BACKGROUND AND IMPORTANCE: Syringomyelia, or the formation of fluid-filled cysts within the spinal cord, associated with delayed spinal arachnoiditis is an uncommon complication of aneurysmal subarachnoid haemorrhage. To date, about 18 cases have been reported in medical literature, with just two reported in patients under the age of 35 years.
CLINICAL PRESENTATION: A 27-year-old female patient complained of sudden, severe headaches in the occipital region, nuchal rigidity, and drowsiness when she presented at our institution. A head computed tomography scan revealed intraventricular bleeding…

BACKGROUND: Adhesive arachnoiditis is an uncommon lesion caused by an inflammatory reaction in spinal nerves. Reports of substantial symptomatic thoracolumbar (TL) adhesive arachnoiditis after spinal surgery are rare. To the best of our knowledge, this is the first presentation of delayed adhesive arachnoiditis with cauda equina syndrome after decompression and fusion for a traumatic TL flexion-distraction injury.
PRESENTATION OF CASE: A 51-year-old man presented to the emergency room with absence of lower extremity muscle power and partial sensation preservation below T12 after slipping. Mag…

BACKGROUND: Data regarding the neuroradiology features of the West Nile virus neuroinvasive disease (WNV NID) is rather scarce. To contribute to the knowledge of the WNV NID, we present a patient with a combination of encephalitis and acute flaccid paresis, with cauda equina arachnoiditis as the main magnetic resonance (MR) finding.
CASE SUMMARY: A 72-year-old female patient was admitted due to fever, headache and gait instability. During the first several days she developed somnolence, aphasia, urinary incontinence, constipation, and asymmetric lower extremities weakness. Cerebrospinal fluid…

We report on an unusual case of idiopathic arachnoiditis mimicking a spinal cord tumor in a 50-year-old man with progressive paraparesis. The medical imaging work-up showed an enlarged terminal cone with adjacent cysts. Although there was no enhancement of the terminal cone or the surrounding structures, the diagnosis of spinal cord tumor was maintained and surgery was carried out. Exploration of the cauda equina and the tissue surrounding the terminal cone did not reveal a tumor. The patient improved dramatically after surgery but the symptoms recurred 1 year later. We review arachnoiditis, i…

BACKGROUND: Dorsal spinal arachnoid bands are a rare cause of focal spinal cord compression and thoracic myelopathy that alter cerebrospinal fluid dynamics and may lead to progressive neurological deficits and syringomyelia. Nevertheless, the clinical outcomes of surgical treatment remain incompletely characterized. Here, we evaluated the safety and clinical outcomes of 6 microsurgical decompressions for dorsal thoracic arachnoid bands in a single-center cohort.
METHODS: Six patients underwent surgery for dorsal thoracicspinal arachnoid bands with/without syringomyelia (2021-2025) at a single

Arachnoid cysts (ACs) are benign congenital lesions that are often detected incidentally during imaging studies performed for various etiological reasons. These cysts are typically asymptomatic, and in most cases do not require surgical intervention. Although ACs are commonly encountered, they are rarely complicated by intracystic hemorrhage or subdural hematoma. In this report, we present a case of a nontraumatic rupture of an AC, which remained undiagnosed until the age of 52 years.

Spinal intradural ventral cysts are rare. This article reports a rare case of the coexistence of arachnoid and endodermal cysts in the cervical spine. A 34-year-old man presented with left upper limb pain and thermal hypoalgesia in the right hand that began four years earlier, progressing to additional pain and numbness in the left arm. He also exhibited weakness in the left extremities and disturbances in fine hand movements. Magnetic resonance imaging (MRI) revealed two intradural extramedullary cysts at the C3/4 spinal level compressing the spinal cord from the left ventral side. Surgery, i

INTRODUCTION: Spinal arachnoid cysts are rare benign lesions filled with cerebrospinal fluid and lined by an arachnoid membrane. We report an observation of an intradural arachnoid cyst in the lumbar region causing spinal cord compression.
OBSERVATION: This is a 37-year-old female patient with no past medical history who consulted for bilateral lumbosciatica, hyperalgia, poorly systematized pain associated with sphincter disorders, such as dysuria and constipation, weakness in the lower limbs, and walking difficulties. The physical examination revealed a positive Sonnelli sign at the lumbar l

BACKGROUND: Multiple system atrophy (MSA) is an α-synucleinopathy characterized by prominent cerebellar or parkinsonian features. This is pathologically due to the deposition of α-synuclein in glial cells, the mechanism by which s not entirely clear. We describe three patients with posterior fossa arachnoid cysts later diagnosed with clinically established MSA, suggesting the co-existence of two rare pathologies through a shared mechanism versus by chance.
CASES: Patients ranged from 45 to 67 years of age at presentation, each describing 2-3 years of progressive symptoms. All displayed cerebe

Intravenous arachnoid granulations (AGs) are protrusions of the arachnoid membrane into the venous lumen and function as contributors to the cerebrospinal fluid (CSF) flow circuit. Patients with Parkinson disease (PD) often present with accumulation of alpha synuclein. Previous works have provided evidence for neurofluid circulation dysfunction in neurodegenerative diseases associated with changes in CSF egress, which may have implications regarding AG morphology. The present study aims to investigate group differences in AG volumetrics between healthy and PD participants, as well as relations

Background: Arachnoid cysts (ACs) are non-neoplastic, intracranial cerebrospinal fluid (CSF)-filled cavities lined with arachnoid membranes. Most of them are found incidentally and are asymptomatic. The aim of this study was to describe the clinical and demographic characteristics of ACs and to investigate the frequency of complications. Methods: ACs were classified as cerebral and cerebellar, and were evaluated according to location and size using the Galassi classification. Results: Evaluation was made of 103 patients with AC admitted to the pediatric neurology department. The patients compr

Arachnoid cysts, fluid-filled lesions within the central nervous system, pose diagnostic challenges. This study examines a unique case of a quadrigeminal arachnoid cyst in a 13-year-old girl, emphasizing accurate identification and treatment. The patient's symptoms of blurred vision and headaches led to the discovery of papilledema and imaging revealing a sizable cyst causing obstructive hydrocephalus. Urgent surgical intervention involved suboccipital craniectomy and infratentorial-supracerebellar cyst drainage, resulting in favorable postoperative outcomes. Further analysis of anatomical var

BACKGROUND: Neurocysticercosis is a parasitic infection of the central nervous system caused by the helminth Taenia solium. Racemose neurocysticercosis is a rare form of the disease that specifically involves cerebrospinal fluid-filled spaces in the brain and carries a high rate of complications and mortality.
OBSERVATIONS: This report describes the case of a 37-year-old man who developed headaches and nausea, which were found to be secondary to racemose neurocysticercosis. He ultimately required an endoscopic third ventriculostomy, which provided transient symptom relief. In the weeks follow

BACKGROUND: Intracranial arachnoid cysts are benign collections of cerebrospinal fluid that are often asymptomatic and discovered incidentally. An interhemispheric location of these lesions is rare, with only a few such cases reported in the literature. Though spontaneous regression of arachnoid cysts has been described in other locations, to date this phenomenon has not been reported in interhemispheric fissure cysts.
OBSERVATIONS: In this report, we describe a patient with a large, multiloculated interhemispheric arachnoid cyst diagnosed on prenatal ultrasound. She did not exhibit neurologi
