Recreational Nitrous Oxide Misuse: Anaesthetic Challenges and Perioperative Complications Including Airway Burns and Neurological Sequelae.

Recreational use of nitrous oxide (N2O) ("hippy crack") has increased in recent years, with serious complications being reported, including airway cold injuries, neurological disease, and thrombotic events. This review highlights the perioperative implications of N2O misuse, with a focus on airway injury, neurological sequelae, and anaesthetic management. Cold burns from direct inhalation can compromise airway safety, while chronic use leads to functional vitamin B12 inactivation, resulting in subacute combined degeneration and peripheral neuropathy. In such patients, intraoperative use of N2O

Cutaneous mucormycosis in advanced HIV disease.

Angionvasive mucormycosis is an emerging fungal disease known to affect mainly diabetics or subjects with profound neutropenia. Infection usually occurs through the inhalation route, but cutaneous inoculation may occur after trauma or burns. However, mucormycosis remains unusual in HIV infection. We report a fatal case of cutaneous mucormycosis due to Rhizopus arrhizus involving the scalp following herpes zoster infection. The patient was a 42-year-old man with advanced AIDS failing on salvage antiretroviral therapy. The fungus was diagnosed on the basis of histopathology and culture. Our case

Massive Burn Injuries: Characteristics and Outcomes From a Single Institution.

Massive burn injuries are a unique patient population with unique treatment paradigms. Data from 155 adult patients, admitted from 2009 to 2019, with >50% total body surface area burns (TBSA) were collected and analyzed. Average burn size was 70% TBSA and 63% had a concomitant inhalation injury. Approximately 30% of patients (46/155) transitioned to comfort care-only measures within 24 hours of admission. Standard treatment patients were younger (37 ± 13 vs 60 ± 19 years; p < .00001), male (94% vs 28%; p = .001) and had smaller TBSA (66 ± 13 vs 80 ± 16; p < .00001). Of the standard treatment g

[Resuscitation of the burned child in critical condition].

Recent advances in surgical techniques, control of infection and nutritional support have dramatically increased the survival rates of burned children. The characteristics of severely burned pediatric patients dictate that management be different from that required for adults in the intensive care unit. The formulas for fluid replacement should be based on body surface rather than weight in children and adjusted for degree of stress and age, with appropriate monitoring and treatment of hypothermia, pain and associated psychological disorders. Early assessment and treatment of airway obstructio

Toxic smoke compounds and inhalation injury--a review.

Although the role of inhalation injury in contributing to the mortality of burn patients has received increasing attention, the available information on the toxic and injurious effects of smoke generated from uncontrolled fires is scarce. This is because each fire generates its own variety of smoke, depending on the nature of the combusting materials and the burning conditions. This review summarizes the actions of the primary smoke components, heat, particulate materials, asphyxiants (carbon monoxide and hydrogen cyanide), and pulmonary irritants, without covering the whole field of smoke tox

The radiographic spectrum of pulmonary complications in burn victims.

The spectrum of pulmonary complications in 28 of 66 burn victims is analyzed according to time of onset and resultant radiographic features. Immediately recognizable pulmonary abnormalities are usually due to chemical pulmonary edema and inhalation pneumonitis. Complications manifested 2-5 days after injury include pulmonary microembolism, adult respiratory distress syndrome, and atelectasis. Delayed complications are major pulmonary embolism, pneumonia, and adult respiratory distress syndrome. In this series, 25 patients (89%) developing pulmonary complications died. The critical role of seri

Successful utilization of ECMO to treat an adult burn patient with ARDS.

We present an interesting case of the first adult reported in the United States to suffer from thermal burns, adult respiratory distress syndrome (ARDS) and to be treated with extracorporeal membrane oxygenation (ECMO) who survived. Our patient is a 26 year old male who sustained thermal burns (12% TBSA) to his face and anterior trunk and broncoscopically demonstrable inhalation injury. He was transported to our regional burn center for burn wound care and ventilatory support. The patient was treated with silver sulfadiazine 1% to his wounds which healed per primam. Because of low oxygen satur

The Presence and Presentation of Microstomia in Burn Survivors: A Burn Model System Study.

Microstomia, or small oral aperture, often results from facial burns. This condition appears as scars form within the oral tissue and at the oral commissures, narrowing the oral opening, and leading to mobility, speech, respiratory difficulties, emotional distress, and social interaction challenges. This study examines the presence of reported burn microstomia and its related clinical factors. Pediatric and adult data from a multicenter longitudinal database from 2003 to 2005 were analyzed to determine the frequency of microstomia at discharge. Summary statistics were used to describe clinical

MicroRNA-engineered extracellular vesicles for burn wound repair: burn-specific design and translational perspectives.

Burn wound healing remains a major clinical challenge. Mesenchymal stem cell-derived extracellular vesicles engineered to deliver defined microRNA cargo or modulate endogenous microRNA profiles have emerged as promising cell-free therapeutics, but native extracellular vesicle preparations remain constrained by low yield, heterogeneity, insufficient targeting, and incomplete cargo control. Moreover, much of the current evidence still comes from non-burn wound models. This review presents a burn-oriented framework that aligns upstream parental-cell programming, downstream vesicle modification, a

Multi-Organ Failure in Adult Patients Admitted to Intensive Care with Severe Burns-A Retrospective Cohort Study.

Introduction: Multi-organ failure (MOF) is the predominant cause of death in patients with burns who survive their initial injury. MOF can be "sterile" in nature (driven by the burn injury) or related to sepsis. The time course and predictors of MOF in major burns have not been well characterised. Methods: This is a retrospective cohort study of all adult patients with total body surface area (TBSA) burns > 15% admitted to the ICU of a UK tertiary burn centre between 2016 and 2025. Sequential Organ Failure Assessment (SOFA), DENVER2, and American Burn Association Sepsis Criteria were calculate

The impact of patient demographics and comorbidities upon burns admitted to Tygerberg Hospital Burns Unit, Western Cape, South Africa.

In South Africa, burns are a major public health problem responsible for significant morbidity and long-term physical disability. This is, in part, due to a significant proportion of the urban population living in poorly constructed, combustible accommodation. The presence of co-morbid diseases such as diabetes and malignancy in patients with burns has been associated with a poorer outcome. The impact of other diseases such as HIV has yet to be defined. A retrospective data collection study analysed the 221 patients admitted to Tygerberg Hospital Burns Unit in 2011 and the first six months of

Dose estimates for exposure to radioactivity in gas mantles.

In this paper dose estimates are given for internal and external exposure that result, due to radioactive thorium, from the use of the incandescent mantles for gas lanterns. The collective, effective dose equivalent for all users of gas mantles is estimated to be about 100 Sv per annum in the Netherlands. For the population involved (ca. 700,000 persons) this is roughly equivalent to 5% to 10% of the collective dose equivalent associated with exposure to radiation from natural sources. The major contribution to dose estimates comes from inhalation of radium during burning of the mantles. A pes

Infection and Burn Injury.

Burn injury is debilitating and among one of the most frequently occurring traumas. Critical care improvements have allowed for increasingly positive outcomes. However, infection, whether it be localized to the site of the wound or systemic in nature, remains a serious cause of morbidity and mortality. Immune suppression predisposes the burn population to the development of invasive infections; and this along with the possibility of inhalation injury puts them at a significant risk for mortality. Emerging multi-drug-resistant pathogens, including Staphylococcus aureus, Enterococcus, Pseudomona

Case Report: Acute pancreatitis obscured by paralytic ileus in a patient with extensive burns.

Extensive burns are associated with numerous severe complications. While paralytic ileus is relatively common, the co-occurrence of acute pancreatitis (AP) is rare, clinically insidious, and highly fatal. Diagnosis is particularly challenging due to masking by critical illness and coexisting ileus. We present a 32-year-old male with 65% TBSA flame burns and inhalation injury, admitted in hypovolemic shock. Resuscitation followed Parkland protocol, and escharotomy was performed. On day 2, abdominal distension and reduced bowel sounds suggested paralytic ileus, managed with nasogastric decompres

Butane detection after long-term treatment of burns in two autopsy cases.

A man and a woman were rescued from a room that had exploded. Many empty cassette gas cylinders were found in the room. The man and woman were hospitalized immediately for the treatment of burns. The woman died 6 days later, and the man died 31 days later without regaining consciousness. Carbonization and hardening of the frontal facial skin and parts of the left and right fingers were observed on the man's body. In both cases, systemic burns had led to progressive systemic edema and markedly suppressed circulation. Analytical samples for butanes obtained from their bodies at autopsy were stor

Clinical outcome of HIV positive patients with moderate to severe burns.

Human immunodeficiency virus (HIV) infection is a world wide and growing problem. Little is found in the literature concerning the treatment and outcome of patients suffering from HIV infection who are treated for burns. The aim of this study was to assess whether the outcome of HIV positive patients suffering from burn wounds differed from those who do not have HIV infection. Thirty three patients formed the HIV positive study group. HIV negative controls were matched for age, degree of burns, sex and inhalation injury. The mean age of the patients was 31.6 years and the mean total body surfa

Predictors of Mortality in Burn Patients at Selected Tertiary Public Hospitals in Addis Ababa, Ethiopia: A Two-Year Retrospective Study.

Background: Burn injuries are a major cause of morbidity and mortality in low- and middle-income countries, yet remain underreported due to limited data systems. This study describes the epidemiology of burn patients admitted to two major burn centers in Addis Ababa, Ethiopia, and identifies predictors of in-hospital mortality. Methods: A cross-sectional study was conducted among patients with new burn injuries admitted between 1 September 2021, and 1 November 2023, at the Addis Ababa Burn, Emergency, and Trauma Center (AaBET) and Yekatit 12 Medical College (Y12MC) hospitals. Data were extract

[Antibiotic treatment of infections in burn patients - a systematic review].

BACKGROUND: Due to the loss of the natural skin barrier function with reduced immune competence as a result of a plasma loss and the numerous intensive care interventions, burn patients are particularly at risk for infection. STUDY DESIGN: systematic review METHODS: A systematic review of German and English literature between 1990 and 2018 analyzes the epidemiological and diagnostic aspects as well as the therapeutic use of antibiotics in infections of burn patients in clinical trials. RESULTS: A total number of 53 randomized controlled clinical trials met the inclusion criteria. Various typ

Description of Streptococcus pneumoniae infections in burn patients.

BACKGROUND: Longer survival in burn patients has resulted in more infectious complications, typically with Klebsiella pneumoniae, Pseudomonas aeruginosa, Acinetobacter baumannii, and Staphylococcus aureus. Although Streptococcus pneumoniae infections are common in the community and can cause nosocomial infections, the incidence and risk factors for pneumococcal infections in burn patients is unclear. METHODS: We performed an electronic retrospective chart review to collect rates of and risk factors for S. pneumoniae infections in patients with thermal burns from March 2003 through June 2008.

Proper early weaning from ventilator assistance influences the overall survival of patients with severe burns: A case-control study.

BACKGROUND: Ventilator assistance is crucial in the treatment of severely burned patients. However, there is no consensus on the optimal duration of ventilator assistance, and whether it has an impact on the short - and long-term prognosis of severely burned patients is still unclear. METHODS: We incorporated 185 severely burned patients in our study. Kaplan-Meier analysis and Chi-square tests were applied to reveal the associations of days of ventilator assistance with patients' overall survival (OS) and other clinical variables. Multivariable Cox regression was applied, and the model was di
