[Investigation of Helicobacter pylori colonization in pharyngeal and palatine tonsils with rapid urease test and immunohistochemical analysis].

OBJECTIVES: We investigated the presence and frequency of Helicobacter pylori in palatine and pharyngeal tonsil tissues of children. PATIENTS AND METHODS: The study included 20 patients (15 males, 5 females; mean age 8.2 years; range 5 to 13 years) with chronic or recurrent tonsillitis and/or snoring. Of these, 17 patients underwent adenoidectomy and tonsillectomy, two patients underwent adenoidectomy and tonsillectomy with bilateral ventilation tube insertion, and one patient underwent adenoidectomy with bilateral ventilation tube insertion. H. pylori colonization was sought in all tonsillar…

Assessment of the nasopharyngeal airway. A rhinomanometric and radiographic study in children with adenoids.

An assessment of the nasopharyngeal airway in 24 children referred to hospital for adenoidectomy was based on symptoms, radiography and rhinomanometry. The only symptom that was significantly correlated to a small size of the nasopharyngeal airway was Snoring. By radiography, measurement of the airway as well as of the depth of soft tissue of the posterior wall was found well correlated to nasal respiratory resistance which was measured by a new technique. By this, rhinomanometry could be performed in all children down to the age of 5 years. Both radiography and rhinomanometry give valuable pa…

Adenoid hypertrophy affects screening for primary ciliary dyskinesia using nasal nitric oxide.

AIM: In patients with primary ciliary dyskinesia (PCD), the release of nitric oxide (NO) is extremely low by epithelia of the nasopharynx and sinuses. Measurement of nasal NO (nNO) is recommended as a screening test for PCD. The study aimed to evaluate if adenoids affects nNO and may deteriorate the performance of the test. METHODS: In 48 nonallergic patients between 5 and 18 years of age with chronic symptoms of nasal obstruction and indications for adenoidectomy, the measurements of nNO by chemiluminescence analyser and nasal patency by active anterior rhinomanometry were performed both bef…

Does Helicobacter pylori have a role in the development of chronic otitis media with effusion? A preliminary study.

OBJECTIVE: To investigate the possible relationship between chronic otitis media with effusion (COME) and Helicobacter pylori (HP). PATIENTS AND METHODS: Twenty-five patients (7 girls and 18 boys; age range 2-10 years, mean age 7.8 years) with COME with or without adenotonsillary disease were included in this study. Myringotomy was performed on all patients. Nine patients also underwent adenoidectomy, and 10 patients underwent adenotonsillectomy. The adenoidectomy specimens and middle ear effusions were examined for HP colonization by the Campylobacter-like organism (CLO) test. Adenoidectomy …

Endonasal endoscopic nasopharyngectomy for the treatment of nasopharyngeal papillary adenocarcinoma: Report of a rare case.

We report a case of low-grade nasopharyngeal papillary adenocarcinoma in a 9 year-old male that was diagnosed incidentally after an adenoidectomy procedure and treated with endonasal endoscopic nasopharyngectomy without any adjuvant therapy. The patient has been followed up for 3 years with no evidence of recurrence. We point out the importance of preoperative fiberoptic nasopharyngoscopy in the absence of longstanding symptoms in school-aged children and histopathologic examination of adenoidectomy specimens in the presence of atypical findings. We also suggest endonasal endoscopic resection …

Effects of oropharyngeal surgery on velopharyngeal competence.

PURPOSE OF REVIEW: This article addresses the question of whether it is safe to perform pharyngeal surgery on children who are at risk for s … intelligible speech. There are also children, otherwise thought to be normal, who develop VPI after adenoidectomy or tonsillectomy. Greater knowledge of the causes for VPI has led to the strategies limiting the risk of speech disorders after tonsillectomy or adenoidectomy. SUMMARY: Children at risk for VPI who have OSAS may not experience deterioration of thei …

Indications of congenital palatal incompetence before diagnosis.

By questionnaire, parents of 28 children with CPI were asked about pregnancy and delivery; speech, developmental, and health history; and the circumstances of the CPI diagnosis. A normal control group was used for comparison. CPI children had lower birth weights and, during early childhood, were more poorly understood by parents and more frequently had nasalized speech. In 50%, CPI diagnosis was made following adenoidectomy. Data and clinical findings indicate that possible predictors of CPI are unusually defective speech production, particularly characterized by nasalization, as a young child…

Surgical treatments for otitis media with effusion: a systematic review.

BACKGROUND AND OBJECTIVE: The near universality of otitis media with effusion (OME) in children makes a comparative review of treatment modalities important. This study's objective was to compare the effectiveness of surgical strategies currently used for managing OME. METHODS: We identified 3 recent systematic reviews and searched 4 major electronic databases. Eligible studies included randomized controlled trials, nonrandomized trials, and cohort studies that compared myringotomy, adenoidectomy, tympanostomy tubes (tubes), and watchful waiting. Using established criteria, pairs of reviewers…

Why do palatine tonsils grow back after partial tonsillectomy in children?

Within the last decade, adenoidectomy with partial tonsillectomy has been revived in children with obstructive sleep-disordered breathing caused by adenotonsillar hyperplasia, generating debate about remaining tonsillar tissue regrowth. The study examined potential risk factors of the regrowth. Prospective, nonrandomised, case series feasibility study of children meeting the criteria for palatine tonsils regrowth after partial tonsillectomy performed in patients with obstructive sleep-related breathing disorder was carried out. Out of 793 operated children, 294 after adenoidectomy and 373 afte…

Preoperative oral antiemetics for reducing postoperative vomiting after tonsillectomy in children: granisetron versus perphenazine.

Источник пометил публикацию как отозванную

UNLABELLED: In a prospective, randomized, double-blinded trial, we evaluated the efficacy of two antiemetics given orally, granisetron and perphenazine, for preventing postoperative vomiting after tonsillectomy with or without adenoidectomy in children. One hundred pediatric patients, ASA physical status I, aged 4-10 yr, received either granisetron 40 microg/kg or perphenazine 70 microg/kg (n = 50 each) orally 1 h before surgery. We used a standard general anesthetic technique. The rate of complete response, defined as no emesis and no need for rescue antiemetic medication, during 0-3 h after …

Quantitative bacteriology in adenoid tissue.

Over the past several years the indications for adenoidectomy have become increasingly controversial. Attempts to justify the operation in recurrent otitis media by correlating cultures of the nasopharynx with cultures of middle ear fluid have been inconclusive. Using quantitative bacteriologic techniques, we have studied the levels of aerobic and anaerobic bacteria per gram of tissue in adenoids removed from 48 patients. In seven patients, adenoidectomy was performed for nasal obstruction alone, in 17 patients for chronic serous otitis media, and in 24 patients for recurrent suppurative and s…

Von Willebrand disease as cause of unanticipated bleeding following adeno-tonsillectomy.

Von Willebrand disease (vWD) is a frequent autosomal bleeding disorder. We report two unsuspected patients over 6 years of age with this disease operated by tonsillectomy and adenoidectomy (T&A) at our Department. Preoperative hematologic work-up (with activated partial thromboplastic time, prothrombin time, fibrinogen, and platelet count) was normal. Both patients had undergone previous adenoidectomy without complications. In both cases, profuse bleeding was noted in the immediate postoperative period. The therapeutic measures in these situations are discussed and a review of the current lite…

The relationship of adenoid weight to tonsillar weight.

Nearly 70 per cent of contemporary ENT consultants perform adenoidectomy as a routine when removing the tonsils. It has been stated in the literature that adenoid and tonsillar hypertrophy tend to co-exist and so a study was set up to investigate this hypothesis, with the possibility that it may be of diagnostic importance. A series of 45 children admitted for the operations of tonsillectomy and adenoidectomy was investigated. No correlation was found between adenoid weight and tonsil weight. In view of this, it was concluded that tonsil and adenoid hypertrophy do not necessarily co-exist and …

Sleep apnea syndrome associated with a type I Chiari malformation.

We report a 1-year-old girl with a type I Chiari malformation who presented with sleep apnea syndrome. Our patient experienced a change in characteristics of sleep apnea from obstructive to central after adenoidectomy was performed under general anesthesia. The patient also developed dysphagia, which worsened after adenoidectomy. Both disorders greatly diminished after posterior fossa decompression. Our results suggest that type I Chiari malformation should be considered in children with sleep apnea syndrome even when obstructive characteristics predominate. When the malformation is present, t…

An objective assessment of halitosis in children with adenoid vegetation during pre- and post-operative period.

OBJECTIVES: Although most specialists in otorhinolaryngology and pediatrics find halitosis to be a common problem in children with adenoid hypertrophy, there are no objective data on this topic in the literature. Whether adenoid hypertrophy is a risk factor for halitosis or whether halitosis is a sign of adenoid hypertrophy remains unclear. Thus, the aim of this study was to investigate whether children diagnosed with adenoid hypertrophy have a higher probability of halitosis than do children in the normal population and whether adenoidectomy can decrease oral malodor. METHODS: Forty children…
