The brain matrix and multifocal brain damage following a single injection of ketamine in young adult rats: conspicuous changes in old age.

Male rats were seized with lithium and pilocarpine and then injected within 30 min. with either acepromazine or ketamine. These rats as well as age-matched normal rats were observed daily for one year. The rats which had received the ketamine after the seizures were significantly heavier than either the normal rats or the other group of seized rats. The bulk of this increased weight was due to the marked increase in white, extremely dense adipose tissue. Compared to the acepromazine-treated rats, the ketamine-treated rats did not exhibit spontaneous seizures and exhibited cerebral widths compa

[Pattern of the brain damages and its developmental mechanisms in monozygotic twins].

Neurologic morbidity is observed more frequently in twins than in singletons. Low birthweight and premature birth are observed more frequently in twins but cannot always explain the cause of cerebral palsy, although circumstances related to twinning may cause brain damage. We attempted to select the patients whose brain damages might be caused by circumstances related to twinning and discussed the developmental mechanisms of their brain damages. We divided the cases into 3 groups. One comprised the monozygotic (MZ) twins whose co-twins had died. The second comprised the MZ twins with the compl

Glucose, lactic acid, and perinatal hypoxic-ischemic brain damage.

Investigations suggest that hyperglycemia, superimposed on hypoxia-ischemia or cerebral ischemia, accentuates brain damage in adult experimental animals and humans, but not in immature animals. Fundamental differences in the immature and adult brain, which account for the age-specific paradox, are discussed. Based on currently available data, we recommend that glucose supplementation not be curtailed during labor and delivery of asphyxiated human infants; on the contrary, glucose therapy may substantially reduce hypoxic-ischemic brain damage.

Category-specificity and modality-specificity in semantic memory.

Studies of agnosia have revealed two apparently orthogonal dimensions along which knowledge may break down. In some cases, knowledge of specific categories (such as living things) seems lost, regardless of the modality being tested. In other cases, knowledge in specific modalities (such as vision) seems lost, regardless of the category of stimuli being tested. These different sets of phenomena suggest different organizations for knowledge in the brain, the first by category and the second by modality. Unfortunately, possible confoundings between category, modality, and difficulty level in the

Gradiental approach to neocortical functional organization.

Examination of neuropsychological syndromes suggests strong congruence between neuroanatomical and functional neocortical metrics. Two cortical syndromes disrupt cognitively close functions if and only if their neuroanatomical territories are close. To capture this relationship, the concept of a cognitive gradient is introduced as a basic unit of macroscopic neocortical brain-behavioral analysis. A cognitive gradient is a continuous distribution of related functions along an axis defined at its extremes by a pair of sensory projection or motor areas. Cortical gradiental structure is viewed as

Prognostic utility of SEPs in comatose children.

Somatosensory evoked potentials (SEPs) were recorded in 73 comatose children upon admission to the intensive care unit and were studied in respect to initial neurologic status and final outcome. SEP results were graded normal, increased interpeak latencies, and unilaterally or bilaterally absent cortical responses. Of the 50 patients with Glasgow Coma Scale scores less than 7 upon admission, only 3 had SEPs within the normal range, while 37 had unilaterally or bilaterally absent SEPs. None of the 27 who died had normal SEPs; 1 had increased interpeak latencies, 26 had more abnormal SEPs. The 1

Temperature instability as an early predictive factor of brain death in paediatric near-drowning victims.

Predicting outcome after near-drowning has been extensively studied. During four years, 42 near drowned children were aggressively treated with positive pressure ventilation, barbiturates and hypothermia. This mode of treatment makes it difficult to clinically assess the child's prognosis. Temperature instability after rewarming is an early negative predictive factor following treatment of near-drowning, and in conjunction with cerebral flow studies avoids the potential commitment to prolonged and unwarranted cardiovascular and respiratory support.

Critical care of the child with increased intracranial pressure.

The nursing care of children with increased intracranial pressure is extremely complex. For children with intracranial pressure monitoring devices in place or for those who are maintained on a barbiturate coma regimen, the nurse must have extensive knowledge regarding the proper operation of the complex equipment that is helping to maintain the child's life. However, no matter how sophisticated the monitoring equipment or no matter what new research protocol is being followed, the child's life is dependent upon the nurse providing care. If the nurse fails to interpret the clinical findings cor

[Value of computerized axial tomography of the brain in children born with asphyxia (author's transl)].

Computerized axial tomographies (CAT) of the brain were performed between the ages of 2 days and 2 years in 27 infants having suffered from severe birth asphyxia. CAT were normal in 11 babies, hemorrhage was found in 5 infants, hypodense areas in 3, brain edema in 4, atrophy in 5, pseudo-porencephalic cysts in 2 and lesions of the central nuclei in 2. An abnormal CAT was demonstrated in 12 of 17 patients who inhibited major neurological troubles during the neonatal period and in 4 of 10 who suffered less important disorders. 16 of 23 survivors were followed up after they left the hospital. 7 s

EEG monitoring for induced hypotension for surgery of intracranial aneurysms.

EEG was monitored at bilateral scalp sites outside the operative field during hypotensive aneurysm surgery in 21 patients. Mean arterial blood pressure at axillary level was 50-60 mm Hg (average 55 mm) for 1.9-5.3 hours (average 3.6). Four new deficits were noted immediately post-operatively, all related to the operated site: these were attributable to intra-operative rupture with forced vascular clipping, vasospasm, or edema. In no instance was hypotension solely responsible for a new deficit. EEG showed persistent slowing in relation to surgery in only 1 patient, where aneurysmal rupture led

Potential Protective Effect of Dl-3-n-butylphthalide on Chronic Cerebral Ischemia Brain Injury.

Chronic cerebral ischemia is one of the common ischemic cerebrovascular diseases. Chronic cerebral ischemia can lead to brain dysfunction, and its pathophysiological mechanism involves inflammation, blood-brain barrier destruction, oxidative stress, and other factors. As it is difficult to detect, it is easily overlooked, and it is often only observed following the onset of cognitive dysfunction. At present, there are only a few drugs for its treatment. Dl-3-n-butylphthalide (NBP), a compound extracted from celery seed, may play an important role in protecting against brain damage caused by ch

Correlation of Cerebral and Subcutaneous Glycerol in Severe Traumatic Brain Injury and Association with Tissue Damage.

BACKGROUND: This study is a substudy of a prospective consecutive double-blinded randomized study on the effect of prostacyclin in severe traumatic brain injury (sTBI). The aims of the present study were to investigate whether there was a correlation between brain and subcutaneous glycerol levels and whether the ratio of interstitial glycerol in the brain and subcutaneous tissue (glycerolbrain/sc) was associated with tissue damage in the brain, measured by using the Rotterdam score, S-100B, neuron-specific enolase (NSE), the Injury Severity Score (ISS), the Acute Physiology and Chronic Health

[The impact of uncontrolled hypertension on the CNS].

The brain is a target of organ damage due to hypertension. In addition to acute damage in the form of hypertensive encephalopathy, ischaemic stroke, and intracerebral haemorrhage, hypertension causes chronic changes in the brain tissue that, over the course of years, will be manifested by impaired brain functions including cognitive deficit. Hypertension is also a risk factor for progression of cognitive disorder to overt dementia. It is commonly accepted that the earlier in life hypertension occurs, the greater the risk of developing dementia in old age. The pathophysiological mechanism under

[Transcranial Doppler sonography in children. 2. Clinical uses].

Transcranial Doppler sonography of basal cerebral arteries is helpful for diagnosis and control of the course of paediatric diseases like patent ductus arteriosus, perinatal brain damage, increased intracranial pressure, cerebral malformations and stenoses and occlusions of main cerebral arteries. The technique also allows control of hyperventilation therapy of cerebral oedema. With a few exceptions, the Doppler results are not specific for a particular disease. However, they do supply important information on the specific pathophysiology of cerebral haemodynamics. The evaluation of the record

Computerized axial tomography in the detection of brain damage. 2. Epilepsy, migraine, and general medical disorders.

The cranial computerized axial tomography (CAT) findings in groups of patients with epilepsy, migraine, hypertension, and other general medical disorders have been reviewed to assess the frequency and patterns of focal and diffuse brain damage. In addition to demonstrating focal lesions in a proportion of patients with seizures and in patients presenting with a stroke, the CAT scan showed a premature degree of cerebral atrophy in an appreciable proportion of patients with long-standing epilepsy, hypertension and diabetes, and in some patients with migraine, valvular and ischaemic, heart diseas

Partial complex I deficiency due to the CNS conditional ablation of Ndufa5 results in a mild chronic encephalopathy but no increase in oxidative damage.

Deficiencies in the complex I (CI; NADH-ubiquinone oxidoreductase) of the respiratory chain are frequent causes of mitochondrial diseases and have been associated with other neurodegenerative disorders, such as Parkinson's disease. The NADH-ubiquinone oxidoreductase 1 alpha subcomplex subunit 5 (NDUFA5) is a nuclear-encoded structural subunit of CI, located in the peripheral arm. We inactivated Ndufa5 in mice by the gene-trap methodology and found that this protein is required for embryonic survival. Therefore, we have created a conditional Ndufa5 knockout (KO) allele by introducing a rescuing

The activation of red blood cell transketolase in groups of patients especially at risk from thiamin deficiency.

Erythrocyte transketolase activation by thiamin diphosphate has been studied in elderly patients with moderate or severe chronic dementia, acute alcoholic admissions and chronic alcoholics with evidence of brain damage, mostly of the Wernicke-Korsakoff type. Significantly more patients in each group than controls showed abnormal activation of transketolase, not only by 0.3 mM thiamin diphosphate (TDP) but also in further activation by increase to 3 mM. This indicated the presence in a proportion of the alcoholic and the demented patients of an abnormal enzyme variant, similar to that previousl

[Neonatal EEG findings soon after birth in the intrauterine growth retarded infant].

We performed electroencephalograms in 32 neonates of intrauterine growth retardation (IUGR) within seven days of birth. EEG findings were compared with those observed in neonatal brain damage. Seven EEGs had a dysmature pattern which is seen in the subacute and chronic stages of prolonged and mild neonatal brain depression, and five had a disorganized pattern which is seen in the subacute and chronic stages of severe neonatal brain depression. The other EEGs had a normal neonatal pattern. Seven neonates who had a dysmature pattern were severe IUGR (< or = 2.5 SD). Their head circumference was
