Effect of ankle orthoses on functional performance for individuals with recurrent lateral ankle sprains.

Ankle orthoses are commonly used for protection against initial ankle sprain injuries and for protection against reinjury. The purpose of this study was to compare the effects of the DonJoy Ankle Ligament Protector (ALP) and the Aircast Sport-Stirrup (AS) on the functional performance tasks of a 40-meter sprint, a figure-of-eight run, and standing vertical jump. Subjects were 14 males and nine females who reported a history of two or more lateral ankle sprains of the same ankle and no sprains of the contralateral ankle. Each subject performed all functional tasks on the first test day with bot…

Ankle arthrodesis in the management of traumatic ankle arthrosis: a long-term retrospective study.

A retrospective study was carried out of 48 patients who underwent ankle arthrodesis for tramatic arthritis from January 1967 through January 1976. Following ankle fusion, the most common complaint was subtalar pain and the most frequent finding was limitation of subtalar motion (56%). Subtalar or triple arthrodesis was required in 12 patients, with good results in the majority of the cases. An infection rate of 22% was encountered, felt to be consequent to the failure to properly employ prophylactic antibiotics; which, when employed, were associated with less than 6% incidence of sepsis 69% o…

Acute Effect of Ankle Kinesio and Athletic Taping on Ankle Range of Motion During Various Agility Tests in Athletes With Chronic Ankle Sprain.

BACKGROUND: Taping is a preventive measure commonly used for protecting and strengthening the ankle joint to prevent further musculoskeletal damage. Ankle taping prevents excessive range of motion (ROM) of the ankle joint and allows the improvement of proprioception to adjust balance. Appropriate ankle stability is essential for various activities, such as sprinting, turning, cutting, and jumping, which are associated with agility. AIM: To assess the acute effect of Kinesio taping and athletic taping on the ankle ROM of athletes with chronic ankle sprain during various agility tests that incl…

Total ankle replacement versus nonoperative management for end-stage ankle osteoarthritis: A comparative analysis.

BACKGROUND: The optimal management of end-stage ankle osteoarthritis remains debated. This study compared short-term outcomes between non-operative treatment, total ankle replacement (TAR), and cases in which indicated surgery was delayed. METHODS: In this secondary analysis of prospectively collected data, treatment survival, Foot and Ankle Outcome Score (FAOS), and patient satisfaction were assessed at baseline and at 1- and 2-year follow-up. RESULTS: In a total of 316 patients one-year treatment survival was highest for TAR (99.3%), followed by NOM (94.4%) and delayed surgery (80.9%). TAR…

Operative Management of Lateral Ankle Instability: Examination Under Anesthesia, Ankle Arthroscopy and Stabilization Technique.

BACKGROUND: Ankle sprains are common injuries that can result in chronic ankle instability. INDICATIONS: Indications for surgical management include recurrent instability, failed conservative management for 3 to 6 months, and inability to return to play or activity. TECHNIQUE DESCRIPTION: The patient's ankle is initially examined under anesthesia with anterior drawer testing to confirm the diagnosis of ankle instability. Ankle arthroscopy is then preformed to identify and treat additional intra-articular pathology prior to modified Broström repair. During arthroscopy, care is taken to protec…

Novel passive ankle-foot prosthesis mimics able-bodied ankle angles and ground reaction forces.

BACKGROUND: During gait, the human ankle both bends with ease and provides push-off forces that facilitate forward motion. The ankle is crucial for support, stabilization, and adapting to different slopes and terrains. Individuals with lower limb amputation require an ankle-foot prosthesis for basic mobility. METHODS: Inspired by the role of the ankle-foot in an able-bodied gait, the 3D printed Compliant and Articulating Prosthetic Ankle (CAPA) foot was designed. It consists of four articulating components connected by torsion springs and produces forces that are dependent on the ankle angle.…

Predictors of peroneal pathology in Broström-Gould ankle ligament reconstruction for lateral ankle instability.

BACKGROUND: Chronic ankle instability has a well-known association with intra- and extraarticular ankle pathologies, including peroneal tendonitis and subluxation. Patients with peroneal pathology are at risk for failure of conservative treatment for their ankle instability, thus identifying these patients is important and helps to guide management. There has been no literature looking at, in patients with chronic ankle instability, which associated ankle pathologies and patient characteristics are predictive of peroneal pathology. METHODS: A retrospective chart review was performed on all pa…

Talofibular interval changes after acute ankle sprain: a stress ultrasonography study of ankle laxity.

INTRODUCTION: Quantifying talocrural joint laxity after ankle sprain is problematic. Stress ultrasonography (US) can image the lateral talocrural joint and allow the measurement of the talofibular interval, which may suggest injury to the anterior talofibular ligament (ATFL). The acute talofibular interval changes after lateral ankle sprain are unknown. METHODS: Twenty-five participants (9 male, 16 female; age 21.8 ± 3.2 y, height 167.8 ± 34.1 cm, mass 72.7 ± 13.8 kg) with 27 acute, lateral ankle injuries underwent bilateral stress US imaging at baseline (<7 d) and on the affected ankle at 3 …

Effect of postoperative early ankle motion exercise using hinged ankle-foot orthoses in clubfoot.

The authors proposed early application of hinged ankle-foot orthoses for improving postoperative range of ankle motion after the complete subtalar release operation for clubfoot. Forty-eight patients (74 feet) with clubfoot were divided into two groups: group A patients (20 feet) were immobilized in plaster casts for 6 weeks after surgery, followed by non-hinged ankle-foot orthoses, and group B patients (54 feet) were immobilized in plaster casts for 4 weeks after surgery, followed by hinged ankle-foot orthoses. The range of ankle plantarflexion in group B showed a significant improvement comp…

Are There Differences in Ankle Mechanics After Total Ankle Arthroplasty in Patients Suffering From Post-fracture vs Post-sprain End-Stage Ankle Osteoarthritis?

BACKGROUND: To our knowledge, no study is available comparing the change in ankle mechanics during gait after total ankle arthroplasty (TAA) based on the origin of the osteoarthritis. As the nature of trauma is different in patients sustaining post-fracture ankle osteoarthritis (PFOA) from those sustaining post-sprain ankle osteoarthritis (PSOA), it could be expected that the outcomes of TAA, in terms of ankle mechanics during gait, would be different in the 2 groups. A prospective matched comparative study was therefore performed to investigate whether patients sustaining PFOA had different o…

Ankle Brachial Index: Quick Reference Guide for Clinicians: WOCN Society Ankle Brachial Task Force.

Lower extremity arterial disease (LEAD) affects millions, particularly the elderly and those with diabetes mellitus. The Ankle Brachial Index (ABI) compares systolic pressures in the ankle and brachial arteries to assess lower extremity arterial perfusion. Despite its benefits, the ABI has limitations like low sensitivity in asymptomatic patients and inaccuracies due to arterial calcification. An ABI is beneficial for screening for LEAD in individuals with lower extremity wounds, assessing arterial blood flow before compression therapy, and evaluating wound healing potential. Understanding ABI…
