Association Between Concussion and Lower Extremity Injuries in Collegiate Athletes.

BACKGROUND: Concussions have been associated with elevated musculoskeletal injury risk; however, the influence of unreported and unrecognized concussions has not been investigated. HYPOTHESIS: The purpose of this study was to examine the association between concussion and lower extremity musculoskeletal injury rates across a diverse array of sports among collegiate student-athletes at the conclusion of their athletic career. The hypothesis was that there will be a positive association between athletes who reported a history of concussions and higher rates of lower extremity injuries. STUDY D

Common injuries and ailments of the female athlete; pathophysiology, treatment and prevention.

With increasing numbers of women competing in high school and collegiate athletics, it is important that physicians become familiar with injury patterns and medical conditions unique to the female athlete. Observations and clinical data have elucidated unique biomechanical, anatomic and hormonal factors that predispose skeletally mature female athletes to anterior cruciate ligament (ACL) injuries, patellofemoral disorders and lower extremity stress fractures. Additionally, younger female athletes are particularly at risk of developing components of the "Female Athlete Triad" (more recently inc

Thermoregulation during intermittent exercise in athletes with a spinal-cord injury.

PURPOSE: Individuals with a spinal-cord injury have impaired thermoregulatory control due to a loss of sudomotor and vasomotor effectors below the lesion level. Thus, individuals with high-level lesions (tetraplegia) possess greater thermoregulatory impairment than individuals with lower-level lesions (paraplegia). Previous research has not reflected the intermittent nature and modality of wheelchair court sports or replicated typical environmental temperatures. Hence, the purpose of this study was to investigate the thermoregulatory responses of athletes with tetraplegia and paraplegia during

British athletics muscle injury classification: a new grading system.

The commonly used muscle injury grading systems based on three grades of injury, representing minor, moderate and complete injuries to the muscle, are lacking in diagnostic accuracy and provide limited prognostic information to the clinician. In recent years, there have been a number of proposals for alternative grading systems. While there is recent evidence regarding the prognostic features of muscle injuries, this evidence has not often been incorporated into the grading proposals. The British Athletics Muscle Injury Classification proposes a new system, based on the available evidence, whi

Opinions regarding the management of hand and wrist injuries in elite athletes.

Injuries to the hand and wrist are commonly encountered in athletes. Decisions regarding the most appropriate treatment, the timing of treatment, and return to play are made while balancing desires to resume athletic activities and sound orthopedic principles. Little recognition in the literature exists regarding the need for a different approach when treating these injuries in elite athletes and the timing to return to play. This study explored the complexities of treating hand and wrist injuries in the elite athlete. Thirty-seven consultant hand surgeons for teams in the National Football Le

Combined ACL-posterolateral corner injury in a skeletally immature athlete.

Combined injury to the anterior cruciate ligament (ACL) and posterolateral structures of the knee is a rare yet increasingly recognized clinical entity. In children or adolescents with open growth plates, this injury pattern is considered to be even more rare than in adults. Because knee ligaments are felt to be stronger than the adjacent physeal plates, the energy of pathologic translational and rotatory stress commonly results in fracture or avulsion injuries rather than ligamentous injury. The appropriate treatment for either injury remains controversial in the skeletally immature patient,

Etiology, pathophysiology, and most common injuries of the lower extremity in the athlete.

The majority of injuries during sport are to the lower extremity (more than 50%), most of which occur in children and young adults younger than 25 years and involve fractures, ligament injuries, or tendon injuries. This article discusses the etiology of some of the most common foot and ankle sports-related injuries. The authors focus on clinical findings, associated injuries, pathophysiology, and current trends. Many of these factors are discussed in detail elsewhere in this issue.

Rehabilitation and return to play after foot and ankle injuries in athletes.

Rehabilitation after acute ankle injury can be categorized in to early, middle, and late phases. This paper aims to cover some of the key concepts from initial management to end stage rehabilitation that could be applied to many musculoskeletal injuries around the foot and ankle. Pathology specific rehabilitation, functional performance tests, and sports specific and return to play issues are also addressed, as the aim of any rehabilitation programme is to return the athlete to the field of play as quickly as it is safe to do so.

MR imaging of traumatic and overuse injuries of the wrist and hand in athletes.

Traumatic and overuse injuries of the hand and wrist are common in athletes. Increasingly, MR imaging is being used to complement clinical and radiographic assessment in the diagnosis and management of these injuries. MR imaging is able to image accurately the bones, tendons, ligaments, nerves, and other small structures of the hand and wrist. This article provides an overview of traumatic and overuse injuries of the hand and wrist in athletes and a review of the MR imaging appearances.

Muscle activation imbalance and low-back injury in varsity athletes.

There are conflicting findings in the literature regarding erector spinae activation imbalance in people with low-back pain (LBP). Some studies have found asymmetric recruitment between muscle pairs in people with LBP, whilst other studies have not; some reported people with LBP recruit more lumbar muscles whilst other have reported greater thoracic activity. Using 242 varsity athletes, EMG activity of thoracic and lumbar erector spinae pairs was recorded during an isometric trunk extension. Activation imbalance among muscle pairs and levels was compared between athletes with and without a his

Sports-related knee injuries in female athletes: what gives?

Knee injuries occur commonly in sports, limiting field and practice time and performance level. Although injury etiology relates primarily to sports specific activity, female athletes are at higher risk of knee injury than their male counterparts in jumping and cutting sports. Particular pain syndromes such as anterior knee pain and injuries such as noncontact anterior cruciate ligament (ACL) injuries occur at a higher rate in female than male athletes at a similar level of competition. Anterior cruciate ligament injuries can be season or career ending, at times requiring costly surgery and re

Ulnar collateral ligament injury in the overhead athlete: diagnosis and treatment.

The ulnar collateral ligament (UCL) is more commonly injured than previously suggested. Injury to this ligament can result in secondary symptoms and problems in other parts of the elbow, including the ulnar nerve, the flexor-pronator musculotendinous unit, the radiocapitellar joint, and the posterior compartment of the elbow, in addition to being a cause of loose bodies within the elbow. This article briefly reviews the anatomy, biomechanics, and pathophysiology of injury, in addition to discussing evaluation and treatment of the athlete with an injured UCL.

MR imaging of the distribution and location of acute hamstring injuries in athletes.

OBJECTIVE: Although hamstring injuries are common in athletes, the distribution and location of such injuries have not been well defined. We used MR imaging to determine the frequency of injury by muscle, involvement of one or more muscles, and location of injuries within the musculotendinous unit. SUBJECTS AND METHODS: We performed MR imaging on 15 consecutive college athletes with clinically diagnosed acute hamstring injuries. A hamstring injury was diagnosed and located on MR imaging by identifying high signal intensity within the muscle on T2-weighted images. RESULTS: We found that 10 at

Direct injury to the axillary nerve in athletes playing contact sports.

We performed long-term followup (31 to 276 months) of 11 contact athletes who had sustained isolated injuries to their axillary nerves during athletic competition. There were no known shoulder dislocations. Electromyographs were taken of 10 patients, and all patients had confirmation of clinically defined injuries that were confined to their axillary nerves. Nine injuries were sustained while tackling opposing players in football; two were sustained in hockey collisions. In seven athletes, the mechanism of injury was a direct blow to the anterior lateral deltoid muscle. In four athletes, there

Surgical treatment of acute type-V acromioclavicular injuries in athletes.

In a prospective study, 18 consecutive athletes with an acute type-V acromioclavicular sprain were treated with a coracoclavicular repair using a double velour Dacron graft. All patients were reviewed after a mean follow-up period of 6 years (range: 2-9 years). At follow-up, 12 patients (66.7%) showed a good or excellent result according to the Imatani evaluation system, and six patients (33.3%) demonstrated a fair or poor result according to the same system. Loss of reduction was encountered in eight shoulders (44.4%) despite an initial anatomical reduction. No correlation was seen between th

Anterior cruciate ligament injuries in the young athlete with open physes.

From 1980 to 1985, 40 patients under the age of 14 with open physes were treated for midsubstance tears of the ACL at the Methodist Sports Medicine Center. In this series, 16 were treated conservatively with rehabilitation, bracing, and counseling on activity modification. The remaining 24 patients underwent arthroscopic examination and either an extraarticular or intraarticular reconstruction based on growth potential. The average followup was 27 months for the conservative group and 26 months for the surgical group. In the conservative group, six patients underwent arthroscopy for meniscal t
