Clinical results of management of aphakia with Soemmering ring in microcornea.

PURPOSE: To present clinical outcomes of glued IOL scaffold for management of aphakia with Soemmering ring in eyes with microcornea. DESIGN: Prospective, Interventional series. SETTING: Private practice. METHODS: 11 eyes of 6 patients (3 males, 3 females) with microcornea underwent glued IOL scaffold procedure for aphakia associated with Soemmering ring. All patients had undergone initial cataract surgery at age less than 10 years and were rendered aphakic. MAIN OUTCOME MEASURES: Uncorrected and best-corrected visual acuity (UCVA; BCVA), Specular count and IOL centration. RESULTS: The mea…

Implantation of a scleral fixated (Carlevale) EDOF IOL in aphakia.

PURPOSE: To present a case of lens dislocation in which an Extended Depth of Focus (EDOF) scleral fixated (Carlevale) intraocular lens (IOL) … fficient scleral fixation using the Carlevale FIL SSF IOL seems like a promising option in cases of aphakia or zonular dehiscence. EDOF lens technology is widely used in routine cataract surgery for demanding patients who require a wider range of vision without glasses. Combining these two technologies lead to a lens design (Carlevale FIL SSF EVOLVE) that …

The simultaneous treatment of post-traumatic aphakia, aniridia, and astigmatism.

To present a case of a 20-year-old female who underwent a novel surgical technique for post-traumatic aphakia, aniridia, and astigmatism. Prior to surgery, the artificial iris (AI) (Customflex Artificial Iris, HumanOptics) was sutured to a toric intraocular lens (IOL) (Carlevale, Soleko), folded, and then implanted in the sulcus through a small corneal incision. The procedure was complication-free and resulted in favorable functional and cosmetic outcomes. The Carlevale IOL provides stable support for the AI, effectively addressing post-traumatic aphakia, aniridia, and astigmatism. This techni…

Management of Contact Lenses and Visual Development in Pediatric Aphakia.

Congenital cataract is among the main causes of treatable vision loss in childhood. The first weeks and months of life are a critical time for the development of vision. Therefore, early cataract surgery and effective multifaceted treatment of the resulting aphakia in the early stages of life are of great value for the management of vision development. Among the treatment models, contact lenses (CL) have an important place in infancy and early childhood up to the age of 2 years. Although good visual gains were not considered very likely, especially in unilateral aphakia, important steps have b…

New Device for the Correction of Aphakia Without Capsular Support.

PURPOSE: This study describes a prototype developed for aphakia without capsular support (AWCS) and its proof of concept. METHODS: This descriptive study used a 3D software to create and analyze virtual prototypes before manufacturing. A nylon-6/nylon-6.6 copolymer filament and a 3D printer were used for prototyping. A device implantation technique was developed using a 23-gauge hypodermic needle. Two opposing markings, 2 mm posterior to the limbus, were made to determine the location of the scleral punctures and the final position of the device. After adequate centralization and positioning …

Rare association of familial aniridia, microcornea with myopia and aphakia.

Aniridia is a rare congenital malformation that may be associated with various ocular and systemic manifestations. We describe two cases of familial total aniridia associated with microcornea, high myopia and dislocated lens. No systemic abnormality was noted in any of the cases.

[The evolution of cataract surgery, extra- and intraocular correction of aphakia].

The evolution of cataract surgery during the recent 2 centuries, and especially in the last 50 years, witnessed essential changes--from reclination of the lens and a traditional extracapsular cataract extraction to cataract microsurgery including phacoemulsification. A lack of the lens in the eye, i.e. aphakia, is considered to be a serious complication. The optic extraocular correction of aphakia is implemented mainly by the use of spectacles and contact lenses, which are not deprived of essential shortcomings. H. Ridley was the first to implant, in 1949, a polymethylmethacrylate (PMMA) intra…

Optical considerations in the contact lens correction of infant aphakia.

BACKGROUND: Correction of infant aphakia with conventional soft and rigid designs induces significant amounts of positive spherical aberration. METHODS: Different levels of positive spherical aberration were induced by rigid lenses positioned close to the eye. Visual acuity and depth of focus were measured for twelve young adult subjects. RESULTS: Positive spherical aberration acts to produce an increase in relative depth of focus, but because of the reduction of best visual acuity, there is a concurrent decrease in absolute depth of focus. As the spherical aberration increases, there is a p…

Effects of aphakia on the geniculostriate system of infant rhesus monkeys.

The effects on the visual system of rearing rhesus monkeys with monocular aphakia, corrected with extended-wear contact lenses, were assessed with anatomical, electrophysiological and behavioral methods. The major finding was that the effects of the various treatments on the aphakic eye varied in degree depending upon the amount of focused pattern input received by the aphakic eye compared to its fellow eye. The behavioral, electrophysiological and anatomical assessments of the treatment effects on the aphakic eyes correlated closely with each other. Because this experimental paradigm is simil…

The correction of unilateral aphakia in children treated for orbital rhabdomyosarcoma.

The survival rate of children with localized orbital rhabdomyosarcoma is now greater than 90% 3 years after diagnosis as a result of advances in radiation and chemotherapy. Ninety percent of these children develop cataracts within 1 to 4 years after the completion of radiotherapy. The correction of aphakia in these children is complicated by the concurrent keratoconjunctivitis and dryness associated with radiotherapy. Three patients with a diagnosis of orbital rhabdomyosarcoma underwent treatment for uniocular cataract. Two of the patients were unable to use extended wear contact lenses. Both …

[Prognostication of results in correction of aphakia by lamellar refractive keratoplasty].

The authors analyze the possibility of predicting the results of aphakia correction by interlamellar refraction keratoplasty from one of the eye parameters measured before surgery, the anterior corneal surface radius. The prognosis efficacy has improved by 10% vs. the prediction from the mean value; this is insufficient for practical application of the method. The prediction may be made more accurate if the number of the eye parameters measured before surgery is increased and the measurements are more precise.
