Presentation and diagnosis of small acoustic tumors.

With the recent advent of magnetic resonance imaging and auditory brain stem response, it is now possible to diagnose acoustic tumors while they are still quite small. As a result, it is becoming obvious that the clinical presentation of these smaller lesions can be somewhat variant to what is considered typical for an acoustic neuroma. Likewise, although the sensitivity of auditory brain stem response for larger tumors is believed to be quite good, the sensitivity for smaller tumors has recently been questioned, particularly when the patient is first seen early in the course of the disease wi

Translabyrinthine and transotic surgery for acoustic neuroma.

The results and complications of translabyrinthine and transotic surgery for petrous apex lesions between 1980 and 1992 are presented. An acoustic neuroma was found in 52 patients. In 1988, the translabyrinthine approach was modified into the transotic approach and replaced the former technique. There was no mortality in this series, but two patients had mild brainstem infarcts and there was post-operative bleeding into the cerebellopontine angle in one. Cerebrospinal fluid (CSF) leakage was seen in six patients and meningitis in two. Three suffered deep vein thrombosis in their legs. There wa

Facial myokymia due to acoustic neurinoma.

The case of a 62-year-old female patient who presented with facial myokymia is reported. The patient had a 13-year history of progressive left-sided hearing loss. In further course, involuntary, wormlike, rippling movements of the left facial muscles developed. Computed tomography revealed a tumor located in the left cerebellopontine angle. Electrophysiologic examinations confirmed the diagnosis of facial myokymia. The tumor, which evolved from the eighth cranial nerve, was totally removed microsurgically. The tumor was histologically verified to be an acoustic neurinoma. Postoperatively, the

[Laser CO2 in the surgery of acoustic neuroma].

The advantages derived from the use of the CO2 Laser in the treatment of tumours of the cranial base have been widely reported. The authors present their results after the resection of 12 cases of giant acoustic neurinomas operated on with microsurgical technique and CO2 Laser, comparing these results with a similar series, already published (Neurochirurgie, 1984, 30, 17-24), of 12 cases operated on only with microsurgical technique. The present methodology includes the evaluation with magnetic resonance imaging and with pre and intra-operative evoked potentials. A 60 Watt CO2 Laser adapted to

Acoustic analysis of infantile stridor: a review.

The review compares five methods that utilise electronic/computer acoustic processing techniques for the analysis of infantile stridor sounds. The first method uses traditional spectrographic techniques to produce time/frequency/intensity three-dimensional representation of the waveform. The second method is computer-based and uses the fast Fourier transformation (FFT) to show the frequency composition of the waveform. The third uses linear prediction coefficients (LPCs) to produce a power spectrum and inverse filtering to estimate the cross-sectional area of the human upper airway. The fourth

Non-acoustic tumors of the cerebellopontine angle.

52 patients with non-acoustic neurinoma tumors of the cerebellopontine angle undergoing surgery over a 16 year period (1974-1989) are analyzed with regard to epidemiologic factors, presenting symptoms, neurological findings, diagnostic procedures, surgical approaches, recurrences, and longterm survival rates. Meningiomas (20 cases) and epidermoid tumors (16 cases) outnumber the rare tumors which exhibit wide histological variation. Trigeminal neuralgia was found as a significantly frequent sign of epidermoids. Total removal was possible in nine meningiomas (45%), fourteen epidermoid tumors (87

Acoustic characteristics of post-thyroplasty patients.

This study investigated changes in voice quality after thyroplasty type I in eight adults with unilateral vocal fold paralysis. A silicone rubber implant was inserted through a window in the thyroid ala and placed between the inner and outer perichondrium to externally medialize the abducted vocal fold. Measures of fundamental frequency (vocal pitch), pitch range, maximum phonation time, s/z ratio, pitch perturbation (vocal jitter), and amplitude perturbation (vocal shimmer) were made 1 to 2 weeks preoperatively and 1 month postoperatively. Postoperative voice quality was characterized by an i

[Acoustic and mechanical interpretation of the "on-off" effect].

The on-off effect is a particular form of the stapedial reflex. It is presented as a double positive deflection appearing when the stimulus starts and stops. This type of reflex is very frequent in beginning otospongiosis. The mixed theory assimilates the annual ligament of the stirrup to an elastic pin which obeys the physical laws of a spring. Otospongiosis produces a shortened sement and thickening of the annular ligament, with an increase in its rigidity, responsible for deafness of the low sounds at the beginning of the disease. The acoustic and mechanical theory enables a clear explanati

[Acoustic rhinometry: the bat principle of the nose].

All cross-sectional areas of the upper airway can be measured by an acoustic signal using the acoustic reflection technique, or acoustic rhinometry. The plane of the cross-sectional areas measured was determined in nasal models. The isotemporal layers were found to be nearly parallel to the nasal valve. The acoustically measured cross-sectional areas correlated with the cross-sectional areas of cuts from nasal models. After digitizing these cuts, a CAD software calculates cross-sectional areas in all orientations and at all distances. The difference between the measured and calculated cross-se

[Unusual circumstances of disclosure of acoustic neurinoma].

While progressive unilateral deafness remains the most typical symptom of the development of acoustic neuroma, the study of at least 1,200 cases operated since 1966 shows that in at least 16.5% of all cases, the triggering signal to seek medical advice was either otologic but atypical, or non-otologic but including symptoms in another pair of cranial nerves, or without any apparent relationship with the area showing a tumor of the eight pair, or even occurring under etiological circumstances that were misleading at first.

Changes in clinical features of acoustic neuroma.

During a 14-year period from 1976 to 1989, the authors experienced 160 cases of acoustic neuroma (AN). The clinical features were investigated in 132 of these cases (excluding those cases of recurrence and neurofibromatosis 2). The cases were divided into three periods for study. There were no significant differences in the patients' age, sex, or affected side among the three periods. The number of intracanalicular tumors have increased in recent years. The proportion of medium-sized and large tumors, however, has not changed, comprising about 70% of the total number of cases. Tumors of 20 mm

The clinical application of acoustic distortion products.

Otoacoustic emissions provide an objective measure of hair-cell function that is independent of retrocochlear activity. Because of their frequency specificity, distortion-product emissions have great potential for clinical use. The present report describes the results of initial studies in patients with known cochlear or retrocochlear disease, diagnosed with standard otologic and audiologic tests. The cochlear group consisted of those diagnosed with noise-induced hearing loss, Meniere's disease, or hereditary hearing loss, whereas patients with acoustic neuroma comprised a retrocochlear group.

[Acoustic oto-emissions. Clinical uses].

Otoemissions are acoustic signals emitted by the cochlea in response to a short acoustic stimulation (click) that may be recorded with a miniaturized microphone placed within the external auditory canal. They reflect the active mechanisms of the cochlea as they relate to the contractile properties of the outer hair cells, which determine the cochlear properties of sensitivity to and selectivity of frequencies. We have been studying for 5 years the clinical applications of otoemissions in over 500 patients. Otoemissions in 100% of normal ears may be recorded, and the detection threshold is gene

Involvement of cochlear nerve in acoustic tumours.

With conventional light and transmission electronmicroscopy we studied 10 cases of acoustic nerve tumour, 3 of which proved to be instances of von Recklinghausen neurofibroma and 7 of schwannoma. Schwannomas were not found to infiltrate the cochlear nerve. Hearing loss, if present in cases of schwannoma, could be related to non-specific lesions of the uninfiltrated cochlear nerve in the vicinity of the vestibular nerve tumour. Only neurofibromas were found to infiltrate the cochlear nerve. Distinction between tumour infiltration and non-specific lesion could be made by electron microscopy.

Acoustic characteristics of speech following uvulopalatopharyngoplasty.

Selective uvulopalatopharyngoplasty has resulted in the improved management of sleep apnea; however, the effect of the surgery on speech has not been systematically evaluated. In this study, the speaking fundamental frequency, reading rate, and first and second formants of the vowels of four patients were measured acoustically presurgically and postsurgically. The results indicate that the length of time needed to read a passage was reduced in all four patients 2 weeks following surgery compared to the presurgical duration. In addition, the second formant of nine vowels studied was lower posts

Acoustic otoscopy in the diagnosis of otitis media.

Acoustic otoscopy detects middle ear pathology by measuring the ability of the tympanic membrane to reflect sound. Fluid or thickening of the tympanic membrane increases sound reflection. We conducted a study to compare acoustic otoscopy with pneumatic otoscopy in identifying middle ear pathology in 80 children (160 ears) presenting to the emergency department with ear or upper respiratory complaints. We then evaluated the use of acoustic otoscopy in 34 adults (68 ears) with and without ear complaints. Using a reflectivity of 5 or more units to signify acute pathology in children, the sensitiv

[The facial nerve in acoustic tumor surgery].

The results of facial nerve function in twenty-four cases of acoustic neuroma removal using translabyrinthine approach from 1982 to 1987 were reported. Facial nerve had been paralysed in one case preoperatively. The facial nerve function were preserved in 21 out of 23 cases (91.3%). The appearance of facial nerve during surgery was as follows: normally located in 9 cases, displaced and deformed in 12 cases and invaded by the tumor in 3 cases. The size and location of the tumor determine the chance of preservation of facial nerve function. The procedures of manipulating the facial nerve and pre

Otosclerosis masking coexistent acoustic neuroma.

The coexistence of an acoustic neuroma with otosclerosis can prove to be a great diagnostic dilemma. In such cases, the diagnosis of acoustic neuroma is usually delayed while more common causes of sensorineural hearing loss associated with otosclerosis or its surgery are considered. Any asymmetric progression of a sensorineural loss or shift in discrimination after stapes surgery, whether sudden or gradual, should provoke suspicion of a second pathologic process. These include perilymph fistula, labyrinthine otosclerosis or ischemia, and acoustic neuroma. The use of auditory brainstem response

[Preservation of hearing in surgery of acoustic neuroma].

Between 1981 and 1984, 44 patients underwent acoustic neuroma removal by a posterior cranial fossa approach in an attempt to preserve hearing. 7 tumors were small (less than 20 mm), 28 were medium (20 to 40 mm) and 9 large (greater than 40 mm). Of these 44 patients, 4 had normal hearing preoperatively, 15 had serviceable hearing, 10 poor hearing and 15 no hearing. Postoperative hearing was preserved in 11 cases. Of the 19 patients with normal or serviceable hearing, 8 (42%) had serviceable hearing preserved postoperatively. Hearing preservation was achieved in 43% of the cases of small tumors,
