Prospective blind comparative clinical study of two point fixation of zygomatic complex fracture using wire and mini plates.

BACKGROUND: The zygomatic maxillary complex (ZMC) fractures are one of the most frequent injuries of the facial skeleton due to its position and facial contour. Assaults, road traffic accidents and falls are the principal etiologic factors that may cause fractures of zygomatic bone. The different fixation methods are applied to treat the zygomatic bone fractures, with many more classifications which have been described in the literature for the ease of management. The type of the fracture, its severity and associated facial fractures usually interferes the treatment modality. PURPOSE OF STUDY

Unilateral biportal endoscopic technique combined with percutaneous transpedicular screw fixation for thoracolumbar burst fractures with neurological symptoms: technical note and preliminary report.

BACKGROUND: Previous studies on thoracolumbar fractures with neurological symptoms have focused on how to achieve satisfactory fracture reduction, adequate nerve decompression, and stable spinal alignment. With the development of the minimally invasive spine surgery technique, achieving satisfactory treatment results and reducing iatrogenic trauma at the same time has become a new goal of spinal surgery. This research used percutaneous transpedicular screw distraction to partially reduce the fractured vertebrae, followed by completing nerve decompression and reducing residual displacement bone

A new sequential animal model for infection-related non-unions with segmental bone defect.

BACKGROUND: The treatment of fracture-related infections (FRI) is still a challenge for orthopedic surgeons. The prevalence of FRI is particularly high in open fractures with extensive soft-tissue damage. This study aimed to develop a new two-step animal model for non-unions with segmental bone defects, which could be used to evaluate new innovative bone substitutes to improve the therapeutic options in humans with FRI and bone defects. METHODS: After randomization to infected or non-infected groups, 30 Sprague-Dawley rats underwent a transverse osteotomy of the mid-shaft femur with a 5 mm de

[Reposition instrument for Bennett's fracture-dislocation].

A special hook and plate can be used to adapt small central fragments in the open reduction of Bennett's fracture-dislocation. The hook is positioned behind the small central fragment and compression exerted between the hook and a plate attached to the opposite side of the base of the first metacarpal bone using an adjustable nut, thus holding the fragments together. The reduction device simplifies X-ray controls and internal fixation with Kirschner wires.

Self-stabilizing osteotomy for frontal bar advancement.

This technique is simple, quick to perform, and produces a rigid block against posterior relapse of the advanced frontal bar in surgery for bicoronal synostosis. This stability is achieved without the need to place a bone graft across the craniectomy site in small infants with rapidly expanding brains. Finer and fewer interosseous wires are required, decreasing the chance of transcutaneous palpation, and this principle can be incorporated into most osteotomy patterns around the orbits.

Rigid facial skeletal fixation: advances in treatment.

Rigid facial skeletal fixation with miniplates, combined with craniofacial reconstruction techniques, has dramatically changed the treatment of facial fractures. Our use of these newer techniques in 142 facial fractures treated during the past 14 months has resulted in improved aesthetics and function, with a low complication rate. Advances over other techniques include (1) craniofacial incisions with wide subperiosteal exposure, (2) rigid skeletal fixation that virtually eliminates the need for continued intermaxillary fixation, suspension wires, or tracheostomy, (3) primary bone grafting wit

Immobilization of the mallet finger. Effects on the extensor tendon.

Since the mallet finger that is treated with isolated splinting of the distal interphalangeal (DIP) joint can be moved freely proximal to the DIP joint, we sought to determine whether such motion might cause a tendon gap that could explain the extensor lag that often follows treatment. Experiments were performed on 32 cadaveric fingers with open mallet finger lesions, immobilizing either the DIP joint alone or both the DIP and PIP joints, while repeatedly flexing and extending the more proximal finger and wrist joints. For each experiment, the gap in the extensor tendon was measured. Joint mot

Is arthroscopic-assisted treatment applicable for the treatment of pediatric scaphoid nonunion?

Pediatric scaphoid nonunion is uncommon. Immobilization is initially attempted; however, it causes delays in both intervention and healing. Therefore, surgical intervention has gained broader acceptance and has been implemented in recent years. Among these, arthroscopic management has emerged as a contemporary technique. This study evaluated the clinical outcomes of pediatric scaphoid nonunion treated with arthroscopic-assisted bone grafting and Kirschner (K) wire fixation. Six pediatric patients with scaphoid nonunion treated with arthroscopic bone grafting and K-wire fixation between Novembe

Indications for and Risks Associated With Implant Removal After Pediatric Trauma.

A wide range of implants are used in the treatment of pediatric fractures, including wires, plates, screws, flexible rods, rigid rods, and external fixation devices. Pediatric bones differ from adult bones both mechanically and biologically, including the potential for remodeling. Implants used in pediatric trauma patients present a unique set of circumstances regarding indications, risks, timing of implant removal, weight-bearing restrictions, and long-term sequelae. Indications for implant removal include wire/pin fixation, when substantial growth remains, and infection. When considering imp

Reduction of the scaphoid fracture with DISI alignment.

Eight cases of symptomatic malunion and nonunion of scaphoid fractures with dorsiflexed intercalated segment instability (DISI) alignment were treated by open reduction, anterior wedge-shaped bone grafting, and internal fixation using Herbert's screw. In four malunited fractures, osteotomy at the fracture line was done before the reduction. The DISI alignment was reduced by insertion of the Kirschner wire into the lunate and flexing it anteriorly. After the bone grafting to the anterior bony defect of the scaphoid, which was created by the Kirschner wire reduction, internal fixation was accomp

Arthrodesis of the interphalangeal joints of the hand by two-dimensional intraosseous wiring.

BACKGROUND: Numerous techniques for arthrodesis have been described to fix interphalangeal (IP) joints, and the fixation method should be considered on a case-by-case basis. This study aimed to investigate the availability of IP joint arthrodesis of the hand, using a two-dimensional intraosseous wiring (two-DIOW) method. METHODS: A total of 43 joints (19 thumb IP joints, 9 proximal finger interphalangeal (PIP) joints and 15 distal interphalangeal (DIP) joints in 29 patients with a mean age of 66 years (range, 24-85 y) were retrospectively analyzed. All operations were performed with two-DIOW

Combination of a miniplate with tension band wiring for inferior patellar pole avulsion fractures.

BACKGROUND: The tension-band technique, with Kirschner wires or cannulated screws, is one of the most commonly used fixation techniques for patellar fractures. However, it may be not useful for inferior patellar pole fractures as it may lead to fragment reduction loss and fixation failure. We present a surgical technique that combines a miniplate with tension band wiring for inferior patellar pole avulsion fractures and report surgical outcomes. PATIENTS AND METHODS: Between June 2012 and May 2016, 17 consecutive patients (mean age: 51 years) with inferior patellar pole fractures (AO/OTA 34-A

Atlantoaxial Reconstruction: The Artful Evolution of Craniovertebral Junctional Spine Surgery.

The atlantoaxial (C1-2) junction is among the most technically demanding regions for cervical spine surgery owing to its complex osseoligamentous anatomy and proximity to critical neurovascular structures. Numerous posterior fixation constructs have been developed to optimize biomechanical rigidity and promote arthrodesis. Since Gallie's introduction of posterior wiring with autologous bone grafts in 1939, evolving techniques have focused on enhancing fusion rates while minimizing risk to adjacent structures. This paper outlines the historical evolution of C1-2 posterior instrumentation, curre
