Total and free cortisol responses and their relation to outcomes after cardiopulmonary bypass in infants.

BACKGROUND: Hypothalamic-pituitary-adrenal (HPA) axis dysfunction may be partially responsible for the hemodynamic instability experienced by infants after cardiopulmonary bypass (CPB). We report the full spectrum of the HPA response surrounding CPB for infant congenital cardiac surgery. METHODS: We enrolled 84 infants who received 1 mg/kg of dexamethasone before initiation of CPB. Total cortisol (TC), free cortisol (FC), adrenocorticotropic hormone (ACTH), and corticosteroid-binding globulin (CBG) were measured at 3 time points: immediately before CPB (TP1), on intensive care unit arrival (T

Plasma cortisol levels in Guillain-Barré syndrome.

OBJECTIVE: To assess the relationship between plasma cortisol level and Guillain-Barré syndrome-related complications, notably respiratory failure. One third of patients with Guillain-Barré syndrome develop respiratory failure, which is predicted by few early indicators. Adrenal function has rarely been studied in Guillain-Barré syndrome. DESIGN: Prospective study. SETTING: Intensive care unit in a teaching hospital. PATIENTS: Patients with Guillain-Barré syndrome referred to our unit (n = 102). INTERVENTIONS: Plasma cortisol levels were measured before baseline and 60 mins after corticotr

Approach to the Virilizing Girl at Puberty.

UNLABELLED: Virilization is the medical term for describing a female who develops characteristics associated with male hormones (androgens) at any age, or when a newborn girl shows signs of prenatal male hormone exposure at birth. In girls, androgen levels are low during pregnancy and childhood. A first physiologic rise of adrenal androgens is observed at the age of 6 to 8 years and reflects functional activation of the zona reticularis of the adrenal cortex at adrenarche, manifesting clinically with first pubic and axillary hairs. Early adrenarche is known as "premature adrenarche." It is mos

Somatic mutations in the KCNJ5 gene raise the lateralization index: implications for the diagnosis of primary aldosteronism by adrenal vein sampling.

CONTEXT: Somatic mutations in the selectivity filter of KCNJ5 K(+) channel were found to be associated with higher plasma aldosterone concentrations in the patients with an aldosterone-producing adenoma (APA). OBJECTIVE: We investigated whether plasma aldosterone levels and the lateralization index are higher from the side with the APA with the mutation, as compared with those without the mutation. DESIGN: From 170 consecutive APA patients with comprehensive clinical and KCNJ5 data and a conclusive diagnosis, we recruited 91 patients with adrenal vein sampling and follow-up data. We measured

[Cortisol levels in cord blood with various violations of neonatal adaptation].

Shifting extra uterine existence is a serious test of maturity of all functional systems of the newborn, so in recent years, the more attention is payed for the adaptation of the newborn. It is known that the pathological course of pregnancy is a violation of the functional state of the adrenal cortex, and prolonged exposure of the pathological factor leads to the inhibition of adrenocortical function and a decrease of the adaptive capabilities of a newborn. Therefore, serum cortisol levels could be a predictor of complications in preterm infants. Based on this, it seems appropriate to study c

Shilajit attenuates behavioral symptoms of chronic fatigue syndrome by modulating the hypothalamic-pituitary-adrenal axis and mitochondrial bioenergetics in rats.

ETHNOPHARMACOLOGICAL RELEVANCE: Shilajit has been used as a rejuvenator for ages in Indian ancient traditional medicine and has been validated for a number of pharmacological activities. AIM OF THE STUDY: The effect of processed shilajit which was standardized to dibenzo-α-pyrones (DBPs;0.43% w/w), DBP-chromoproteins (DCPs; 20.45% w/w) and fulvic acids (56.75% w/w) was evaluated in a rat model of chronic fatigue syndrome (CFS). The mitochondrial bioenergetics and the activity of hypothalamus-pituitary-adrenal (HPA) axis were evaluated for the plausible mechanism of action of shilajit. MATERI

Post-awakening cortisol and resilience: unravelling their impact on cognitive decline.

Age-associated cognitive decline is a growing social problem and a major concern for older adults, highlighting the need to identify modifiable factors to mitigate it. The basal functioning of the hypothalamic-pituitary-adrenal (HPA) axis, and, especially, the post-awakening cortisol levels, seems to play a key role in cognitive performance related to functions dependent on the prefrontal cortex and hippocampus. Regulating these cortisol levels is crucial in this context, and resilience has been recognized as vital for successful aging and cognitive functioning. This study was designed to exam

Early androgen treatment decreases cognitive function and catecholamine innervation in an animal model of ADHD.

The spontaneously hypertensive rat (SHR) has been used as an animal model of attention deficit hyperactivity disorder (ADHD). The present study was designed to determine whether exposure to elevated androgen levels early in development demonstrated impairments in cognitive functioning, neuroendocrine control, and brain development parallel to those seen in ADHD children. The animals (SHR and Wistar (WKY) controls) were implanted with testosterone on postnatal day 10 and tested for behavior in a spatial cognition paradigm on postnatal day 45. Plasma samples were collected for determination of a

Takotsubo cardiomyopathy associated with adrenal insufficiency in the context of long-term steroid use mimicking acute coronary syndrome.

Takotsubo cardiomyopathy (TCMP) is an important, though under-recognised, syndrome which mimics acute coronary syndrome (ACS) presenting with similar clinical, biochemical and ECG features. A 68-year-old man was referred as ACS for emergency coronary angiography; however, a history of lethargy, weight loss and electrolyte abnormalities prompted further investigations. Angiography was postponed, adrenal insufficiency confirmed and steroid replacement commenced. Echocardiography demonstrated reduced left ventricular (LV) function (45%) with regional wall motion abnormalities, although angiograph

The Association Between Heavy Alcohol Use and Lung Function: A Clinical Study of Adult-Onset Asthma.

BACKGROUND: In animal studies, alcohol exposure has been reported to damage lungs. However, no human studies exist on the association between alcohol consumption and lung function in asthma. OBJECTIVE: To evaluate the association between heavy alcohol consumption and lung function decline in patients with adult-onset asthma. METHODS: In the Seinäjoki Adult Asthma Study, 201 patients were observed for 12 years after the diagnosis of new-onset asthma. Based on detailed questionnaires and laboratory indices of heavy drinking, patients were classified as heavy alcohol consumers or non-heavy alco
