OBJECTIVE: The anterior only surgical procedure including discectomy, open reduction and fusion is used as a recommended approach in the treatment of unilateral cervical facet dislocations, but is difficult to achieve satisfactory anterior open reduction by vertebra distractor to spread the facet joints, especially for delayed management of unilateral cervical facet dislocation (7-21days). The goal of this study was to report an anterior pedicle spreader technique to distract directly the facet joint for anterior reduction and the results of 4 patients with successful application and describe

STUDY DESIGN: Immunohistochemical analyses on the axial skeleton from wild type mice.
OBJECTIVE: In the clinic, we have previously observed cervical spine defects associated with deviations in the posterior part of the occipital bone and with morphologic and functional variations in the craniofacial skeleton. As examples, cervical spine fusions occurred frequently in patients with mandibular overjet and even more frequently and more caudally in the cervical spine in patients with sleep apnoea. The aims of the present study were to elucidate this association between the spine and the cranium b

Therapeutic targets have been defined for axial and peripheral spondyloarthritis (SpA) in 2012, but the evidence for these recommendations was only of indirect nature. These recommendations were re-evaluated in light of new insights. Based on the results of a systematic literature review and expert opinion, a task force of rheumatologists, dermatologists, patients and a health professional developed an update of the 2012 recommendations. These underwent intensive discussions, on site voting and subsequent anonymous electronic voting on levels of agreement with each item. A set of 5 overarching

Despite the increasing prevalence of spinal infections, the subcategory of holospinal epidural abscesses (HEAs) is extremely infrequent and requires unique management. Panspinal imaging (preferably MRI), modern aggressive antibiotic therapy, and prompt surgical intervention remain the standard of care for all spinal axis infections including HEAs; however, the surgical decision making on timing and extent of the procedure still remain ill defined for HEAs. Decompression including skip laminectomies or laminoplasties is described, with varied clinical outcomes. In this review the authors presen

INTRODUCTION: Stable hangman's fractures are usually treated with a Halo vest fixation; however, this is not always effective in patients with polytrauma. These patients benefit from minimally invasive surgery because it allows for early rehabilitation and reduced nursing care. This is the case report on percutaneous screw fixation using three dimensional fluoroscopy-assisted navigation in a patient with polytrauma and a hangman's fracture.
CASE REPORT: The patient was a 69-year-old woman involved in a traffic accident. Radiographs and CT showed bilateral fractures through the neural arch at

OBJECTIVE: To evaluate the results of surgical treatment using pedicle screws going through C2 pedicles for fixating the spondylolisthesis of the axis in patients who presented pseudoarthrosis after clinical treatment, or who have no condition for fixation with "halo vest", due to serious head trauma.
METHOD: Ten patients have been operated from June 1998 to April 2002, nine suffering from traumatic spondylolisthesis of the axis caused by car accident and one horse fall. Four of those patients have undergone clinical treatment and presented signs of pseudoarthrosis, suffering intense pain at

A 33-year-old female dialysis patient suffered from osteomyelitis and luxation of the dens axis with cervical myelopathy. In the past she had had three attacks of anaphylaxis after treatment with dialyzers that had been sterilized with ethylene oxide. IgE-type antibodies directed against human serum albumin-ethylene oxide complexes could be demonstrated in the patient's serum by radioallergosorbent techniques. Immediately after an operation in which acrylic bone cement (Palacos-R) sterilized with ethylene oxide was implanted for stabilization of the cervical spine, the patient developed massiv

The authors preferred the term of "fracture of the posterior arch of the axis", rather than the restrictive terms of bipedicular or bi-isthmic fractures commonly referred to in the literature. It was possible, from 34 cases to joint out 4 different figures of fractures of the posterior arch of axis: isthmus, body and articular process, pedicle and lamina. From this classification it is possible to get indications for surgical treatment, as some of those fractures need internal fixation and others not. On completing Xrays study with research of instability between C2-C3 it's possible to fix a s

A complex malformation of the first and second branchial arches and of the first branchial cleft is described; there was also involvement of the axial skeleton and the limbs. The syndrome has been described by us as dysostosis temporo-sphenofacialis and could not be classified with any previously described entity. The reason for this was due to the imcomplete description of previously published cases, but also the large number of possible combinations of changes in various organs which can occur. In order to understand the condition more clearly, the aetiology and pathogenesis of abnormalities

A clinical study of fractures of the odontoid process showed a 62 per cent failure on union. Of many features studied, only displacement and its direction had a definite bearing on non-union of the fracture. Blood supply was considered as a possible etiological factor. The blood supply of the human odontoid was elucidated by means of studying 19 human autopsies by means of microangiography. The dog was found to have a similar blood supply and hence, was used as the experimental model. Two osteotomies were performed; one below the accessory ligaments and one above the accessory ligaments. All o

Open-door laminoplasty is widely used for patients with cervical spondylotic myelopathy (CSM). However, the loss of cervical lordosis (LCL) seems to be unavoidable in the long-term follow-up after surgery, which may affect the clinical outcomes. The risk factors for this complication are still unclear. In this study, patients who underwent open-door laminoplasty between April 2016 and June 2021 were enrolled. Cervical X-rays were obtained to measure the C2-7 Cobb angle, C2-7 sagittal vertical axis (SVA), T1 slope (T1S) and ranges of motion (ROM). Cervical computed tomography (CT) scans and mag

STUDY DESIGN: Retrospective cohort study.
OBJECTIVE: Determine the indications, complications, and clinical outcomes in patients requiring fusions from the cervical spine to the pelvis. Several investigators have examined fusions from the thoracic spine to the sacrum, but no similar study has been performed for cervical-to-pelvis fusions.
METHODS: Patients from 2003 to 2014 with an upper instrumented vertebrae (UIV) in the cervical spine (any level) and a lower instrumented vertebrae (LIV) in the sacrum or pelvis were included in the study. Those with infectious or acute trauma-related defor

BACKGROUND: Transarticular C1-C2 screw fixation, first described by Magerl, is a widely accepted used technique for C1-C2 instability with a good biomechanical stability and fusion rate.
METHOD: We present a 69-year-old woman, who was diagnosed with a C2 Odontoid fracture type III and primarily treated with conservative treatment and collar. During first 2 weeks of follow-up, the patient developed cervical pain associated with C1-C2 instability. A minimally invasive posterior C1-C2 transarticular screw instrumentation with a percutaneus approach was performed.
RESULTS AND CONCLUSION: Minimal

BACKGROUND: Axial compression injuries of the cervical spine occur during contact sports, automobile collisions, and falls. The objective of this study was to use flexibility tests to determine biomechanical instability of the cervical spine due to simulated axial compression injuries.
HYPOTHESIS: We hypothesized that the axial compression injuries cause severe biomechanical instability throughout the cervical spine.
MATERIALS AND METHODS: The injuries were simulated using 2.4m/s head-first impacts of a cadaveric cervical spine model (n=10) mounted horizontally to a torso-equivalent mass on

A 15-year-old boy suffering from osteoid osteoma of the right lamina of the axis presented with torticollis and local pain without neurological deficits. The tumour was treated by en-bloc resection and laminoplasty with autogenous iliac crest. The follow-up at 14 months showed complete pain relief and a perfect incorporation of the transplanted bone.

Posterior displacement of the odontoid after fracture occurs much less frequently than does anterior displacement. Experience with four patients suggests that anatomic reduction may not be possible and prolonged attempts to gain reduction are not advisable. Those cases should be managed with early application of a halo vest. A rotating frame should be avoided. The potential for respiratory arrest, at least in the older individual, is great and can be precipitated even by a change of position in the process of turning on the rotating frame. The mechanism of the respiratory failure is obscure.

BACKGROUND: Proximal junctional kyphosis (PJK) following adult spinal deformity (ASD) surgery is a well-documented complication, but associations between radiographic PJK and cervical malalignment onset remain unexplored.
OBJECTIVE: To study cervical malalignment in ASD surgical patients that develop PJK.
METHODS: Retrospective review of prospective multicenter database. Inclusion: primary ASD patients (≥5 levels fused, upper instrumented vertebra [UIV] at T2 or above, and 1-yr minimum follow-up) without baseline cervical deformity (CD), defined as ≥2 of the following criteria: T1 slope minu
