Anti-ulcer therapy. Past to present.

Until 1950 the clinical treatment of peptic ulcer disease relied on dieting and antacids. However, recent controlled studies suggest that the natural course of peptic ulcer disease is not affected by diet. Antacids are primarily used to relieve distress, although high- and low-dose antacid regimens have been reported to promote duodenal ulcer healing. Initial favorable reports following the introduction of synthetic anticholinergic drugs have not been confirmed. Pirenzepine is an anticholinergic compound with a specific action on the muscarinic receptors of the parietal cells. Although pirenze…

Control of Constipation in Patients Receiving CHOP or CHOP-Like Chemotherapy Regimens for Non-Hodgkin's Lymphoma.

Management of constipation in patients receiving cyclophosphamide, doxorubicin, vincristine and prednisolone (CHOP) or CHOP-like chemotherapy regimens is important for prevention of paralytic ileus. We reported earlier that the laxative action of magnesium oxide is reversed by the concomitant use of antacids in cancer patients receiving opioid analgesics. Here, we assessed the prevalence of prophylactic laxative medication for the control of constipation in patients receiving CHOP or CHOP-like regimens for non-Hodgkin's lymphoma. Data obtained from 211 eligible patients were retrospectively an…

Suppressive effect of antiulcer agents on granulocytes--a role for granulocytes in gastric ulcer formation.

Many clinicians have believed that H2-blockers and proton pump inhibitors ameliorate gastric ulcers via their antacid function. We examined the effects of these antacids on granulocytes. Gastric ulcer patients were administered an H2-blocker or proton pump inhibitor for a week and the number of granulocytes and the superoxide production were examined. To determine the trafficking of granulocytes, mice were exposed to restraint stress for 24 hr. The H2-blocker decreased the number of granulocytes, while the proton pump inhibitor suppressed their superoxide production in humans and mice. The maj…

Gastrointestinal tract complications of nonsteroidal anti-inflammatory drug treatment in rheumatoid arthritis. A prospective observational cohort study.

BACKGROUND: Gastrointestinal tract (GI) complications associated with nonsteroidal anti-inflammatory drug (NSAID) use are the most common serious adverse drug reactions in the United States. Nonsteroidal anti-inflammatory drugs cause both minor GI side effects such as abdominal pain and vomiting and serious GI events such as ulcers and bleeding. This study evaluates the event rates for all NSAID-induced GI complications in patients with rheumatoid arthritis, describes the time course of these events, and evaluates the role of prophylactic therapy with antacids and H2 receptor antagonists. MET…

Development of a Raft-Forming System Using Plantago major Mucilage as a Natural Polymer for Sustained Gastric Release of Artemisia annua L. Phenolic Extract.

Background/Objectives: Alginate-based rafts are typically used for antacids and provide a floating, gastric-retaining gel in the stomach that acts as a barrier to the entrance of stomach acid into the esophagus. Various technologies have been developed to enhance the sustained release of drugs and bioactive components and overcome challenges associated with gastric retention. Modern methods, such as the alginate raft, not only function as an antacid in gastroesophageal reflux disease but also can serve as a delivery system that extends the drug release time in gastric medium. This study assess…

High-dose proton-pump inhibitors as a diagnostic test of gastro-oesophageal reflux disease in endoscopic-negative patients.

BACKGROUND: To evaluate a high dose of a proton-pump inhibitor as a diagnostic test in endoscopy-negative patients presenting with symptoms indicating gastro-oesophageal reflux disease. METHODS: 64 patients were studied in a prospective, randomized, double-blind study, using a cross-over design. After a run-in period with the diary registration of basic GORD symptoms and recording of the consumption of antacid tablets, the patients were given either 60 mg of lansoprazole once daily or placebo in randomized order. Symptoms were recorded, as well as antacid tablets taken in order to relieve pai

Double-blind, placebo-controlled study of ranitidine for gastroesophageal reflux symptoms during pregnancy.

OBJECTIVE: To determine whether ranitidine (Zantac) taken once or twice daily is effective for relieving symptoms of gastroesophageal reflux among pregnant women who had failed conservative measures. METHODS: Volunteers with heartburn despite antacids were sought among our obstetrics clinic population for this double-blind, placebo-controlled, triple crossover trial. After a baseline week, 20 patients were randomized to receive the three following weekly regimens: ranitidine 150 mg twice daily, placebo in the morning and ranitidine 150 mg in the evening, or placebo twice daily. Anti-heartburn effects of a fenugreek fiber product.

Frequent heartburn occurs in many people, some of whom prefer alternative treatments over conventional drugs. In a pilot study of subjects with frequent heartburn, 2 week intake of a fenugreek fiber product, taken 30 min before two meals/day, diminished heartburn severity. This conclusion was based on symptom diary results and reduced the use of a mild antacid as a rescue medicine. Improvement for each of the 2 weeks was judged by comparison with results from a baseline week. Placebo also produced some statistically significant effects, but the fiber product's effects differed significantly fr

Chronic alcoholic gastritis. Roles of alcohol and Helicobacter pylori.

We assessed the relative roles of alcohol and infection with Helicobacter pylori in the pathogenesis of chronic gastritis in alcoholic patie … iated with almost complete normalization of histologic findings. Four control subjects who received antacids alone showed no improvement. Dyspeptic symptoms in H pylori-positive patients significantly improved after elimination of this organism, whereas there was no change with antacid treatment.

Effect of acid inhibition on gastric endocrine cells.

There is increasing evidence that the numbers of antral G cells and of fundic argyrophil (ECL) cells are influenced by agents that inhibit gastric acid secretion. Antral G cells increase in states of achlorhydria in man and animals provided atrophic antral gastritis is absent. In rats, treatment with substituted benzimidazoles like omeprazole and BY 308 increase G-cell densities dose-dependently. Even high doses of histamine H-2 antagonists and antacids increase antral G-cell densities. In contrast to G cells, antral D cells decrease in these instances. Fundic ECL cells increase in all experim…

15 (R)-15-methyl prostaglandin E2 does not prevent gastrointestinal bleeding in seriously ill patients.

A prospective, randomized trial was designed to compare the relative efficacy of 15 (R)-15-methyl prostaglandin E2 with antacid (usually Mylanta II) in 46 patients admitted to a respiratory-surgical intensive care unit. Bleeding was assessed by a modification of the Hemoccult slide test. Three of 22 patients in the antacid group bled, and 12 of 24 patients in the prostaglandin group bled, for a highly significant difference (p = 0.008). Patients in whom prophylaxis failed tended to have a greater number of risk factors. Other prostaglandin analogues that do not require conversion from an inact

Comparative randomized, double-blind study of oxmetidine versus cimetidine for short-term treatment of duodenal and prepyloric ulceration.

101 patients with endoscopically verified duodenal or prepyloric ulcerations were treated in a 4- to 8-week double-blind randomized trial with oxmetidine 400 mg twice daily (51 patients) or cimetidine 1 g daily (50 patients). Both groups had free access to antacid tablets for symptomatic relief. There were no significant differences between the two treatment alternatives with regard to ulcer healing, relief of ulcer pain, or antacid consumption. The ulcer healing rate after 4 weeks of treatment was 80% in the oxmetidine group and 74% in the cimetidine group and, after 8 weeks, 92% and 86%, res

[Treatment of Helicobacter pylori infections].

H. pylori infection is a very common infection of the human stomach, related to various diseases such as gastritis, ulcerative disease of the duodenum and stomach, mucosa associated lymphoid tissue lymphomas and gastric cancer. It is generally accepted that the majority of related diseases is curable once H. pylori infection is successfully eradicated. H. pylori eradication will only be achieved after administration of combination therapies including antibiotics and antacids. Resistance to metronidazole and clarithromycin is an emerging problem, leading to impairment of eradication efficacy, i…

Considerations for selection of parenteral histamine (H2)-receptor antagonists.

Parenteral histamine (H2)-receptor antagonists are safe and effective therapy for the prophylaxis of stress-related erosive syndrome. Of the three agents available, parenteral cimetidine is the most widely studied; parenteral formulations of ranitidine and famotidine have not been investigated as extensively. Parenteral cimetidine appears to be effective in reducing incidence and complications associated with gastrointestinal hemorrhage caused by stress-related erosive syndrome, although its effects on bleeding that has already begun are not clear. Antacids are also effective in decreasing the…
