Comparison of the efficacy of eutectic mixture of local anesthetics (EMLA) and dorsal penile nerve block (DPNB) in neonatal circumcision.

BACKGROUND: Neonates feel pain. There is a concern among practitioners that pain of injecting analgesics to neonates prior to circumcision could as well be the same as the pain of the procedure. This has made many reluctant to offer effective analgesia for circumcision. If eutectic mixture of local anesthetics (EMLA) provides analgesia comparable to dorsal penile nerve block (DPNB), it will obviate needle prick and encourage analgesia use in neonatal circumcision. AIM: To determine how the analgesic efficacy of EMLA compares with that of DPNB in neonatal plastibell circumcision. METHODS: A p

[Beginning of continuous wound infusion with local anesthetics : With special emphasis on the contributions from Walter Capelle and Ewald Fulde].

Wound infusion with local anesthetics is a proven and safe analgesic procedure for modern perioperative patient care. Even the pioneers of local anesthesia practiced wound analgesia and emphasized the shortcomings of "single-shot" wound infusions. At the same time, they drew attention to the importance of long-lasting pain relief to prevent sequelae, especially after upper abdominal surgery with pneumonia, embolic events or postoperative ileus. In the early 1930s there were first sustained efforts to improve the efficiency and quality of pain therapy, especially after upper abdominal surgery b

Effect of Repetitive Intracutaneous Injections with Local Anesthetics and Steroids for Acute Thoracic Herpes Zoster and Incidence of Postherpetic Neuralgia.

BACKGROUND: Treatment of established postherpetic neuralgia (PHN) is difficult and often disappointing. In this study, we assessed the efficacy of repetitive intracutaneous injections with local anesthetics and steroids in acute thoracic herpes zoster (HZ) pain, herpetic eruption, and incidence of PHN. METHODS: Ninety-three patients with acute thoracic HZ were randomly assigned to receive a standard treatment of antiviral medication with p.o. analgesics or the standard treatment with the addition of repetitive intracutaneous injections of a local anesthetic and steroid mixture. Patients were

[Local anesthesia in the children undergoing the fibroendoscopic study of the nasal cavity, nasopharynx, and larynx: are topical anesthetics needed?].

This prospective randomized study with double blind control was designed to evaluate the effectiveness of various anesthetic techniques employed prior to fibroendoscopy of the nose, nasopharynx, and larynx of the children. The study included 160 children at the age varying from 3 to 14 (mean 7.4±2.96) years randomly allocated to four statistically comparable groups matched for age and sex. The following preparations were used to treat the children prior to fibroendoscopy: physiological solution (group 1), a 0.05% xylometazoline solution (group 2), a 10% lidocaine solution (group 3), and a mixt

A systematic review of the effects of adding neostigmine to local anesthetics for neuraxial administration in obstetric anesthesia and analgesia.

BACKGROUND: Drugs used in obstetric patients must accomplish two goals: efficacy and safety for both mother and fetus. Neostigmine has been co-administered epidurally and intrathecally with local anesthetics and other adjuncts in the obstetric setting. The aim of this meta-analysis was to assess the efficacy and incidence of adverse events related to the use of neostigmine in obstetric anesthesia. METHODS: A meta-analysis of randomized-controlled human trials was conducted using the data sources Google Scholar and PubMed (updated 1 November 2014). Inclusion criteria were: random allocation to

Absorption of lidocaine and prilocaine after application of a eutectic mixture of local anesthetics (EMLA) on normal and diseased skin.

A eutectic mixture of 5% lidocaine and prilocaine was applied under occlusion for 1 or 2 hours on 25-100 cm2 areas of normal and diseased skin, and the absorption was followed by measuring the concentrations of the drugs in the draining vein and the general circulation at different time intervals after the application. The analgesic and vascular effects in the skin were also recorded. When the mixture was applied on normal skin the absorption was more rapid from the face than from the forearm. The absorption from diseased skin was faster than that from normal skin, with higher plasma concentra

Opioids added to local anesthetics for single-shot intrathecal anesthesia in patients undergoing minor surgery: a meta-analysis of randomized trials.

Opioids are widely used as additives to local anesthetics for intrathecal anesthesia. Benefit and risk remain unclear. We systematically searched databases and bibliographies to February 2011 for full reports of randomized comparisons of any opioid added to any intrathecal local anesthetic with the local anesthetic alone in adults undergoing surgery (except cesarean section) and receiving single-shot intrathecal anesthesia without general anesthesia. We included 65 trials (3338 patients, 1932 of whom received opioids) published between 1983 and 2010. Morphine (0.05-2mg) and fentanyl (10-50 μg)

Long-term evaluation of usefulness of skin and incremental challenge tests in patients with history of adverse reaction to local anesthetics.

A variety of adverse reactions to local anesthetics has been described, some of which are thought to be allergic. Different protocols of prick and intradermal skin tests as well as subcutaneous challenge tests are used to select a local anesthetic which can safely be used. Their long-term effectiveness has not yet been assessed. Twenty-eight patients with a history of adverse reaction to local anesthetics were evaluated over a 3-year period. Loss of consciousness occurred in eight patients, skin reaction in nine, and vagal symptoms in eight. Various reactions were recorded in the remaining thr

Rethinking the Boundaries of Local Anesthetic Systemic Toxicity in the Era of Sustained-Release Formulations.

Sustained-release local anesthetic formulations introduce prolonged and dynamic exposure patterns that challenge current frameworks for assessing local anesthetic systemic toxicity (LAST). Recent pharmacovigilance findings, pharmacokinetic evidence, and reports of delayed and recurrent LAST further support the need to re-evaluate LAST across three conceptual boundaries. First, the exposure boundary challenges the use of recommended maximum doses as stand-alone safety thresholds. Co-administration, formulation stability, and the local tissue microenvironment may influence drug release and syste

Local anesthetic systemic toxicity with liposomal bupivacaine: a case series and literature review.

BACKGROUND: Local Anesthetic Systemic Toxicity (LAST) is an uncommon complication, with manifestations ranging from mild, non-specific neurological symptoms to severe cardiac arrest. Toxicity specifically associated with liposomal bupivacaine is even more rarely documented in the literature. We report two cases of LAST following the administration of liposomal bupivacaine and provide a review of previously reported cases. CASE PRESENTATION: Case 1: A 55-year-old male received 266 mg of liposomal bupivacaine via intercostal nerve block during thoracoscopic lobectomy. During skin closure, he de

Regional Anesthesia in Patients with Hepatic Failure: How Risky is it to Administer Local Anesthetic?

Patients with advanced liver disease challenge anesthesiologists, especially those providing regional anesthesia for postoperative pain management. Coagulopathy, encephalopathy, sepsis and other comorbidities complicate care. Liver failure lowers albumin and α1-acid glycoprotein and slows hepatic clearance, increasing the unbound fraction and effective exposure to amide local anesthetics. Risks are magnified by acidosis, low cardiac output, low body mass index, sepsis, and concurrent renal dysfunction. Neuraxial options are often limited by coagulopathy and the risk of catastrophic neurologic

Spatiotemporal dynamics of local anesthetic diffusion in nerve revealed by a 2D computational model.

Despite their extensive clinical use, the intraneural pharmacokinetics of local anesthetics, including the mechanisms determining their onset and duration, remain incompletely understood, particularly under pathological conditions. We developed a detailed, spatially compartmentalized computational model of human peripheral nerve fascicles to simulate the diffusion, accumulation, and clearance kinetics of lidocaine and bupivacaine under physiological and acidic conditions. The model integrates nerve fiber architecture, extracellular fluid compartments, and capillary-mediated clearance, paramete

Local anesthetic systemic toxicity induced by penile nerve block: A systematic review.

BACKGROUND: Local anesthetics are widely used by non-anesthesiologists to provide analgesia. Penile nerve blocks are used for analgesia during urological surgery. Local anesthetic systemic toxicity (LAST), which is rare but likely fatal, is often caused by a penile nerve block. However, there has been no analysis of cases of LAST induced by a penile nerve block. This Preferred Reporting Items for Systematic Reviews and Meta-Analyses-compliant systematic review aimed to analyze case reports involving LAST induced by a penile nerve block, with a particular focus on the presumed causes, local ane

Elective Ambulatory Unit: Experience of Local Anesthetic Only Surgery During the Pandemic.

BACKGROUND: The second COVID-19 wave severely limited access to elective surgery. METHODS: Between December 2020 and May 2021, 530 patients underwent a procedure in the elective ambulatory unit (EAU), a walk-in and walk-out model of surgery, and we used a prepandemic cohort of day-case patients for comparison. RESULTS: We have had no confirmed cases of COVID-19 transmission on-site. The infection rate for EAU and day-case units for carpal tunnel decompression was 1.36% and 2%, respectively, and this difference was not significant, P = .696. Patient satisfaction was excellent at 9.8 of 10. Th
