Brain stem infarction and diaschisis. A SPECT cerebral perfusion study.

BACKGROUND AND PURPOSE: We studied six patients suffering from pure, unilateral brain stem infarction to explore the association of remote cerebral and cerebellar blood flow changes with damage at different sites of this region of the brain. METHODS: We used single-photon emission computed tomography and [123I]iodoamphetamine to measure regional differences in tracer uptake. Qualitative image analysis and calculated asymmetry indexes were correlated to the location of the infarcted area on magnetic resonance imaging and to the patients' clinical findings. RESULTS: Significant perfusion asymm

Brain stem auditory evoked potentials from bone stimulation in dogs.

Brain stem auditory evoked potential (BAEP) testing with air-conducted click stimuli can be used to diagnose sensorineural deafness in dogs if conductive deafness can be ruled out. Detection of conductive deafness can be performed by recording BAEP elicited by a vibratory stimulus transducer placed against the skull. Air- and bone-conducted BAEP were compared in dogs, varying bone stimulator placement, click polarity, and stimulus intensity. Optimal bone stimulator placement was determined to be over the mastoid process, followed by the mandible and the zygomatic arch. Condensation polarity cl

Correlations of neuroanatomical measures to auditory brain stem response latencies.

Magnetic resonance imaging (MRI) was used to obtain measures presumed to scale the dimensions of the lower auditory pathway in humans for the purpose of further defining the relationship between length of the auditory pathway and auditory brain stem response (ABR) latencies. Specifically, measurements of soft tissue structures, that is, the eighth nerve and brain stem, were made for comparison with skull dimensions and ABR latencies. It was hypothesized that the brain stem dimensions covary significantly with skull dimensions and that the ABR parameters covary with both skull and brain stem di

Brain stem mechanisms of conditioned taste aversion learning in rats.

Acquisition of conditioned taste aversion (CTA) in rats is not prevented by functional decortication, anesthesia or hypothermia applied after intake of the flavored fluid and maintained throughout the action of the poison but is disrupted by bilateral application of 10 ng tetrodotoxin (TTX) into the parabrachial nuclei. The blockade is directly proportional to TTX dosage, indirectly proportional to distance of the injection site from parabrachial nuclei and equally affects CTAs using different CS (saccharin, NaCl) and different US (LiCl, carbachol, amphetamine, cycloheximide). CTA is disrupted

Magnetic resonance images of brain stem infarct in periarteritis nodosa.

A 35-year-old woman displayed unusual symptoms of progressive bulbar dysfunction. At necropsy, systemic necrotizing vasculitis and small pontine infarcts were demonstrated. The angiographic and magnetic resonance imaging findings are described. The latter investigation is a sensitive method for detecting such pontine lesions. To our knowledge, this case is the first report of magnetic resonance imaging findings of brain stem infarct in a patient with periarteritis nodosa.

Auditory brain stem responses in infants with posthaemorrhagic ventricular dilatation.

Nineteen infants with posthaemorrhagic ventricular dilatation had auditory brain stem responses measured during the period of maximal ventricular dilatation. These showed various patterns ranging from normal, through various abnormalities, to complete absence of responses. When serial auditory brain stem responses were studied in parallel with the evolution of posthaemorrhagic ventricular dilatation it was seen that the abnormalities of auditory brain stem response usually resolved irrespective of the persistence or progression of ventricular dilatation. No correlation was found between cerebr

[Clinico-physiologic characteristics of brain stem disorders in comatose states].

Informative signs for classification of disorders of consciousness were distinguished on the basis of clinico-physiological comparisons. Coma is characterized by failure of attempts to wake the patient, deep coma by muscular atonia, and protective inhibition coma by bilateral mydriasis. Opening of the eyes in response to a sound or pain is characteristic for the apallic syndrome, carrying out of instructions for deep stunning, answering questions for stunning. Clear consciousness is characterized by the patient's ability to orientate. Protective inhibition coma is always characterized by coars

Nuclear magnetic resonance imaging (NMR), (MRI), of brain stem tumours.

The NMR scans of twenty-six patients who were referred with the clinically suspected or histologically verified diagnoses of brain stem tumour were reviewed. Twenty-one patients (eleven children and ten adults) had scans that were positive for tumour. Nine of these had histological confirmation. Eleven were irradiated without histology on the basis of the clinical diagnosis. The remaining patient was treated expectantly. The other five patients (two children and three adults) had CT and NMR scans that were negative for tumours. Clinical follow-up of these five cases for 9 to 29 months disclose

[Dynamic study of brain stem evoked potentials in cerebral aneurysms].

Eleven patients with arterial and arteriovenous aneurysms of the cerebral vessels were subjected to 30 repeated examinations of truncal evoked potentials (TEP) to acoustic stimulation. The examination was conducted in the acute stage of subarachnoid hemorrhages, before and after surgical intervention. It is shown that changes in TEP may be recorded along with improvement, deterioration, stable disorders. From the degree and character of TEP disorders, not only the trend of changes in the functional state of the brain trunk can be judged but also the processes underlying these disorders (ischem

Brain-stem auditory-evoked potentials during lidocaine infusion in humans.

Auditory brain-stem responses (ABR) were recorded in six healthy male volunteers during intravenous infusion of lidocaine that achieved systemic blood levels similar to those seen with conduction anesthesia and antiarrhythmic therapy. Following an initial loading dose of lidocaine (1 mg/kg), subjects noted prominent tinnitus, perioral numbness, and drowsiness. All of these symptoms except drowsiness abated during continued infusion as blood concentrations reached equilibrium. All subjects noted that the click stimuli used to elicit ABR varied markedly in intensity and character throughout the

[Brain stem infarction, systemic lupus erythematosus and circulating lupus anticoagulant].

A 30 year old man admitted with a brain stem infarct presented with intellectual deterioration. A diagnosis of systemic lupus erythematosus (S.L.E.) was based on the presence of five A.R.A. criteria :photosensitivity, arthralgia, leukopenia and thrombopenia, false positive syphilitic serology, antinuclear factors on immunofluorescence. A lupus type circulating anticoagulant (L.C.A.) was no longer detected after corticotherapy. This appears to be a case of the "hematologic" form of S.L.E. in which the presence of L.C.A. predisposes towards thrombosis, not only of veins--which is typical--but al

Auditory brain stem response abnormalities in patients with Bell's palsy.

To evaluate the hypothesis of CNS involvement in Bell's palsy, the auditory brain stem responses (ABR) of 31 patients were studied. In nine of these patients ABR abnormalities were found. None of these patients showed evidence of dysfunction of the cochlear nerve. Six of the patients who had abnormal ABR were retested after they recovered from the facial paresis. Five of these patients showed persistent ABR abnormality, and one showed a normalization of the ABR. These results may be consistent with an injury at the brain stem level in some patients with Bell's palsy. The possible causative age

3H-Codeine binding in the guinea pig lower brain stem.

Saturable binding of (-)-3H codeine was found in the guinea pig medulla (KD = 5.6 x 10(-7) M, Bmax = 1.4 pmol/mg protein), whereas little stereospecific binding was detected (KD = 4.4 x 10(-5) M). The saturable binding of (-)-3H codeine was slightly enhanced by Na+ and by Mg++ but not by Li++ and Ca++. The enhancement appears to be due to an increase in the number of receptor sites. (-)-3H-Codeine binding was displaced by (-)- and (+)-codeine, morphine, (-)- and (+)-methadone but not by barbiturates. Naloxone, at a high concentration (1 x 10(-5) M), inhibits the binding by only 40%. This agree

Clinical and pathologic correlates of brain stem auditory response abnormalities.

Short-latency auditory evoked responses were recorded in over 100 neurologic patients. Abnormalities of each response component were correlated with postmortem or radiologic localization of different brain stem lesions. These findings suggested that waves I-VII largely reflect activity at the following levels of the auditory pathway: acoustic nerve (I), pontomedullary junction (II), caudal pons (III), rostral pons or midbrain (IV), midbrain (V), thalamus (VI), and thalamus or auditory radiation (VII). When this information was applied prospectively to the evaluation of brain stem dysfunction,

Respiratory rate and pattern disturbances in acute brain stem infarction.

Respiratory rates and patterns were studied in 23 patients with acute brain stem infarction using impedance pneumography. Autopsy was obtained in six of eight fatal cases. Pontine lesions were present in all patients, with coexistent infarction of midbrain in four and of medulla in nine. Respiratory rate and pattern abnormalities observed included Cheyne-Stokes respiration, Cheynb-Stokes variant pattern and tachypnea. Abnormalities of respiratory rate and pattern of varying duration were observed at some time in all patients. All patients in whom prominent Cheyne-Stokes respiration or tachypne

[Clinicophysiological studies of orbicularis oculi reflex in brain stem diseases].

Orbicularis oculi reflex is consisted of two components; one is early reflex response (R1) which is 14.1 +/- 2.2 msec in latency, and the other is late reflex response (R2) that is 35.0 +/- 8.6 msec in latency (mean +/- 2SD). R1 is observed on the ipsilateral side of stimulation and R2 is visible bilaterally on ipsiand contralateral sides of stimulation. Clinically orbicularis oculi reflex can be recorded easily, however, its clinical and physiological significance has not been clearly solved. Orbicularis oculi reflex was tested on patients with the brain stem lesions of various kinds, and the
