuff lesions. Functional outcome after proximal humeral fractures treated with hemiarthroplasty.

OBJECTIVE: To evaluate functional outcome after hemiarthroplasty for displaced proximal humeral fractures and to review whether prosthesis type, intraoperative technique or previous ipsilateral shoulder surgery could affect the outcome. METHODS: We reviewed the medical records and radiographs of patients who had undergone hemiarthroplasty for proximal humeral fractures between 1992 and 2000. We identified 45 patients, 39 with acute fractures and 6 with fracture-related complications. One surgeon performed 17 surgeries (38%), and the rest were carried out by 11 other orthopedic surgeons. An adjustable shoulder abduction orthosis for the post-operative management of tendon transfers: A preliminary study.

Transfer of Musculus Latissimus dorsi/M. Teres major to the rotator cuff with or without M. Subscapularis/M. Pectoralis release is a widely used procedure for restoring shoulder abduction and external rotation in squeal of obstetric brachial plexus palsy. After the operation a shoulder abduction orthosis in maximal external rotation and 90 - 100 degrees abduction is utilized following six weeks of immobilization in a shoulder spica cast for protecting the newly transferred muscle from undue elongation. However this in turn may cause contracture of the external rotators. To overcome this proble

Cerebral ischemia during shoulder surgery in the upright position: a case series.

We report 4 cases of ischemic brain and spinal cord injury after shoulder surgery in the beach chair position, using data from medical legal case reviews. We argue that the correlation between cardiovascular risk factors and cerebral ischemic complications for this type of surgery is poor in these middle-aged patients. Rather, our analysis suggests that the sitting position and the head position create specific physiological conditions that may be conducive to cerebral and spinal cord ischemia during this type of surgery. Thromboembolic events may be an additional cause of adverse neurologic o
