[Sleep apnea-hypopnea syndrome and stroke].

For many years attention has been drawn to the frequent association between sleep apnea-hypopnea syndrome and stroke. Numerous and very different epidemiological studies are available that point to a possible causal relation. In a parallel way, there has been an increase over these years in the knowledge of the different intermediate physiopathological mechanisms by which apnea could theoretically favour the appearance of cerebral ischemia. An increase in arterial pressure, the appearance of arrhythmias, hemodynamic changes in cerebral circulation and a prothrombotic state are outstanding amon…

[Sleep apnea syndrome and heart failure].

More than half of all patients with stable heart failure are affected by sleep apnea syndrome. The mechanism of apnea may be either obstructive, by collapse of the upper airways, or central with nocturnal Cheyne-Stokes respiration. Changes in intrathoracic pressure, awakening and hypoxemia all lead to an increase in left ventricular afterload and to sympathetic activation, which can aggravate heart failure and prognosis. Treatment with positive pressure ventilation improves symptoms as well as cardiac function and prognosis. Sleep respiratory disorders are underdiagnosed and undertreated in pa…

Sleep apnea: clinical investigations in humans.

Sleep apnea syndrome (SAS), a common disorder, is characterized by repetitive episodes of cessation of breathing during sleep, resulting in hypoxemia and sleep disruption. The consequences of the abnormal breathing during sleep include daytime sleepiness, neurocognitive dysfunction, development of cardiovascular disorders, metabolic dysfunction, and impaired quality of life. There are two types of SAS: obstructive sleep apnea syndrome (OSAS) and central sleep apnea syndrome (CSAS). OSAS is a prevalent disorder in which there is snoring, repetitive apneic episodes, and daytime sleepiness. Anato…

Sleep apnea: implications for heart failure.

It is likely sleep apnea is highly prevalent in patients with congestive heart failure (CHF). However, as awareness is low and consensus guidelines do not exist, sleep apnea is not routinely screened for or diagnosed in CHF practice. Untreated sleep apnea may promote fatigue as well as left ventricular dysfunction, disease progression, and increased mortality. Available screening tools lack sensitivity or specificity, and there are insufficient numbers of sleep laboratories to accommodate the potentially large number of patient referrals with CHF for definitive diagnosis. In CHF patients with …

Obstructive sleep apnea and cardiovascular risk.

Obstructive sleep apnea (OSA) is a form of sleep disordered breathing characterized by episodes of apnea (during sleep) lasting at least 10 seconds per episode. The apneic periods are associated with arterial hypoxemia and disruption of normal sleep as a result of awakenings. It is increasingly being recognized that OSA is a public health hazard and there is increasing evidence that it is associated with an increase in morbidity (and possibly mortality). Patients with OSA also utilize the healthcare resources at higher rates than control patients long before their diagnosis is confirmed. Early…

Central sleep apnea: Pathophysiology and treatment.

Central sleep apnea (CSA) is characterized by a lack of drive to breathe during sleep, resulting in repetitive periods of insufficient ventilation and compromised gas exchange. These nighttime breathing disturbances can lead to important comorbidity and increased risk of adverse cardiovascular outcomes. There are several manifestations of CSA, including high altitude-induced periodic breathing, idiopathic CSA, narcotic-induced central apnea, obesity hypoventilation syndrome, and Cheyne-Stokes breathing. While unstable ventilatory control during sleep is the hallmark of CSA, the pathophysiology…

Pediatric obstructive sleep apnea: current management.

Obstructive sleep apnea syndrome (OSAS) is a condition frequently encountered in children, and belongs to a spectrum of diseases called sleep-disordered breathing. Its pathophysiology is related to decreased ventilation despite respiratory effort, leading to episodes of hypopnea and apnea. Left untreated, this condition could have a significant impact on both physical and mental health. Primary snoring is often the presenting symptom reported by parents, and should warrant careful screening for OSAS. Although overnight polysomnography remains the gold standard diagnostic test for OSAS, its fea…

[Sleep apnea syndromes and cardiovascular disease].

Sleep-disordered breathing is very common and is associated with an increased risk of cardiovascular disease, cardiac arrhythmia and stroke. There are two types of sleep apnea: obstructive and central. The objective of this review is to provide a broad perspective of the pathophysiological and clinical aspects of the two types of apnea and to discuss their cardiovascular adverse effects. The diagnosis of sleep apnea syndrome is based on polysomnography, and severity is measured with an apnea-hypopnea index that counts the total number of apneas per hour of sleep. Recent large epidemiologic stu…

[Sleep apnea syndrome after total laryngectomy].

INTRODUCTION: Tracheostomy was the treatment of choice for sleep apnoea syndrome before nasal continuous positive airway pressure was developed. The occurrence of sleep apnea syndrome after total laryngectomy is unusual. CASE REPORT: The authors report the case of a 73 year-old man presenting with the symptomatology of sleep apnoea syndrome, several years after a total laryngectomy for cancer. The polysomnographic recording confirmed severe sleep apnoea syndrome with an apnea-hypopnea index of 89. Clinical examination showed a marked reduction in the diameter of the tracheostomy orifice durin…

Sleep apnea syndrome and its complications.

In this article we summarize the available information regarding the epidemiology, the pathophysiology as well as the risk factors and complications of the sleep apnea syndrome (SAS). Central, obstructive and mixed forms of SAS are known, however, the obstructive form is (resulting from the actual high prevalence of obesity) definitely the most frequent. Latest years of experimental and clinical research have pointed towards the clinical importance of this sleep related breathing disorder. High prevalence in the population and especially the cardiovascular complications (e. g. systemic and pul…

Cardiovascular consequences of obstructive sleep apnea.

Sleep apnea is associated with several cardiovascular disease conditions. A causal relationship between sleep apnea and each of these diseases is likely, but remains to be proven. The clearest evidence implicating OSA in the development of new cardiovascular disease involves data that show an increased prevalence of new hypertension in patients with OSA followed over 4 years [3]. Circumstantial evidence and data from small study samples suggest that OSA, in the setting of existing cardiovascular disease, may exacerbate symptoms and accelerate disease progression. The diagnosis of OSA always sh…

[Sleep apnea syndrome and cardiovascular diseases].

The sleep apnea syndrome is a common disease, recognised as a public health problem. Cardiovascular disease is the most frequent cause of morbidity and mortality in these patients, however the underlying mechanisms of this association have not been clearly established. In sleep apnea syndrome different phenomena can be produced which may explain the appearance of cardiovascular problems, such a progressive hypoxia in relationship with the apnea, the increases of intrathoracic pressure cause by the efforts of ventilation system against close upper airway and the modifications of the autonomic n…

Central Sleep Apnea and Hypoventilation Syndrome.

Central sleep apnea hypopnea syndrome (CSAHS) and sleep hypoventilation syndrome (SHVS) are two distinct clinical syndromes with clearly defined diagnostic criteria. It is important to distinguish between normo/hypocapnic and hypercapnic CSAHS prior to treatment. Nasal continuous positive airway pressure is currently considered the primary treatment of choice for normo/hypocapnic CSAHS. The initial management of hypercapnic CSAHS and SHVS should include identification of any treatable causes and discontinuation of any sedative medications. Medroxyprogesterone may be effective in the long term …

[Obstructive sleep apnea syndrome in childhood].

INTRODUCTION: Obstructive sleep apnea syndrome (OSAS) is a common problem in children. It is characterized by a partial airway obstruction associated with hypoxemia and hypoventilation rather than complete airway obstruction. DEVELOPMENT: Adeno-tonsillar hypertrophy is the leading cause but there are other risk factors like craniofacial abnormalities. OSAS may lead to neurodevelopmental abnormalities, growth and cardiorespiratory failure. It differs significantly from adult OSAS in its clinical presentation, polysomnographic findings and management. In spite of the fact that the diagnosis is …

[Endocrine disturbances in sleep apnea syndrome].

Sleep apnea may interact endocrine rhythms by a number of mechanisms. Repetitive apneas will cause sleep fragmentation and disorganization of sleep stages and cycles. How this compares with hormonal changes secondary to sleep deprivation is unclear. Hypoxia may have direct central effects on neurotransmitters, which in turn will affect hypothalamic-pituitary hormone production. And, sudden arousal from sleep may produce a central stress response leading to hormonal changes. These factors may interact and lead to changes in the central of sleep and endocrine rhythms. We describe Growth hormone …

[Sleep apnea in multiple system atrophy].

Multiple system atrophy(MSA) characterized clinically by any combination of autonomic, extrapyramidal or cerebellar symptoms and signs. MSA patients frequently manifest a variety of sleep-related respiratory disturbances, which are life threatening problems. Above all, a common and serious complication is upper-airway obstructive sleep apnea associated with stridor, which is caused by vocal cord abductor paralysis(VCAP). For the early diagnosis of VCAP, it is important to perform laryngoscopy during sleep, because VCAP does not appear during awakefulness in the early stage of MSA. Polysomnogra…
