Focal myositis of the perioral musculature.

Focal myositis, a benign inflammatory pseudotumor, is a relatively newly defined clinicopathologic entity which arises as a rapidly enlarging swelling within an isolated skeletal muscle. It occurs most often in the lower extremity, and we are reporting what we believe to be the first cases of perioral involvement. There is no apparent age or sex preference. Focal myositis must be differentiated from neoplasm, nodular pseudosarcomatous fascilitis, proliferative myositis, myositis ossificans, polymyositis, and, in the oral region, salivary gland lesions and hypertrophic branchial myopathy. No le

Papillary thyroid microcarcinoma presenting as a large cystic lymph node: A case report.

Papillary thyroid carcinoma (PTC) may deviate from the expected clinical course, posing diagnostic challenges to clinicians. The present study presents a case of longstanding swelling on the left side of the neck, initially diagnosed as a cystic lesion, which was later revealed to be metastatic thyroid cancer. A 21-year-old woman presented with a 3-year history of a neck mass that had initially been diagnosed as a likely hemangioma or branchial cleft cyst in 2019. However, subsequent scans in 2022 revealed a suspicious cyst and a thyroid nodule, which were confirmed to be PTC with metastasis t

A Rare Presentation of a Branchial Cleft Cyst: Can It Cause Syncope in a Pediatric Patient?

Branchial cleft cysts (BCCs) are a congenital malformation most commonly seen in children and adolescents. BCCs are usually incidental findings and are benign in nature. In this report, we present a case of a 13-year-old female with syncope as a rare complication of a fluid-filled second BCC. The patient initially presented with a unilateral non-tender swelling on the right side of the neck and submandibular region, which was suspicious of an inflammatory process. After initial lab investigations came back negative, imaging studies of the neck with computed tomography (CT) with intravenous con

Malformations of the first branchial cleft: duplication of the external auditory canal.

OBJECTIVE: Malformations of the first branchial cleft are uncommon and only sporadic reported in the literature. The may be present as a swelling or inflammatory opening on the neck, blind cysts or fistula associated with the external auditory canal. In this retrospective study, clinical features, diagnostic and therapeutic pitfalls are described in nine pediatric cases. PATIENTS AND RESULTS: Between 1998 and 2002, duplication of the external auditory canal were diagnosed in nine patients aged from 7 months to 14 years. Three infants had cysts in the external ear canal; one had a sinus on the

Concurrent Occurrence of Ear Tag With Posterior Talon Cusp, Fissured Tongue, and Ankyloglossia: A Case Report.

Ear tags or accessory auricles are branchial cleft remnants that clinically appear as asymptomatic nodules or papules in the preauricular region. They occur in various syndromes affecting the first and branchial arches during embryogenesis. The presence of an ear tag can have a psychological impact on one's life due to its unesthetic appearance, thereby affecting their quality of life. Talon cusp usually occurs in the maxillary central or lateral incisor. A fissured tongue or cerebriform tongue is characterized by the presence of horizontal or vertical grooves, usually affecting the dorsum of

A Type-II First Branchial Cleft Anomaly Presenting as a Post-Auricular Salivary Fistula: A Rare Presentation.

First branchial cleft anomalies are rare with the average age of presentation as 19 years. There is an average delay of 3.5 years between initial presentation and adequate treatment due to diagnostic dilemma. A very rare variant of first branchial cleft anomaly presenting as a post-auricular salivary fistula is reported. A 12-year-old girl presented with a history of intermittent watery discharge, more so at the time of meals from a right post-auricular opening for last 3 years. Computed tomography sialography revealed a fistulous tract connecting the sub segmental duct of the parotid gland ex

Pathways of avian neural crest cell migration in the developing gut.

The NC-1 and E/C8 monoclonal antibodies recognize a similar population of neural crest cells as they migrate from vagal levels of the neural tube and colonize the branchial arch region of 2- to 3-day-old chicken embryos. Some of these immunoreactive cells then appear to enter the gut mesenchyme on the third day of incubation just caudal to the third branchial cleft. After entering the gut, these cells migrate in a rostral-caudal direction, using primarily the superficial splanchnic mesodermal epithelium of the gut as a substratum. The antigen-positive cells remain preferentially associated wit

Intra-Operative Detection of a Left-Sided Non-Recurrent Laryngeal Nerve during Vagus Nerve Stimulator Implantation.

Left sided non-recurrent laryngeal nerves (NRLN) are very rarely observed during surgery in the head and neck region. Arising directly from the cervical aspect of the vagus nerve, the NRLN lies in a vulnerable position distant from its normal location. NRLNs are normally associated with embryological branchial arch aberrations and subsequent vascular anomalies. The anomalous course of the NRLN makes it more susceptible to injury during surgery in the neck region. Knowledge of this anatomical variant will reduce the potential for injury and resultant vocal cord paralysis. During microsurgical d

Histamine Stimulates Ciliary Beat Frequency via the H2 Receptor in the Protochordate Botryllus schlosseri.

Histamine is a biogenic molecule that plays a role in many physiological pathways via binding to a specific receptor. Histaminergic receptors belong to the large family of seven-transmembrane α-helix domain receptors classified in mammals into four distinct classes: H1, H2, H3, and H4. Despite being widely studied in vertebrates, few data are available on the invertebrate receptors, with only predicted H1 and H2 sequences for nonchordate deuterostomes. Here, we report the first characterized transcript sequence for an H2 receptor from the colonial ascidian Botryllus schlosseri, describing the

Branchio-oculo-facial syndrome in a newborn caused by a novel TFAP2A mutation.

We present an 18-day old boy with bilateral cervical cutaneous defect in the retroauricular region, low-set and posteriorly rotated ears, bilateral microphtalmia and bilateral pseudocleft of the upper lip. Histopathological evaluation of cervical cutaneous defect showed ulceration on the surface and ectopic thymus tissue in the deep dermis with cortex, medulla and Hassal's corpuscles. Clinical findings led to the diagnosis of Branchio-oculo-facial syndrome, characterized by branchial defects (erythematous cutaneous defects in cervical region), ocular anomalies (microphthalmia, anophthalmia, la

Thyroglossal Duct Cyst Associated with Xanthogranulomatous Inflammation.

Xanthogranulomatous inflammation (XGI) is an uncommon destructive chronic inflammatory process mainly occurring in the kidney and gallbladder, characterized by the accumulation of foamy histiocytes, multinucleated giant cells (Touton type), cholesterol clefts and chronic inflammatory cells. The head and neck region is an uncommon site for XGI. This type of inflammatory reaction has been defined in branchial cleft cyst, salivary gland tumors following fine-needle aspiration biopsies, Rathke's cleft cyst in the pituitary gland, and colloid cyst in the 3rd ventricle. We present herein a unique ca

The facial nerve coursing across the oval window area.

A case of unilateral conductive deafness since birth is described. At exploration of the middle ear, the facial nerve was found to have an anomalous course across the oval window region, the stapedial crura ending on the uncovered facial nerve. This case is believed to represent the 9th case to be recorded. This anomaly may be due to failure of union between the branchial part and otic capsule part of the stapes, in combination with a defective development of the laterohyale resulting in a partially absent facial canal.

Pyogenic thyroiditis due to branchial pouch sinus.

Suppurative thyroiditis due to branchial pouch sinus is a rare disease, affecting children and young adults. It presents as an abscess in the lower lateral neck, usually on the left side. Suppurative thyroiditis can be suggested by ultrasound, radionuclide scintigraphy or CT scan, but the diagnosis can only be established by barium swallow showing the sinus from the apex of the pyriform sinus to the perithyroid region. Complete surgical excision of the sinus is essential. Two patients are presented and the role of the various imaging modalities in the diagnosis is discussed.

The regional pattern of retinoic acid synthesis by RALDH2 is essential for the development of posterior pharyngeal arches and the enteric nervous system.

Targeted inactivation of the mouse retinaldehyde dehydrogenase 2 (RALDH2/ALDH1a2), the enzyme responsible for early embryonic retinoic acid synthesis, is embryonic lethal because of defects in early heart morphogenesis. Transient maternal RA supplementation from E7.5 to (at least) E8.5 rescues most of these defects, but the supplemented Raldh2(-/-) mutants die prenatally, from a lack of septation of the heart outflow tract (Niederreither, K., Vermot, J., Messaddeq, N., Schuhbaur, B., Chambon, P. and Dollé, P. (2001). Development 128, 1019-1031). We have investigated the developmental basis for

Tissue effects on the expression of serotonin, tyrosine hydroxylase and GABA in cultures of neurogenic cells from the neuraxis and branchial arches.

The phenotypically diverse neurones of the enteric nervous system are developmentally derived from precursors that migrate to the bowel from the vagal and sacral regions of the neuraxis. In order to gain insight into the generation of enteric neuronal diversity, we examined the expression of serotonin (5-HT), tyrosine hydroxylase and GABA in vitro. In the mature avian intestine, intrinsic neurones contain 5-HT or GABA but not tyrosine hydroxylase. These markers were demonstrated immunocytochemically, singly or simultaneously. All three phenotypic markers developed in cultures of cranial, vagal

Management of a First Branchial Cleft Fistula in an Adult Patient: A Case Report and Literature Review.

First branchial cleft anomalies (FBCAs) are uncommon congenital malformations that develop due to incomplete closure of the first branchial cleft during embryonic development. They represent a small proportion of all branchial cleft anomalies. These lesions, which may present as cysts, sinuses, or fistulas, pose significant diagnostic and therapeutic challenges due to their nonspecific clinical manifestations and complex anatomical relationships, particularly with the facial nerve and parotid gland. This case report describes a 47-year-old male patient presenting with recurrent infections and

Thymus-forming potential of the second pharyngeal pouch and its regulation by local mesenchyme in avian embryos.

The thymus derives from the endoderm of pharyngeal pouches (PPs). The number and location of PPs with thymus-forming potential differ among jawed vertebrates, and ectopic thymus locations in mice and humans suggest a broader thymus-forming potential in PP endoderm than previously ascribed. We used the quail-chick chimera system to test if non-canonical pouches could form a thymus and examined the role of pharyngeal arch (PA) mesenchyme in this process. After testing several tissue associations, we identified thymus-forming potential in both non-canonical second PP and canonical third/fourth PP

First and second branchial arch involvement in mandibulofacial dysostosis Guion-Almeida type.

Mandibulofacial dysostosis Guion-Almeida Type (MFDGA; OMIM#610536) is a rare autosomal dominant genetic disorder caused by heterozygous pathogenic variants in the EFTUD2 gene. Mandibulofacial dysostoses are characterised by the core triad malar hypoplasia, maxillary hypoplasia and dysplastic ears, all derived by the impaired development of the first and second branchial arches. Differential diagnosis is often challenging. The early genetic diagnosis is extremely useful, not only for the correct management of cranial malformations, but also for the early diagnosis and treatment of the comorbidi
