Congenital dislocation of the hip associated with congenital muscular torticollis.

Four recently identified cases of congenital dislocation of the hip associated with congenital muscular torticollis out of 49 consecutive cases of frank dislocation of the hip suggest that: the coexistence of congenital muscular torticollis and congenital dislocation of the hip occurs more commonly than is generally appreciated; the presence of either abnormality should initiate a careful appraisal for the presence of the other; although both conditions are seen more commonly in children delivered by breech extraction, a direct causal relationship has not yet been established although the abno

[Effect of mode of delivery on early morbidity and mortality of premature infants (< or = 34th week of pregnancy)].

In a retrospective analysis (1985-1994) the influence of the mode of delivery on mortality and early morbidity of low birth weight infants (< or = 2500 gm and < or = 34 weeks of gestation) was assessed (n = 450). We only included cases that offered a choice between abdominal and vaginal delivery (cephalic and breech presentation, premature rupture of membranes and preterm labour). We found significantly higher survival rates in infants with birth weights of 1500 gm or less in case of breech and cephalic presentation after cesarean births. In infants with cephalic presentation and birth weights…

Antepartum diagnosis and intrapartum management of lethal renal defects.

Bilateral renal agenesis (Potter syndrome) in the neonate is incompatible with life. Unfortunately, these infants are often delivered by cesarean section, as premature labor and breech presentation are commonly associated with the condition. In reviewing the obstetric course of 6 women who recently delivered such babies, the authors have found that antepartum diagnosis is possible in most cases. specifically, vaginal delivery should be strongly considered for patients in premature labor with intact membranes and breech presentation when there is ultrasound evidence of an abnormally shaped feta…

Implementation of the external cephalic version in breech delivery. Dutch national implementation study of external cephalic version.

BACKGROUND: Breech presentation occurs in 3 to 4% of all term pregnancies. External cephalic version (ECV) is proven effective to prevent vaginal breech deliveries and therefore it is recommended by clinical guidelines of the Royal Dutch Organisation for Midwives (KNOV) and the Dutch Society for Obstetrics and Gynaecology (NVOG). Implementation of ECV does not exceed 50 to 60% and probably less.We aim to improve the implementation of ECV to decrease maternal and neonatal morbidity and mortality due to breech presentations. This will be done by defining barriers and facilitators of implementati…

Modes of delivery of first and second twins as a function of their presentation. Study of 614 consecutive patients from 1992 to 2000.

OBJECTIVES: To study modes of delivery and neonatal morbidity of twins as a function of their presentation. STUDY DESIGN: The study related to 614 consecutive patients who gave birth to twins in the maternity ward of the Hôpital Robert Debré from 1992 to 2000. Group A (n=529) included patients who gave birth after 33 weeks of gestation and group B (n=85) before 33 weeks. The parameters studied were the mode of delivery, the need for intubation at birth, 5-min Apgar score <9, transfer to intensive care, death in the neonatal period. The chi-square test was used for statistical analysis. RESUL

Management of breech and twin labor during registrarship: A two-year prospective, observational study.

INTRODUCTION: Breech presentation and twin pregnancy are regarded as stressful situations for medical staff. This stress is often associated with an increased likelihood of intervention during labor - especially when the on-shift obstetrician lacks experience. MATERIAL AND METHODS: We performed a 2-year prospective, observational study of cesarean section (CSDs) and assisted vaginal (AVDs) deliveries in a tertiary maternity unit for attempted vaginal deliveries of breech presentations and twin pregnancies. The obstetric management decisions taken by a group of four registrars were compared wi…

The temporomandibular joint in children after breech delivery.

In an effort to explain the aetiology of early temporomandibular joint (TMJ) injuries, their incidence in children born by vaginal breech delivery and possible correlation with such deliveries were investigated. The study covered 156 children aged from 4 to 6 years, eighty-three of them born by vaginal breech delivery under application of the classical Mauriceau manoeuvre in seventeen cases and the Bracht manoeuvre in sixty-six. Seventy-three children delivered spontaneously in vertex presentation served as the control group. Signs of TMJ injury were found in 59.6% of the total group, the by-b

Breech delivery and mental handicap.

Recent studies with strict management protocols for selection of cases for trial of vaginal delivery for full-term breech presentations show no significant differences in outcome brought about by the birth route. This is in clear contrast to some of the older studies which indicated considerable risk for the vaginally born breech infant. Prospective follow-up studies and carefully matched controlled studies with sophisticated neurological evaluations indicate that breech infants, regardless of mode of delivery, will score slightly less favourably than infants born in vertex presentation. This …

The Use of 360° VR Video, Educational Videos, and High-Fidelity Physical Models in Teaching Breech Birth - A Pilot Feasibility Study.

BACKGROUND: Although there is ongoing debate, the current consensus is that vaginal breech birth carries a marginal increase in perinatal morbidity and mortality. Due to these risks there have been decreasing numbers of vaginal breech births and subsequently clinical exposure to hands-on training has declined. However, to confidently care for women who plan a vaginal breech birth or those presenting in advanced labour with an unexpected breech presentation, education in vaginal breech birth remains necessary. AIMS: This pilot study aimed to assess the ability of a multimodal teaching program …

Fetal femur fracture and external cephalic version.

BACKGROUND: Femoral fractures due to birth trauma are extremely rare. External cephalic version is considered a relatively safe alternative when dealing with breech presentation, but it can be associated with complications. CASE: This patient underwent elective cephalic version for breech presentation at 36 weeks of gestation. Due to concerns of fetal distress after the version, a cesarean delivery was performed. The newborn had a bruised leg at delivery, and X-ray studies confirmed a distal femoral corner fracture. CONCLUSION: This case demonstrates another risk of external cephalic version…

How singleton breech babies at term are born in France: a survey of data from the AUDIPOG network.

Based on data from the AUDIPOG sentinel network between 1994 and 2010, we can say that the rate of singleton breech presentation at term is 3% and remains unchanged despite an external cephalic version rate of 35%. The total cesarean section rate is currently 75%. This rate increased by nearly 20% after the Hannah publication in 2000, regardless of the type of breech and type of maternity unit. The rate of planned cesarean sections increased in particular, going from 40% to 60%, and even reaching 67% for footling breech presentations. The rate is higher in type I maternity units than in type I…

Breech delivery in the all fours position: a prospective observational comparative study with classic assistance.

OBJECTIVE: Vaginal delivery of fetal breech presentation is considered to be a challenge for obstetricians. The purpose of this study was to show that vaginal delivery in all fours position is feasible and safe for mother and child compared with vaginal breech and classic support. METHODS: A single-center prospective observational case series of breech delivery (n=41) in all fours position was compared to a retrospective cohort of breech deliveries in the form of a matched-pair analysis. RESULTS: Deliveries in the all fours position successfully took place without obstetric intervention in 7…
