Treatment of proximal phalangeal fractures with an antegrade intramedullary screw: a cadaver study.

The aim of this study was to quantify the articular cartilage defect created with two different antegrade techniques of intramedullary osteosynthesis with a headless compression screw inserted through the metacarpophalangeal joint. In 12 out of 24 fingers from six cadaveric hands, a trans-articular technique with cannulated headless compression screws (2.2 and 3.0 mm diameter) was used; whereas in the other 12 fingers, an intra-articular fixation technique was used. The areas of the articular surface and the defects created were measured with a digital image software program. All measurements …

Biomechanical assessment of flexible flatfoot correction: comparison of techniques in a cadaver model.

BACKGROUND: Options for surgical correction of acquired flexible flatfoot deformity involve bone and soft-tissue reconstruction. We used an advanced cadaver model to evaluate the ability of key surgical procedures to correct the deformity and to resist subsequent loss of correction. METHODS: Stage-IIB flatfoot deformity was created in ten cadaver feet through ligament sectioning and repetitive loading. Six corrective procedures were evaluated: (1) lateral column lengthening, (2) medial displacement calcaneal osteotomy with flexor digitorum longus transfer, (3) Treatment 2 plus lateral column …

Risk to neurovascular structures using posterolateral percutaneous ankle screw placement: a cadaver study.

BACKGROUND: Over the past 20 years, ankle arthrodesis with use of screw augmentation has become a popular technique to gain fusion of the arthritic ankle. The objective of this cadaver study was to identify the risks to local neurovascular structures using standard operative practices for percutaneous guide pin placement. METHODS: Nine fresh frozen cadaver limbs were used. A guide pin from the Synthes (Paoli, PA) 7.3-mm cannulated set was placed percutaneously into the distal posterolateral leg with the ankle held in neutral position. A layered dissection was then performed from the skin to t…

Formal procedural skills training using a fresh frozen cadaver model: a pilot study.

Graduating medical students are expected to be proficient in a number of procedural skills. A structured curriculum is infrequently available. In addition, the use of a mannequin tends to be unrealistic and students continue to have some anxiety when performing procedures on patients. This pilot study was designed to demonstrate the utility of a fresh frozen cadaver model in practicing procedural skills as compared to mannequins. Seven third-year students carried out a number of basic procedural skills using a mannequin model and a fresh frozen cadaver. We surveyed the students after they had …

Minimally invasive endoscopic reconstruction technique of acute AC-joint dislocations: a cadaver study.

The treatment of acute Rockwood type III AC-joint dislocations is controversial. Problems related to open surgery are soft tissue healing, residual instability and the necessity of hardware removal. After non-operative therapy the cosmetic result may be problematic and in some cases symptomatic instabilities occur. The goal of the present cadaver study was to develop a new, minimally invasive technique for acute AC-joint reconstructions and to analyse its potential risk for neurovascular injuries. The surgical technique was based on an arthroscopically assisted reconstruction of the coracoclav…

Plantar pressure and load in cadaver feet after a Weil or chevron osteotomy.

BACKGROUND: Since metatarsal osteotomy was first used to treat metatarsalgia in the early twentieth century, many techniques have been described to accomplish the basic aim of reduction of load transmission through the operated metatarsal and reduction of localized high pressure on the plantar surface of the metatarsal. Our study examined two popular distal metatarsal neck osteotomies used for the relief of central metatarsalgia and the biomechanical changes that result from their use in a cadaver forefoot model. METHODS: After applying 445 N (100 lbs) of axially directed force, we measured p…

[Dimensions of the lateral condyle in non-arthritic valgus knees. A cadaver study].

PURPOSE OF THE STUDY: Although the valgus slope of the femoral condyles seen in valgus knee is thought to be due to hypoplasia of the lateral femoral condyle, this fact has not been proved. The aim of this study was therefore to evaluate the difference between the size of the lateral femoral condyle of neutral and valgus knees. MATERIAL AND METHODS: There were 41 non-arthritics lower limbs taken from 27 caucasians donors. Angular measurements were taken with respect to the mechanical axis of the bones. Femurs were then isolated from the tibia, all soft tissue was removed with respect to the a

Anatomic placement of the Herbert-Whipple screw in scaphoid fractures: a cadaver study.

An anthropometric cadaver study was performed to determine the optimal placement of a Herbert-Whipple screw in the scaphoid. Twenty pairs of cadaver wrists were examined and anthropometric analysis of the scaphoids using plain radiography, computed tomography, and en bloc resection was performed. Scaphoid vascularity was evaluated using India ink injection. Radiographic and actual measurements of height and width confirmed symmetry in paired scaphoids. Anatomic evaluation defined a safe zone for screw placement. K-wire placement defined the extent of this zone. The entry point of this zone was…

Dorsal intercarpal ligament capsulodesis for scapholunate dissociation: biomechanical analysis in a cadaver model.

The purpose of this study was to evaluate in cadavers a new method for treating scapholunate dissociations, dorsal intercarpal ligament capsulodesis (DILC), and to compare its performance with that of a previously described soft tissue reconstruction, Blatt capsulodesis (BC). A cadaver model was used to simulate normal and abnormal wrist motions. The positions of the scaphoid and lunate and their changes with wrist motion and ligament condition were recorded using biplanar radiographs taken posteroanteriorly and laterally. The scapholunate gap was measured on the posteroanterior radiographs an…

Soft tissue release in total knee arthroplasty. Cadaver study using knees without deformities.

Soft tissue releases are performed to correct fixed deformities in total knee arthroplasty. The goal of this in vitro study was to investigate the relationship between the individual steps in a medial (eight anatomic specimen knees) or lateral (four anatomic specimen knees) soft tissue release sequence, the resulting change in the medial and lateral tibiofemoral gaps, and the change in coronal angulation caused by 10 Nm varus and valgus moments in extension and 90 degrees flexion. An optical encoder was used to measure the coronal angulation. The tibiofemoral gaps were measured with calipers w

Transsection of the peroneal nerve complicating knee arthroscopy: case report and cadaver study.

We report the case of a 36-year-old male patient who sustained a hyperextension trauma of the left knee. After performing diagnostic arthroscopy (partial tear of the anterior cruciate ligament) and partial resection of the anterior cruciate ligament, the patient experienced a complete paralysis of the peroneal nerve. One year after the first surgical procedure, a reconstructive repair of the peroneal nerve and a transfer of the anterior tibial muscle was performed. This complication is reported for the first time in literature.

Effect of medial displacement calcaneal osteotomy on ankle kinematics in a cadaver model.

Although medial displacement calcaneal osteotomy has been advocated for treatment of acquired pes planus, no studies have determined the biomechanical consequences at the ankle of such a procedure. The present investigation examined the alteration in ankle motion that resulted from a medial sliding calcaneal osteotomy. In dorsiflexion, the ankle specimens were found to have altered internal rotation and varus alignment. At maximal dorsiflexion, there was a 76% increase in internal rotation (4.4 degrees +/- 2.5 degrees versus 2.5 degrees +/- 1.7 degrees for intact ankles, P < 0.0004) and an inc
