Monocular blindness in Bayelsa state of Nigeria.

BACKGROUND: Monocular blindness has not received much attention in developing countries despite its numerous disadvantages. A monocularly blind person is at high risk of being bilaterally blind. The objective of this study was to determine the causes of monocular blindness in the study population and to suggest strategies for prevention. METHODS: A prospective study was conducted among new consecutive patients that presented to our clinic over a period of one year and those with visual acuity (VA) less than 3/60 in the worse eye after optical correction or with pin hole as necessary were stud…

1 rhodopsin mutations in congenital night blindness.

While there are over 100 distinct mutations in the rhodopsin gene that are found in patients with the degenerative disease autosomal dominant retinitis pigmentosa (ADRP), there are only four known mutations in the rhodopsin gene found in patients with the dysfunction congenital stationary night blindness (CSNB). CSNB patients have a much less severe phenotype than those with ADRP; the patients only lose rod function which affects their vision under dim light conditions, whereas their cone function remains relatively unchanged. The known rhodopsin CSNB mutations are found clustered around the s…

[Sudden blindness: consider Leber's hereditary optic neuropathy].

In 3 young male patients, aged 10, 19 and 21 years respectively, sequential, severe, painless bilateral visual loss occurred. Ophthalmological examination revealed no other abnormalities and this delayed the diagnosis Leber's hereditary optic neuropathy (LHON). LHON is a mitochondrial genetic disease characterised by bilateral acute or subacute painless loss of central vision. LHON causes blindness, predominantly in young adult males but less frequently in women and children as well. Occasionally, LHON is associated with other neurological and cardiac changes. The first patient recovered his v…

Rapid Assessment of Avoidable Blindness in India.

BACKGROUND: Rapid assessment of avoidable blindness provides valid estimates in a short period of time to assess the magnitude and causes of avoidable blindness. The study determined magnitude and causes of avoidable blindness in India in 2007 among the 50+ population. METHODS AND FINDINGS: Sixteen randomly selected districts where blindness surveys were undertaken 7 to 10 years earlier were identified for a follow up survey. Stratified cluster sampling was used and 25 clusters (20 rural and 5 urban) were randomly picked in each district.. After a random start, 100 individuals aged 50+ were e…

[Blindness following reduction of a malar fracture].

Blindness is the most serious injury to the eye following a midfacial fracture. The onset of blindness complicating the midfacial fracture may be immediate (with the trauma), delayed (hours to days), or postoperative. The incidence of postoperative blindness lies between 0.3 and 8.3%. There are multiple mechanisms responsible for blindness but the most common reason with about 66% is probably increased intraorbital pressure attributable to intraorbital hemorrhage. The management of surgical decompression of the orbit for acute blindness is essential when treatment of a midfacial fracture is in…

Psychiatric and behavioral correlates of factitious blindness.

In factitious disorder, an individual feigns, exaggerates, or actually self-induces physical or psychiatric illness to achieve ends such as mobilizing care and concern, ventilating aggression, diminishing guilty feelings, or gratifying dependency wishes. We present 2 new cases of factitious binocular blindness, which has rarely been reported but readily illustrates the dramatic range of factitious illness behaviors. Psychiatric and behavioral correlates culled from these cases include extravagant claims about the impairment and ludicrous claims for nonvisual sensory abilities. These patients t…

[International initiatives for the prevention of blindness].

Since 1974, when the International Agency for the Prevention of Blindness (IAPB) was founded, international campaigns for blindness prevention have gained tremendous momentum in collaboration with the WHO's blindness prevention programme. In 1999 WHO and IAPB launched the campaign Vision 2020, The Right to Sight, in which WHO, IAPB, member countries of WHO and nongovernmental organizations (NGOs) that are active in this field are all working together. The major emphasis of the programme is on control of the major disorders leading to blindness and the development of infrastructures, of eye car…

River blindness: a success story under threat?

The success of the Onchocerciasis Control Programme is undeniable and exemplary, say the authors, but it is too early to claim victory against river blindness.

Inattentional blindness for a noxious multimodal stimulus.

Previous research has shown that people can miss salient stimuli outside the focus of their attention. This phenomenon, called inattentional blindness, typically is observed when people are given a task requiring them to focus their attention on one aspect of a complex visual scene. While participants are doing this task, an unexpected stimulus appears, and participants' awareness of it is tested shortly thereafter. In the present experiments, noxious bimodal stimuli were used as a test case to measure the strength of inattentional blindness. We tested whether participants would notice a perso…

Visual acuity and X-linked color blindness.

PURPOSE: Optimal sampling for visual acuity requires a fine array of cones with identical sensitivity. Thus, dichromats, whose inner fovea is made up of cones having the same spectral sensitivity, may have better than normal visual acuity. We investigated this by comparing the visual acuities of trichromats and X-linked dichromats, while taking into account the different molecular genetics underlying the disorder. METHODS: Our subjects were age- and refraction-matched groups of normals (n=8) and X-linked dichromats (n=13). The dichromats (four protanopes and nine deuteranopes) were genotyped

Etiology of childhood blindness in Izmir, Turkey.

PURPOSE: To identify the anatomic sites and the etiology of childhood blindness and to discern treatable and preventable causes. METHODS: The records of 998 patients seen in the authors' pediatric ophthalmology unit between June 1998 and May 2002 were examined retrospectively. A total of 148 patients who had visual impairment and blindness according to World Health Organization criteria were included in the study. They are classified according to the etiology based on time of insult and the anatomic site of visual loss. RESULTS: The most common anatomic site of visual loss was retina, with a…

Favorable outcome of epileptic blindness in children.

Acute blindness is a rare presentation of epileptic seizures, referring to loss of sight without loss of consciousness associated with electroencephalographic (EEG) epileptic discharges, mainly representing an ictal phase but also either pre- or postictal. We report a series of 14 children with documented epileptic blindness, describing the accompanying fits and thereafter the response to therapy to resolve the blindness and control associated seizures. All patients experienced episodes of acute complete visual obscuration lasting for 1 to 10 minutes. Seven patients hadaccompanying generalized…

Blindness prevention programmes: past, present, and future.

Blindness and visual impairment have far-reaching implications for society, the more so when it is realized that 80% of visual disability is avoidable. The marked increase in the size of the elderly population, with their greater propensity for visually disabling conditions, presents a further challenge in this respect. However, if available knowledge and skills were made accessible to those communities in greatest need, much of this needless blindness could be alleviated. Since its inception over 50 years ago, and beginning with trachoma control, WHO has spearheaded efforts to assist Member S…

Endoscopic optic nerve decompression for traumatic blindness.

OBJECTIVES: The goal of this study was to compare the outcome of patients with traumatic optic neuropathy (TON) treated with high-dose steroids with the outcome of patients with TON treated with endoscopic optic nerve decompression (EOND) after failing high-dose steroid treatment. METHODS: During this retrospective review of patients with TON seen from 1994 to 1998, all patients were first treated with megadose methylprednisolone for 48 hours. Patients with no improvement or with worsening visual acuity were offered EOND. RESULTS: Eleven of 34 (32%) patients treated with high-dose steroids s
