Principles of rotationplasty.

Rotationplasty is a reconstructive, limb-sparing option for management of lower extremity bone deficiency. This technique involves an intercalary resection, followed by 180° rotation of the distal limb to allow the ankle to function as a knee joint when it is fitted with a modified below-knee prosthesis. Gait analysis and functional outcome studies have reported favorable results with rotationplasty compared with those of above-knee amputation. Moreover, patient satisfaction with rotationplasty is higher than with other limb salvage procedures. The primary drawback of this procedure is patient

Legg-Calvé-Perthes disease: a study of lower extremity length discrepancies and skeletal maturation.

Lower extremity length discrepancies and skeletal maturation have been studied in 147 patients with unilateral Legg-Calvé-Perthes disease followed by orthoroentgenograms for 5 or more years. At the time of initial assessment there was a marked delay in skeletal age as related to chronologic age in 83 percent of the patients. Standing height in the majority of patients both at disease presentation and at skeletal maturation was less than the mean. The average maximum total femoral and tibial discrepancy during the course of the disease was 2.14 cm. The maximum femoral discrepancy averaged 1.38

Traumatic fractures resulting from collisions in children and adolescents: A retrospective observational study.

To investigate the incidence and pattern of child and adolescent (≤18 years old) traumatic fractures (TFs) as a result of collisions. We retrospectively reviewed 270 child and adolescent patients (228 males and 42 females aged 12.8 ± 5.1 years old) with TFs as a result of collisions admitted to our university-affiliated hospitals from 2001 to 2010. The incidence and patterns were summarized with respect to different age groups, sex, etiology, and whether the patient presented with nerve injury. The most common etiologies were struck by object (105, 38.9%) and wounded by person (74, 27.4%). The m

Modeling of the human leg in ski injuries.

This study establishes that the human lower extremity-ski system has a fundamental frequency of approximately 1 Hz. The fundamental mode dominates the system response when loading times are short compared to the first natural frequency. Consequently, the use of a single degree of freedom approximation and the use of impulse criteria can provide important insights into the problem of skiing injuries. Specifically, the sport of snow skiing develops forces with frequency content in the static, transition, and impulse regions of the leg-ski response spectrum. Therefore, ski release bindings must b

Establishing the Turkish version of the SIGAM mobility scale, and determining its validity and reliability in lower extremity amputees.

PURPOSE: The aim of this study is to adapt the Special Interest Group in Amputee Medicine (SIGAM) mobility scale to Turkish, and to test its validity and reliability in lower extremity amputees. MATERIAL AND METHODS: Adaptation of the scale into Turkish was performed by following the steps in American Association of Orthopedic Surgeons (AAOS) guideline. Turkish version of the scale was tested twice on 109 patients who had lower extremity amputations, at hours 0 and 72. The reliability of the Turkish version was tested for internal consistency and test-retest reliability. Structural validity w

[Post-traumatic lymphatic and venous drainage changes in persistent edema of lower extremities].

Mechanical injury of soft tissues and bones of the lower extremity is followed by chronic edema at the site of trauma and distally to it. This complication affects almost every patient with a fracture of the lower limb. The question is whether posttraumatic edema is due to lymphatic obstruction, venous thrombosis or both, or a local cytokine and growth factor hyperactivity at the fracture site. The aim of study was to assess the venous and lymph outflow in patient with chronic postraumatic edema of the lower limbs. A group of 19 patients with chronic edema lasting for more than 3 months was ev

The importance of axial rotation of the lower limb : a systematic review of measurement methods.

AIMS: Assessment of lower limb alignment is a cornerstone of orthopaedics. Few studies look at rotational alignment in the axial plane as measured by femoral version (FV) and tibial torsion (TT), both of which have implications for hip, knee, and ankle pathology. This review provides an overview of the axial rotation and evaluates CT-based measurement methods for FV and TT to identify the most reliable and reproducible techniques for use in clinical practice. METHODS: A systematic PRISMA-guided review assessed original CT-based methods, examining inter- and intraobserver reliability (intracla

A young patient with lower limb Ewing's sarcoma experienced a cerebral infarction: A care-compliant case report.

RATIONALE: Ewing sarcoma (ES) is a rare bone tumor that most often occurs in adolescents and young adults and is a malignant tumor that mainly affects, bones and soft tissues.[1] Trousseau syndrome refers to a series of blood clot events in veins and arteries that occur during malignant tumor progression due to problems with blood clotting and breakdown. It includes clinical manifestations such as pulmonary embolism, arterial thrombosis, deep vein thrombosis, and nonbacterial thrombotic endocarditis. Acute cerebral infarction can also occur as part of Trousseau's syndrome. Due to the rarity of

Below-the-Radar: A Case of Extensive Osseous Metastasis of Prostate Cancer Confined Exclusively to One Lower Limb.

Osseous metastases are a common finding in prostate cancer, usually affecting the central skeleton. Metastases in the peripheral skeleton are less frequent but can occur as isolated findings as well. Therefore, imaging protocols limited to the trunk may miss such lesions and extended range (total-body) PSMA-PET/CT may be helpful for comprehensive evaluation, especially when peripheral metastases are suspected, for example, due to clinical symptoms.

[Not Available].

Beak fractures represent a rare subtype of calcaneal fractures with potential risk of soft tissue complications due to compromised local perfusion. Early diagnosis and timely intervention are crucial to prevent necrosis, infection, and soft tissue defects. This case report describes a 71-year-old male with a beak fracture and delayed intervention with reoccurring soft tissue defects. Given the rarity of the condition, atypical presentation and need for urgent intervention, this emphasizes the importance of awareness of beak fractures of the calcaneus.

Effectiveness of FES-supported leg exercise for promotion of paralysed lower limb muscle and bone health-a systematic review.

Leg exercises through standing, cycling and walking with/without FES may be used to preserve lower limb muscle and bone health in persons with physical disability due to SCI. This study sought to examine the effectiveness of leg exercises on bone mineral density and muscle cross-sectional area based on their clinical efficacy in persons with SCI. Several literature databases were searched for potential eligible studies from the earliest return date to January 2022. The primary outcome targeted was the change in muscle mass/volume and bone mineral density as measured by CT, MRI and similar devi

[Do not overlook Lisfranc injuries].

BACKGROUND: Lisfranc injury is rare and often very subtle. Subsequently, 20-50% of injuries are initially missed. Late or missed diagnosis increases the risk of post-traumatic osteoarthritis. CASE: A 51-year old men visited our surgical outpatient clinic with plantar hematoma and inability to bear weight on the right foot after he tripped three weeks earlier. Initial visitation to the emergency department after trauma led to the diagnosis contusion or ligament injury of the foot/ankle. After additional CT-scan of the foot, the final diagnosis of subtle/stable Lisfranc injury was made. Twelve

Surgical Outcomes, Complications, and Long-Term Functionality for Free Vascularized Fibula Grafts in the Pediatric Population: A 17-Year Experience and Systematic Review of the Literature.

BACKGROUND: In the pediatric population, bony defects of the extremities pose a significant challenge for which free vascularized fibular grafts (FVFGs) represent a valuable reconstructive option. The purpose of this study was to explore surgical outcomes, complications, and long-term functionality of FVFG for this patient group. METHODS: Using MEDLINE and PubMed databases, studies were identified of pediatric extremity reconstruction using FVFG which reported functional outcomes and/or complications. The operative logs at a tertiary pediatric center were then reviewed for cases of FVFG betwe

Dysplasia epiphysealis hemimelica of the lower limb.

Dysplasia epiphysealis hemimelica (DEH) is a rare developmental disorder characterized by osteocartilaginous overgrowth in one or more epiphyses. The disease usually involves a single limb or is hemimelic (lateral or medial compartment), and lower extremities are more frequently affected than upper extremities. Here we present clinical and radiological findings for a male DEH patient at 1.5 and 3.5 years of age. The radiographs obtained at first presentation showed minimal osseous overgrowth and irregularity at the epiphyses around the left knee and ankle joints, respectively. Radiographs obta

[Case study with a gunshot fracture of lower extremities and damage of popliteal artery (case report)].

Severe vascular gunshot injury (popliteal artery damage) and fractures of both low extremities are causes traumatic shock (stage III) and anemia in a 32 years female patient. Being the victim of crime, the patient for 5 hours was in a life-threatening condition that could develop the multiple organ system failure (MOSF) as a result of tissue ischemia and reperfusion and acute irreversible shock. There was an urgent necessity to perform three immediate operations at the same time. Successful recovery required rapid control of the inciting event (i.e., maintenance of effective hemodynamic stabil

Limb lengthening and deformity correction of congenital and acquired deformities in children using the Taylor Spatial Frame.

Background and purpose - Hexapod ring fixators such as the Taylor Spatial Frame (TSF) have shown good outcomes. However, there have only been a few studies comparing the use of TSF with various etiologies of the deformity. We compared the use of TSF in congenital and acquired deformities in children. Patients and methods - We reviewed 213 lower extremity reconstructive procedures with the TSF in 192 patients who were operated between October 2000 and October 2015. 128 procedures (67 proximal tibiae, 51 distal femora, and 10 distal tibiae) in 117 children (median age 14 (4-18) years; 59 girls)
