[Physical breast examination].

The aim of the project was the comparison of the effectiveness of physical breast examination in relation to mammography on the basis of research analysis conducted within the program "The Early Diagnosis of Breast Cancer". The program was conducted at J. Sniadecki Hospital in Nowy Sacz in the year 2002. The physical examination was carried out by professional midwives. The documentation of 400 women who participated in the program was analyzed in order to gain the necessary information. Half of the participants have identified anomalies in breasts (202; 50.5%). There was no difference in the …

[Male breast cancer].

As male breast cancer remains rare entity (less than 1% of cases of breast cancer), most of our current knowledge of it has been extrapolated from its female counterpart. The prevalence of male breast cancer increases with age, and the presentation occurs at an average age of approximately 60 years, 10 years older than in females with the disease. The majority of patients present with a painless, firm, subareolar mass, and the tumors are usually larger than 2 cm in diameter. There may be fixation to skin. Mammography and ultrasonography are useful to distinguish between breast cancer and gynec…

Benign breast disease.

This article describes 25 years of clinical experience in the setting of a Breast Health Center devoted to benign and malignant disease of the breast. During this period, more than 100,000 patients have been evaluated and treated for a wide variety of breast problems. This experience has provided an extraordinary opportunity to evaluate the natural history of benign breast disease and the frequent observation that the relevant medical literature does not reflect the entire spectrum of these conditions or the appropriate treatment. Most textbooks on breast disease emphasize breast cancer and th…

Breast cancer screening.

Mammographic screening reduces breast cancer mortality by 24% in women aged > or = 50 years. Women aged 50-53 are invited for their first screen in the National Health Service Breast Screening Programme (NHSBSP), thereafter at three-year intervals until the age of 64. Two-view mammography is offered at the first (prevalent) screening examination and one-view for subsequent (incident) screens. The screening films are sorted into either 'normal' or 'abnormal requiring further assessment'. Assessment is carried out by a specialist multidisciplinary team using the triple approach: clinical examina…

Benign breast disease.

Benign breast disorders (BBD), classified by the ANDI system (aberrations of normal development and involution), constitute the major workload in breast clinics. Breast pain (mastalgia) is classified as cyclical, and non-cyclical extramammary causes such as ribircage pain have to be identified. Most patients need reassurance alone but those with moderate/severe pain present for > 6 months may need treatment: randomised trials have shown danazol, bromocriptine and tamoxifen to be effective. Fibroadenoma is the commonest benign solid lump in women aged 15-30 years. The diagnosis must be confirme…

Virginal breast hypertrophy.

Macromastia is the massive enlargement of the breast, unilateral or bilateral, disproportional to growth in the remainder of the body. Most patients seen with macromastia have breasts that develop normally at puberty but simply reach excessive size. Virginal breast hypertrophy is a rare and distinct disorder with the rapid onset of macromastia at the onset of puberty. Although the disorder has been reported in the surgical, pediatric, and gynecologic literature, virginal breast hypertrophy has not been reported in dermatologic journals. We report two cases of virginal breast hypertrophy. Patie…

Breast cancer screening.

Breast cancer is the most common malignancy among women in the United States; however, recent data demonstrates a decline in the mortality rate, which may be attributed to early detection from screening programs combined with effective therapies for early stage disease. As a result of the prevalence of breast cancer and its association with highly emotional issues, screening recommendations have aroused debate in the scientific, public, and legislative domains. A general consensus supports breast cancer screening among women between the ages of 50 and 70; however, much controversy exists regar…

Hereditary breast cancer.

Information on genetics is in process of exponential growth, with a corresponding recognition that most medical information, screening and diagnostic tests, and disease processes have a genetic component. Hereditary breast cancer is a good example, showing the complexity of cancer genetics in a clinical context. The contribution of family history to the risk of developing breast cancer has been appreciated for some time. Breast cancer was believed to be a result of multiple genetic and environmental factors. Today it is known that single genes are also responsible for breast cancer and some of…

Interventional breast procedures.

The capability to provide histologic diagnoses of nonpalpable lesions by performance of percutaneous needle biopsy has revolutionized breast imaging in the past decade. The radiologist who performs percutaneous breast biopsies assumes an increased level of responsibility for the patient regarding patient selection, lesion selection, performance of the biopsy procedure, interpretation of results, and patient follow-up. With variable and increasingly numerous options for the biopsy of breast lesions, careful attention must be paid to the selection of patients and types of lesions for different p…

[Hereditary breast cancers].

Recently, genetic analyses in high risk families with several members suffering of breast and/or ovarian carcinoma led to the discovery of two genes, called BRCA1 and BRCA2, clearly responsible for hereditary predisposition of breast carcinoma. Another gene, p53, was also shown to be involved in hereditary predisposition of breast and other tumors in the setting of Li-Fraumeni syndrome. It is very important that women at risk could be seen by a specialized team for genetic counselling and explanation of advances and limits of molecular genetics. Such a team should be multidisciplinary in order…

Unilateral breast atrophy.

This represents the only known case of acquired unilateral breast atrophy described after previously normal bilateral breast development. Our patient developed left breast atrophy with normal microscopic examination (biopsy) subsequent to infectious mononucleosis. Her reconstruction utilized tissue expansion followed by prosthetic augmentation of the affected side. Despite an exhaustive search, no definitive cause to explain these findings has been found. Perhaps there is some unknown underlying viral etiology that may have further implications for other breast pathology.

Nonpalpable breast cancer.

Thirty-seven patients with nonpalpable breast cancer treated between 1981 and 1992 were analysed. Twenty-five (68%) of the patients were detected by microcalcification of mammogram, 8 by nipple discharge and 4 by other means. Biopsy was carried out in all patients to get a final diagnosis and histopathological examinations were performed using continuous sections (especially the section with microcalcification). The incidence of noninvasive carcinoma was 59% (22 cases) and the remaining invasive carcinomas were 3 minimal lesions less than 0.5 cm, 3 lesions from 0.5 to 1.0 cm, 3 diseases more t…

Noninvasive breast cancer.

Noninvasive breast cancer comprises two distinct entities, lobular carcinoma in situ (LCIS) and ductal carcinoma in situ (DCIS). The increased use of screening mammography has caused more cases of noninvasive breast cancer to be discovered. However, the increased detection of LCIS and DCIS has posed problems in patient care for the radiologist, pathologist, surgeon, radiation oncologist, and medical oncologist. The authors review the past history of LCIS and DCIS, as well as the diagnostic challenges and therapeutic considerations for the multidisciplinary breast cancer team.
