Hypotensive extremely low birth weight infants have reduced cerebral blood flow.

OBJECTIVES: Whether extremely low birth weight (ELBW) infants are at risk of cerebral hypoperfusion is uncertain because key issues concerning their cerebral blood flow (CBF) and mean arterial pressure (MAP) are unresolved: (1) whether CBF is pressure-passive or autoregulated; (2) the normal level of MAP; and (3) whether inotropic drugs used to increase MAP might inadvertently impair CBF. We addressed these issues in ELBW infants undergoing intensive care. METHODS: CBF (measured by near-infrared spectroscopy) and MAP were measured in 17 infants aged 1.5 to 40.5 hours. RESULTS: Five infants r

Effects of furosemide versus captopril on postprandial and orthostatic blood pressure and on cerebral oxygenation in patients > or = 70 years of age with heart failure.

Elderly patients with heart failure are at risk of postprandial hypotension (PPH), orthostatic hypotension (OH), and concomitant cerebral oxygenation changes because of altered cardiovascular balance and the use of cardiovascular medications, such as furosemide and captopril. In 24 patients with heart failure (New York Heart Association class II to III, in stable condition, and receiving cardiovascular medication [aged 70 to 83 years]), blood pressure (BP) was measured by Finapres, and cortical concentrations of oxyhemoglobin and deoxyhemoglobin were measured using near-infrared spectroscopy d

Pharmacological interventions in the newborn piglet in the first 24 h after hypoxia-ischemia. A hemodynamic and electrophysiological perspective.

The purpose of this study was to investigate whether combined inhibition of neuronal and inducible nitric oxide synthase (NOS) by 2-iminobiotin, free radical scavenging by allopurinol, and non-protein-bound iron chelation with deferoxamine improved cerebral oxygenation, electrocortical brain activity, and brain energy status during the first 24 h after hypoxia-ischemia (HI) in the newborn piglet. Forty-three newborn piglets were subjected to 1 h of severe HI by occluding both carotid arteries and phosphorous magnetic resonance spectroscopy ((31)P-MRS)-guided hypoxia, whereas five served as sha

New horizons: disrupted brain energy metabolism as a driver of delirium.

Delirium is a highly prevalent neuropsychiatric syndrome characterised by acute inattention, altered arousal and impaired cognition. Cerebral energy insufficiency is hypothesised to drive delirium and both hypoglycaemia and hypoxia can directly precipitate functional deficits and EEG slowing. Here we review the evidence that disrupted energy metabolism may play a causative role in delirium across multiple settings. Neuromonitoring methods including near infrared resonance spectroscopy and Transcranial Doppler suggest an association between altered cerebral perfusion and delirium, albeit with a

Operando XANES-FTIRS study of dehydrogenation of ethanol to acetaldehyde over V2O5/TiO2 and V2O5/CeO2 catalysts.

An operando study of non-oxidative and oxidative dehydrogenation of ethanol to acetaldehyde over V2O5/TiO2 and V2O5/CeO2 catalysts was performed using X-ray absorption near-edge structure (XANES) and Fourier-transform infrared spectroscopy (FTIRS). The experiments were carried out at atmospheric pressure within a temperature range of 50-300 °C. XANES was employed to probe the coordination and oxidation states of vanadium, cerium, and titanium in the catalysts under working conditions. To quantitatively analyze the XANES data, a machine learning-based approach was implemented. FTIRS was used to

A Decision Tree Cost Analysis of Intracranial Bleed Detection Using a Near-Infrared Device Across Various Healthcare Levels.

Traumatic brain injury (TBI), mainly caused by road traffic accidents, is a serious global public health concern. Computed tomography (CT) is the best way to detect intracranial haemorrhage (ICH), but it is not always feasible because it is hard to access, exposes people to radiation, and is expensive, especially in low- and middle-income countries. Portable near-infrared spectroscopy (NIRS) devices offer a non-invasive, point-of-care option for early detection of ICH. The objectives of this study were to estimate the cost per case detected for patients with mild-to-moderate TBI, to estimate i

Transcranial direct current simulation as an adjunctive treatment for treatment-resistant depression in hospitalized patients: A feasibility study protocol.

Transcranial direct current stimulation (tDCS) is clinically effective in treating treatment-resistant depression (TRD), as measured by response, symptom improvement, and disease remission. However, the feasibility and underlying mechanism of tDCS treatment in individuals with TRD during acute psychiatric hospitalization remain poorly characterized. This paper outlines the protocol that aims to investigate the feasibility of implementing a 5-day tDCS treatment in hospitalized patients with TRD and secondarily explore the effects on depression and cognition, and neurophysiological mechanisms un

Cerebellar transcranial direct current stimulation combined with virtual reality training for the balance of older adults: study protocol for a randomized controlled trial.

BACKGROUND: Falls are a major public safety issue, and improving the posture control ability of elderly people is highly important for preventing falls. Transcranial direct current stimulation (tDCS) and virtual reality-based balance training (VRBT) have been proven to be effective in improving postural stability and balance ability. The combination of these two methods may achieve better therapeutic effects. At present, there is a lack of research on the effects of tDCS combined with VRBT on postural stability and fall prevention in elderly individuals. METHODS: This 4-week single-blind, para

Impact of mild hypo- and hyperventilation on cerebral oxygen supply during general anesthesia.

OBJECTIVE: Cerebral blood flow autoregulation is affected by several physiologic and medical factors. Especially arterial carbon dioxide pressures (PaCO2) impact cerebral blood flow. Only extensive changes in end-tidal CO2 have been studied so far. The aim of this study was to evaluate the impact of mild hypo- and hyperventilation on cerebral blood flow as assessed by regional cerebral red blood cell oxygen saturation (rSO2) in two age groups. METHODS: Two groups of patients were compared under general anesthesia before the surgical procedure was started: A younger patient group (age < 40 yea

Neonatal Shock: Current Dilemmas and Future Research Avenues.

Neonatal shock presents a complex clinical challenge and is one of the leading causes of mortality. Traditionally, neonatal shock is equated to hypotension, and therapeutics are often initiated based on low blood pressure (BP) values alone. This fails to address the underlying goal of optimizing the tissue perfusion resulting in both over- and under-treatment of neonatal shock. Also, what defines a normal BP in neonates is still a contentious topic. Further, the most appropriate way of measuring BP in neonates with shock is still debated. Shock secondary to transient circulatory instability an

Advances in the Detection and Identification of Bacterial Biofilms Through NIR Spectroscopy.

Bacterial biofilms play an important role in the pathogenesis of infectious diseases but are also very relevant in other fields such as the food industry. This fact has led to an increased focus on the early identification of these structures as prophylaxes to prevent biofilm-related contaminations or infections. One of the objectives of the present study was to assess the effectiveness of NIR (Near Infrared) spectroscopy in the detection and differentiation of biofilms from different bacterial species, namely Staphylococcus epidermidis, Staphylococcus aureus, Enterococcus faecium, Salmonella

Characteristics of central cortex and upper-limb flexors synchrony oxygenation during grasping in people with stroke: a controlled trial study protocol.

BACKGROUND: Upper limb motor impairment is a common consequence of stroke, and the effectiveness and underlying mechanisms of rehabilitation therapy for improving upper limb function remain uncertain. Functional near-infrared spectroscopy, a reliable wearable neuroimaging technique, holds promise for investigating brain activity during functional tasks. This study aims to explore the synchronous oxygenation characteristics of the central cortex and upper-limb flexors during a grasping task and investigate the rehabilitation mechanisms of upper limb motor function in individuals with stroke. Enhancement of prefrontal functional connectivity under the influence of concurrent physical load during mental tasks.

BACKGROUNDS: Functional near-infrared spectroscopy (fNIRS) is widely used for the evaluation of mental workload (MWL), but it is not yet clear whether it is affected by physical factors during cognitive tasks. Therefore, the combined effects of physical and cognitive loads on hemodynamic features in the prefrontal cortex were evaluated. METHODS: Thirty-three eligible healthy male subjects were asked to perform three types of cognitive tasks (1-back, 2-back and 3-back). Concurrently, isotonic contraction aerobic exercise of the left upper limb was added. During this compound task, fNIRS signal

Abnormal prefrontal cortical activation during the GO/NOGO and verbal fluency tasks in adult patients with comorbid generalized anxiety disorder and attention-deficit/hyperactivity disorder: An fNIRS study.

Generalized anxiety disorder (GAD) and adult attention-deficit/hyperactivity disorder (ADHD) are commonly reported comorbidities. Adult GAD patients with comorbid ADHD are often underdiagnosed and undertreated. To explore the clinical value of functional near-infrared spectroscopy (fNIRS) data for assisting in the accurate diagnosis of ADHD in individuals with GAD, haemoglobin (HbO) concentration changes in the prefrontal cortex (PFC) were detected via fNIRS in 49 patients with both GAD and ADHD, 46 patients with GAD, and 34 healthy controls (HCs) during a verbal fluency task (VFT) and a GO/NO

Value of brain tissue oxygen saturation in neonatal respiratory distress syndrome: a clinical study.

Neonatal respiratory distress syndrome (NRDS) is one of the major causes of pre-term mortality and morbidity among very-low-birth-weight infants (VLBWI) in low- and middle-income countries (LMIC). Some of the neonates pass away despite admission and care in intensive care units (ICUs). The present clinical trial seeks the application value of elevating oxygen saturation in the brain cells of pre-term neonates born with NRDS. Near-infrared spectroscopy (NIRS) was used to monitor the neonates' microscopic cerebral oxygenation levels do determine hemoglobin concentration in brain tissues, whereas

Towards point-of-care manufacturing and analysis of immediate-release 3D printed hydrocortisone tablets for the treatment of congenital adrenal hyperplasia.

Hydrocortisone (HC) is the preferred drug in children with congenital adrenal hyperplasia due to its lower potency as well as fewer reports of side effects. Fused deposition modelling (FDM) 3D printing holds the potential to produce low-cost personalised doses for children at the point of care. However, the compatibility of the thermal process to produce immediate-release bespoke tablets for this thermally labile active is yet to be established. This work aims to develop immediate-release HC tablets using FDM 3D printing and assess drug contents as a critical quality attribute (CQA) using a co

Revealing the interaction between peptide drugs and permeation enhancers in the presence of intestinal bile salts.

Permeability enhancer-based formulations offer a promising approach to enhance the oral bioavailability of peptides. We used all-atom molecular dynamics simulations to investigate the interaction between two permeability enhancers (sodium caprate, and SNAC), and four different peptides (octreotide, hexarelin, degarelix, and insulin), in the presence of taurocholate, an intestinal bile salt. The permeability enhancers exhibited distinct effects on peptide release based on their properties, promoting hydrophobic peptide release while inhibiting water-soluble peptide release. Lowering peptide con

Audiovisual integration in children with cochlear implants revealed through EEG and fNIRS.

Sensory deprivation can offset the balance of audio versus visual information in multimodal processing. Such a phenomenon could persist for children born deaf, even after they receive cochlear implants (CIs), and could potentially explain why one modality is given priority over the other. Here, we recorded cortical responses to a single speaker uttering two syllables, presented in audio-only (A), visual-only (V), and audio-visual (AV) modes. Electroencephalography (EEG) and functional near-infrared spectroscopy (fNIRS) were successively recorded in seventy-five school-aged children. Twenty-fiv
