Clinical features of benign paroxysmal positional vertigo (BPPV) in Taiwan: differences between young and senior age groups.

BPPV is a common cause of vertigo. Several treatment procedures can facilitate recovery. In this study, we aimed to identify the demographic features, resolution and recurrence rates and impacts on daily activities in BPPV patient between young and senior age groups in Taiwan. This retrospective study recruited 218 patients of BPPV. Medical history, canal involvement, treatment required for complete resolution, symptom free period and recurrence rates were evaluated between the two age groups. Up to 80.7% of patients were successfully treated by a single treatment. For patients aged more than

Benign paroxysmal positioning vertigo in multiple sclerosis: diagnosis, pathophysiology and therapeutic techniques.

OBJECTIVE: To report on the most common causes of vertigo in patients with multiple sclerosis (MS) and emphasize appropriate diagnostic techniques and treatment interventions. BACKGROUND: True vertigo is estimated to occur in about 20% of MS patients. Lesions within the vestibular nuclei and in the root entry zone of cranial nerve VIII represent the most common locations where demyelinating activity can provoke vertigo in patients with MS. However, other causes of vertigo should be explored in MS patients in order to avoid unnecessary treatment with corticosteroids and vestibular suppressants

SERUM 25-HYDROXY VITAMIN D AND BONE MINERAL DENSITY IN MULTIPLE CANAL INVOLVEMENT OF BENIGN PAROXYSMAL POSITIONAL VERTIGO.

BACKGROUND AND OBJECTIVE: Benign paroxysmal positional vertigo (BPPV) has been associated with low serum 25-hydroxyvitamin D levels and reduced bone mineral density (BMD), but it remains unclear whether these factors contribute to involvement of multiple semicircular canals. This study aimed to evaluate whether decreased BMD or vitamin D concentrations are associated with multi-canal BPPV. METHODS: In this single-center retrospective study, 102 consecutive patients with idiopathic BPPV confirmed by video-oculography (VOG) were analyzed. All patients had serum 25-hydroxyvitamin D measurements …

Construction and clinical validation of benign paroxysmal positional vertigo intelligent auxiliary diagnosis model based on big data analysis.

BACKGROUND: Benign paroxysmal positional vertigo (BPPV) is the most common type of vertigo in clinical practice. Previous studies have suggested that inflammatory responses and metabolic disorders may be involved in the pathogenesis of BPPV, but systematic analyses based on large samples are lacking. The aim of this study is to construct an intelligent auxiliary diagnostic model for BPPV based on the big data of SRM-IV vertigo diagnostic and treatment system, and to carry out clinical validation. METHODS: The clinical data of 522 vertigo patients were retrospectively analyzed, including 303 B…

A minimal ion-chemistry model for predicting benign paroxysmal positional vertigo risk based on endolymphatic calcium and pH.

Benign paroxysmal positional vertigo (BPPV) arises from detachment of otoconia-calcium carbonate (CaCO3) crystals embedded in a protein matrix-whose stability depends on endolymph ionic composition and pH. Age-related calcium metabolism, acid-base imbalance, and hormonal factors can impair otoconia integrity, yet, to our knowledge, no prior quantitative model integrates these biochemical parameters to predict BPPV risk. Beyond the established mechanical mechanisms of canalithiasis and cupulolithiasis, we introduce a parsimonious biochemical model in which the endolymphatic saturation index (Ω)…

Clinical significance of cervical and ocular vestibular evoked myogenic potentials in benign paroxysmal positional vertigo: a meta-analysis.

PURPOSE: As the pathological cause of benign paroxysmal positional vertigo (BPPV), the dislocation or degeneration of otoconia in the utricle and saccule is suggested. Vestibular evoked myogenic potential (VEMP) could reflect otolithic dysfunction due to these etiologies of BPPV. The aim of this study was to validate the clinical significance of cervical (c) and ocular (o) VEMP in BPPV by a meta-analysis of previous articles. METHODS: Articles related to BPPV with data on cVEMP and oVEMP were collected. The following keywords were used to search PubMed and Scopus for English language articles…

A novel use of intratympanic dexamethasone for intractable posterior canal benign paroxysmal positional vertigo: report of two cases.

BACKGROUND: Benign paroxysmal positional vertigo is a common inner-ear pathology, characterised by episodic vertigo lasting for a few seconds that is associated with sudden change in the head position. Benign paroxysmal positional vertigo is treated with canalolith repositioning manoeuvres. Intractable vertigo describes a small group of patients who either do not improve with canalolith repositioning manoeuvres (persistent cases) or who relapse after improvement of initial symptoms (recurrent cases). These cases are difficult to treat and may have to be treated surgically.Case reportsThis pape…

Recurrence of symptoms following treatment of posterior semicircular canal benign positional paroxysmal vertigo with a particle repositioning manoeuvre.

A consecutive series of 51 patients (34 females and 17 males) with posterior semicircular canal benign positional paroxysmal vertigo (BPPV) were treated with the modified Epley particle positioning manoeuvre (PRM). Follow-up data were available on all 51 patients. After one manoeuvre, 42 patients had a negative Dix-Hallpike test, and after a second manoeuvre, 8 of the remaining 9 patients had a negative Dix-Hallpike test (testing was conducted 1 to 2 weeks after the PRM was performed). Therefore, the overall short-term success rate after two manoeuvres was 50 of 51 patients (98%), which is sim

[Canalith repositioning maneuver: proposal of a new therapy for benign paroxysmal positional vertigo of the posterior semicircular canal].

A new therapy is proposed for the treatment of Benign Paroxysmal Positional Vertigo (B.P.P.V.) of the Posterior Semicircular Canal (P.S.C.): the Canalith Repositioning Maneuver (C.R.M.). The need for a new maneuver to treat B.P.P.V. of the P.S.C. arises from the difficulties encountered in daily practice, under particular conditions (i.e. elderly, obese, traumatized patients and in the presence of rachis pain, etc.), to perform the most common rehabilitative techniques such as the Semont Maneuver and Epley's Canalith Repositioning Procedure (C.R.P.). The results achieved using this new techniq…

Association between vitamin D, vitamin D supplementation and benign paroxysmal positional vertigo: a systematic review and meta-analysis.

BACKGROUND: Benign paroxysmal positional vertigo (BPPV) is one of the most prevalent peripheral vestibular dysfunctions encountered in clinical practice, including dizziness and vertigo, which has a significant impact on people's everyday lives and affects their quality of life in many ways. Researches indicate that individuals with recurrent benign paroxysmal positional vertigo (BPPV) may exhibit vitamin D insufficiency, and certain studies suggest that correcting severe vitamin D deficiency might effectively reduce BPPV recurrence; nevertheless, the findings have been inconsistent. As a resu…
