Role of basophils and type 2 inflammation in bullous pemphigoid pathophysiology: a comparative study of blood and blister fluid.

Bullous pemphigoid (BP) is an autoimmune disease characterized by blister formation and inflammatory cell infiltration. In addition to eosin … ocyte activation and increased type 2 cytokine secretion in BP blister fluid. Under no stimulation, basophils produce more IL-4 compared to CD4+ T lymphocytes in BP blister fluid. Basophils might play a more important role in BP than we early thought. Along with other inflammatory cells and factors, basophils, demonstrated as one of the main producers of IL …

Purification of human basophils and mast cells by multistep separation technique and mAb to CDw17 and CD117/c-kit.

Basophils and mast cells represent distinct cell lineages within the hemopoietic system. Based on the unique cell surface antigen profile of both cells, we have established methods which allow the reproducible purification to homogeneity (> 99%) of normal human basophil granulocytes from the peripheral blood and of mast cells from human dispersed tissues. Basophils (n = 9) were purified by current counterflow elutriation followed by depletion of monocytes with CD14 mAb conjugated to magnetic beads, and subsequent cell sorting for CD217+ cells. Basophil purity was 99.5 +/- 0.4% (range 98.7-99.9…

Effects of trichinellosis on levels of eosinophils, eosinophil major basic protein, creatine kinase and basophils in the guinea pig.

The effects of multiple infections with Trichinella spiralis on the levels of eosinophil major basic protein (MBP), creatine kinase (CK), and leucocytes were studied in the guinea pig. Plasma MBP levels increased after each of four successive infections with T. spiralis. The times of peak elevations in MBP levels and eosinophilia correlated significantly, and both peaks occurred earlier with increased infection dose. CK levels were elevated most dramatically in the primary infection. A direct correlation between peak CK values and larval dose was observed, and the peak occurred earlier with la

The decrease in peripheral blood basophils in a mouse model of IgE-induced inflammation involves their migration to lymph nodes.

BACKGROUND: During the active phase of urticaria, a decrease in peripheral blood basophils, known as basopenia, is observed. We previously reported that basopenia occurs as a result of basophils migrating to the skin in a contact dermatitis model where a Th2 response is induced with oxazolone. OBJECTIVE: Although there is currently no established model for urticaria, given that urticaria is an IgE-mediated immediate-type allergic reaction, we aimed to determine whether an IgE-mediated model could reproduce the decrease in basophils in peripheral blood observed during the active phase of urtic…

An increase in basophils in a case of acute myelomonocytic leukaemia associated with marrow eosinophilia and inversion of chromosome 16.

A patient with acute myelomonocytic leukaemia (M4 subtype) with pericentric inversion of chromosome 16, inv(16)(p13q22), and a marked basophilia is described. Results from transmission electron microscopy suggested that the basophils were immature. Clinically, although leukaemic cells had a tendency to infiltrate a number of tissues and organs, such as skin, gingiva, liver and lung, the patient was successfully treated with chemotherapy. The increase in basophils accompanied by marrow eosinophilia may be the result of abnormal differentiation of leukaemic cells that have the capacity to differ…

IgE levels correlate with FcεRI expression on circulatory basophils but not with their activation response in patients with Hymenoptera anaphylaxis.

BACKGROUND: Measurement of IgE levels, skin prick tests (SPT), and basophil activation tests (BAT) are the gold standards for diagnosing Hymenoptera allergy. Currently, BAT most accurately resembles patient reactivity. However, data examining the influence of serum IgE and basophil FcεRI expression on BAT performance and their clinical relevance are limited. METHODS: Total and specific IgE (sIgE) levels and their ratios, SPT results, high-affinity IgE receptor (FcεRI) expression on basophils, and BAT results from 31 patients with Hymenoptera-induced anaphylaxis were analyzed and correlated wi…

Lung function, airway and peripheral basophils and eosinophils are associated with molecular pharmacogenomic endotypes of steroid response in severe asthma.

INTRODUCTION: Asthma is a complex disease with heterogeneous expression/severity. There is growing interest in defining asthma endotypes consistently associated with different responses to therapy, focusing on type 2 inflammation (Th2) as a key pathological mechanism. Current asthma endotypes are defined primarily by clinical/laboratory criteria. Each endotype is likely characterised by distinct molecular mechanisms that identify optimal therapies. METHODS: We applied unsupervised (without a priori clinical criteria) principal component analysis on sputum airway cells RNA-sequencing transcrip

Identification of basogranulin (BB1) as a novel immunohistochemical marker of basophils in normal bone marrow and patients with myeloproliferative disorders.

In myeloproliferative disorders (MPDs), basophils typically increase in number in the bone marrow (BM) and blood. In chronic myeloid leukemia (CML), basophilia is a diagnostic and prognostic variable. However, no reliable approach for routine detection and enumeration of basophils in BM sections is available. We applied the antibasogranulin antibody BB1 on paraffin-embedded BM sections in 21 control samples (normal BM), 45 patients with CML, 9 with chronic idiopathic myelofibrosis, 11 with polycythemia vera, 19 with essential thrombocythemia, and 7 with indolent systemic mastocytosis. As asses…

Analysis of the lineage relationship between mast cells and basophils using the c-kit D816V mutation as a biologic signature.

BACKGROUND: Mast cells and basophils share similar morphologic and functional properties; however, it is not known whether they are derived from a bilineage (basophil/mast cell)-restricted progenitor. OBJECTIVE: To assess whether basophils and mast cells are derived from common committed progenitors using the c-kit D816V mutation as a biologic signature. METHODS: The D816V c-kit mutation found in mast cells of patients with systemic mastocytosis is used as a trackable genetic marker to assess the lineage relationship between mast cells and basophils. Blood and bone marrow aspirates were coll…

Standardizing a Four-Marker Basophil Activation Test: Influence of Storage Time, Temperature, and IgE Levels.

BACKGROUND: Allergic diseases affect 25% of the global population, yet current diagnostics like specific IgE (sIgE) and skin prick tests lack specificity. The basophil activation test (BAT) offers high specificity but requires methodological standardization. OBJECTIVE: This study develops gating strategies and pre-analytical storage conditions to develop an analytically validated BAT protocol for Dermatophagoides farinae (Df) allergy. METHODS: Seventy-nine adults (Df-sensitized and controls) were enrolled. Basophil activation was assessed by CD63 expression following Df extract, anti-IgE, or

Heat Processing Reduces IgE Binding but Not Basophil Sensitivity to Pea Proteins in Pea-Allergic Children.

BACKGROUND/OBJECTIVES: The increasing use of pea protein in plant-based foods raises concerns about IgE-mediated reactions, particularly in individuals sensitized to peanut. Knowledge on clinically relevant pea allergens and the impact of heat processing remains limited. This study investigated how thermal treatment affects the IgE binding and functional allergenicity of pea proteins in children with a confirmed pea allergy, with or without a concomitant peanut allergy. METHODS: Serum from 11 patients was analyzed using SDS-PAGE, Western blotting, and an indirect basophil activation test (iBA

The Basophil Activation Test in Allergy Diagnostics: Clinical Applications, Methodological Challenges, and Proposed Clinical Frameworks.

The basophil activation test (BAT) is a modern in vitro functional assay used for the diagnosis of immunoglobulin E-mediated allergic reactions. The method is based on flow cytometric assessment of activation marker expression on peripheral blood basophils after stimulation with specific allergens. BAT has gained increasing clinical relevance in allergology, particularly in patients with anaphylaxis risk, polysensitization, drug hypersensitivity, and inconclusive skin or serological test results. This narrative review summarizes current evidence on the diagnostic performance and clinical appli…

Peanut Allergy and the Basophil Activation Test in the Military Healthcare System: A Case Series of Monozygotic Twins.

Based on current standards for accession into the U.S. military, food allergy represents a significant barrier to military service. Skin prick and serum specific IgE testing exhibit high sensitivity but low positive predictive value, often leading to overdiagnosis when a confirmatory oral food challenge (OFC) is subsequently avoided because of concern for anaphylaxis in the clinic. Basophil activation testing (BAT) offers a unique advantage by measuring in vitro basophil responses to peanut allergen and can provide superior risk stratification for safe OFCs. We report the first clinical applic

Diagnosis of propofol allergy (the innovative role of the basophil activation test in the identification of allergen): a case report.

Drug allergies (DA), particularly to anesthetics, are underexplored in Morocco. Traditional diagnostic methods such as skin tests and specific IgE assays are costly, risky, and often inaccessible. The basophil activation test (BAT) is emerging as a safer and more effective alternative, especially for diagnosing propofol allergy, an under-researched area. This study examines the role of the basophils activation test in diagnosing perioperative anaphylaxis to propofol. A 32-year-old woman experienced two anaphylactic reactions during fibroscopy after propofol administration. The basophil activat…

Basophil Activation Test in IgE-Mediated Wheat Allergy: Diagnostic and Clinical Applications-A Narrative Review.

The basophil activation test (BAT) is an emerging tool in the diagnosis and management of IgE-mediated wheat allergies (WAs), particularly in complex or high-risk phenotypes. This narrative review explores the clinical applications of BAT across a spectrum of WA presentations, including wheat-dependent exercise-induced anaphylaxis (WDEIA), contact urticaria, and pediatric food allergy. The BAT provides a functional measure of allergen-triggered basophil activation, bridging the gap between mere sensitization and true clinical reactivity. We highlight its utility in detecting sensitization to s

Nematode Galectin Inhibits Basophilic Leukaemia RBL-2H3 Cells Apoptosis in IgE-Mediated Activation.

Mast cells are essential immune cells involved in the host's defence against gastrointestinal nematodes. To evade the immune response, parasitic nematodes produce a variety of molecules. Galectin 1, produced by Teladorsagia circumcincta (Tci-gal-1), reduces mast cell degranulation and selectively regulates mediator production and release in an IgE-dependent manner. To uncover the activity of Tci-gal-1, we have examined the effect of the protein on gene expression, protein production, and apoptosis in activated basophilic leukaemia RBL-2H3 cells. Rat RBL-2H3 cells were activated with anti-DNP I
