[Follow-up of psychiatric disorders and specific developmental delay disorders in special education students].

In a prospective longitudinal study 72 children who were attending a special school for children with learning disabilities in Erlangen were … cific developmental disorders also decreased over the two-year period. As expected, specific speech articulation disorders had the best prognosis. All other disorders had a poor prognosis. Increases in prevalence were mainly the result of different cutoff points for a given diagnosis at different ages. The high number of specific disorders of motor function at both time …

Radiocarpal Fusion: Indications, Technique, and Modifications.

Degenerative disorders of the wrist may affect isolated joints and inhibit normal functions of the wrist secondary to pain and stiffness. These processes that affect only the radiocarpal joint may be secondary to posttraumatic osteoarthritis, primary osteoarthritis, or rheumatoid arthritis. Radiocarpal wrist arthrodesis may help preserve some of the native wrist kinematics while alleviating pain and improving the range of motion. However, the surgeon must ensure that the patient's pathologic process primarily affects the radiocarpal articulations while relatively sparing the midcarpal articula

Arthroscopic management of painful and stiff scapulothoracic articulation.

We present the case of a patient who had chronic refractory scapulothoracic pain accompanied by the loss of scapulothoracic motion. Despite intensive physical therapy, the insidious onset of scapulothoracic pain and stiffness progressed. A wide range of diagnostic tests did not show a systemic, anatomic, or neurologic cause for the disorder. Finally, the patient elected to undergo an arthroscopic release and decompression of the scapulothoracic articulation. The patient had a dramatic response to surgery; the pain was gone immediately, and by 4 months after surgery, her scapulothoracic motion

The Relationship between Selected Demographic Factors and Speech Organ Dysfunction in Sporadic ALS Patients.

Background and objectives: Speech disorders are observed in 30% of newly diagnosed sporadic amyotrophic lateral sclerosis (ALS) patients. Characterized by a dynamic course, dysfunction of articulation has not so far been well understood. The aim of this study was to analyze the influence of demographic factors (sex, age, duration of the disease) and concomitant diseases (degenerative spine disease, depression, hypertension, hypothyroidism, hyperthyroidism, and allergy) on the functioning of speech organs in ALS patients. Materials and Methods: The study group consisted of 65 patients with spor

Speech and prosody characteristics of adolescents and adults with high-functioning autism and Asperger syndrome.

Speech and prosody-voice profiles for 15 male speakers with High-Functioning Autism (HFA) and 15 male speakers with Asperger syndrome (AS) were compared to one another and to profiles for 53 typically developing male speakers in the same 10- to 50-years age range. Compared to the typically developing speakers, significantly more participants in both the HFA and AS groups had residual articulation distortion errors, uncodable utterances due to discourse constraints, and utterances coded as inappropriate in the domains of phrasing, stress, and resonance. Speakers with AS were significantly more

Multimodal Architecture for Phoneme Labeling Imprecision in Dysarthric Speech: Integrating Transformers and TCNs for Clinical Rehabilitation.

Due to articulation issues and phoneme variability, speech disorders such as dysarthria pose significant challenges in clinical rehabilitation. Traditional automatic speech recognition (ASR) systems, which are typically trained on normative datasets, often fail to accurately decode dysarthric speech. Recent breakthroughs in artificial intelligence and deep learning have allowed the integration of multimodal architectures, such as merging auditory, articulatory, and visual information to record complex speech patterns, making it increasingly possible. In this review, we have explored the conver

The family history of children with elective mutism: a research report.

The family history was studied in children with elective mutism. The samples comprised a series of N = 38 children with elective mutism and a control group of N = 31 children with a similar behavioural phenotype, i.e., the combination of an emotional disorder and a developmental disorder of articulation or expressive language. Interviews were performed with the respective mothers. There was a clear excess of the personality trait of taciturnity in first-, second-, and third-degree relatives. Although mutism was reported almost exclusively in the group of relatives of children that manifested e

PTSD in asylum-seekers: Manifestations and relevance to the asylum process.

Post-Traumatic Stress Disorder (PTSD) is common among asylum-seekers and manifests with symptoms uniquely problematic to the United States asylum-seeking process. Chiefly, hyper-vigilance, avoidance behavior, dissociative amnesia and a tendency towards non-linear narratives hinder the articulation of an effective asylum claim. Moreover, the format and environmental circumstances of the asylum-seeking process further heighten these difficulties.

Using the DIVA model to better understand speech motor disorders.

The Directions Into Velocities of Articulators (DIVA) model provides a comprehensive neurocomputational framework for understanding speech production and speech motor disorders. This theoretical model describes three main control processes: feedforward control, auditory feedback control, and somatosensory feedback control. By mapping these processes to specific brain regions and neural circuits, DIVA offers insights into the mechanistic basis of various speech disorders and provides a foundation for developing targeted therapeutic interventions. This paper reviews the DIVA model's architecture

Painful clicking jaw: a pictorial review of internal derangement of the temporomandibular joint.

Temporomandibular joint disorders are a common cause of chronic musculoskeletal pain worldwide. Among these, internal disc derangement is the most frequent type of disorder. Internal derangement is defined as an abnormal positional and functional relationship between the disc and articulating surfaces. Common clinical symptoms include pain and clicking. Imaging plays a key role in diagnosing temporomandibular joint disorders. It is important for the radiologist to detect early imaging signs of internal derangement, thereby avoiding the evolution of this condition to degenerative joint disease.

Technical feasibility of personalized articulating knee joint distraction for treatment of tibiofemoral osteoarthritis.

BACKGROUND: Knee osteoarthritis is a highly prevalent degenerative joint disorder characterized by joint tissue damage and pain. Knee joint distraction has been introduced as a joint preserving surgical procedure to postpone knee arthroplasty. An often used standard externally fixation device for distraction poses a burden to patients due to the absence of joint flexion during the 6weeks treatment. Therefore, a personalized articulating distraction device was developed. The aim of this study was to test technical feasibility of this device. METHODS: Based on an often applied rigid device, usi

The Temporomandibular Joint and the Human Body: A New Perspective on Cross Talk.

Background: As a unique joint that facilitates the articulation of the upper and lower jaws, the temporomandibular joint (TMJ) is concerned with several critical functions, such as speech and mastication. Pain that can become incapacitating is a result of temporomandibular disorders (TMDs), which are complex disorders affecting the masticatory muscles and the TMJ. Several anomalies and TMDs have an interdisciplinary relationship. Complementary and concurrent disorders may be caused by occlusal anomalies, psychological disorders, and changes in spine posture. Methods: This article examines the

Neurofibromatosis Type 1 Implicates Ras Pathways in the Genetic Architecture of Neurodevelopmental Disorders.

The genetic architecture of neurodevelopmental disorders is largely polygenic, non-specific, and pleiotropic. This complex genetic architecture makes the search for specific etiological mechanisms that contribute to neurodevelopmental risk more challenging. Monogenic disorders provide an opportunity to focus in on how well-articulated signaling pathways contribute to risk for neurodevelopmental outcomes. This paper will focus on neurofibromatosis type 1 (NF1), a rare monogenic disorder that is associated with varied neurodevelopmental outcomes. Specifically, this paper will provide a brief ove

Peer attitudes toward children with cleft (lip and) palate related to speech intelligibility, hypernasality and articulation.

BACKGROUND AND AIMS: In addition to anatomical and physiological problems, children with a cleft (lip and) palate (CP ± L) often face psychosocial difficulties. A complex interaction between patient and environment may induce these problems. Based on the literature, speech disorders may negatively influence a listener's judgement of a speaker. Therefore, the aim of the present study was to investigate the attitudes of peers toward the speech of children with CP ± L. METHOD: Sixty-nine typically-developing children (7-12 years, 34 boys, 35 girls) judged audio-recorded speech samples of nine ch

Consonant articulation accuracy in paediatric cochlear implant recipients.

Hearing loss is a significant risk factor for delays in the spoken language development of children. The purpose of this study was to examine the distribution of articulation errors for English consonants among children with cochlear implants (CIs) who utilise auditory-oral communication. Speech samples from 45 prelingually deafened paediatric CI users were obtained using a single-word picture elicitation task. Samples were audio recorded and transcribed in PRAAT; overall percentage consonant correct (PCC) and individual phoneme error patterns were examined. Results showed an average PCC of 76

Deficits in musical rhythm perception in children with specific learning disabilities.

BACKGROUND: A specific learning disability comes with a cluster of deficits in the neurocognitive domain. Phonological processing deficits have been the core of different types of specific learning disabilities. In addition to difficulties in phonological processing and cognitive deficits, children with specific learning disability (SLD) are known to have deficits in more innate non-language-based skills like musical rhythm processing. OBJECTIVES: This paper reviews studies in the area of musical rhythm perception in children with SLD. An attempt was made to throw light on beneficial effects
