Implementation of an acute care vascular surgery "Surgeon of the Week" model improves efficiency of inpatient care delivery.

OBJECTIVE: Acute care vascular surgery (ACVS) comprises a significant proportion of modern vascular surgery practice. Given the burden of ACVS at our institution, we have transitioned from a traditional "on call" model to a "Surgeon of the Week" (SOW) model, in which a single surgeon covers all daytime inpatient consults and resultant operations over the course of a week. The SOW surgeon has no overnight call, outpatient clinic, and minimizes elective operations during this week, enabling dedicated care to inpatient responsibilities. In this study, we report our 2-year experience with the SOW

Aligning emergency admission and stroke center catchment areas: Association with intravenous thrombolysis rates.

OBJECTIVES: To assess whether co-location of a Primary Stroke Center (PSC) and an Emergency Department (ED) was associated with higher intravenous thrombolysis (IVT) rates after alignment of emergency admission pathways in the Central Denmark Region. METHODS: This retrospective before-and-after cohort study compared two 6-month periods before (July 1 to December 31, 2021) and after (July 1 to December 31, 2022) co-location following a hospital merger in February 2022. Patients were identified through the Danish Stroke Center Database and linked with prehospital records. The primary outcome wa

Continuing medical education in emergency plastic surgery for referring physicians: a prospective assessment of educational needs.

BACKGROUND: Continuing medical education for referring physicians is an essential part of raising the profile of plastic surgery and improving patient care. The authors conducted a prospective cohort study to assess the educational needs of emergency and primary care physicians who refer patients to the on-call plastic surgeon. METHODS: The following information was collected for telephone referrals from emergency and primary care physicians over a 1-year period: date, location of referral center, population of referral center, distance between referral center and tertiary care hospital, pati

Increasing challenges of general practitioner-oncologist interaction in end-of-life communication: a qualitative study.

BACKGROUND: The timely initiation of end-of-life (EOL) conversations is considerered best practice for patients with advanced cancer and therefore recommended in oncological guidelines. General practitioners (GPs) play a key role in the coordination of EOL-care and often claim that they have also the responsibility to initiate the necessary conversations. Nonetheless, the content of these conversations is rarely a subject of communication between GPs and oncology specialists but crucial for timely integration of palliative care. The aim of this study is to explore the GPs' perspectives on init

Implementation of Transitional Care Planning is Associated with Reduced Readmission Rates in Patients Undergoing Lower Extremity Bypass Surgery for Peripheral Arterial Disease.

BACKGROUND: Patients undergoing lower extremity bypasses (LEB) are at a high risk of developing post-discharge complications requiring readmission. Health systems have developed several strategies to mitigate this risk. One such measure is developing comprehensive Transitional Care Program (TCP), which includes phone calls to patients after being discharged from the hospital. Our study aimed to assess short-term readmission, mortality, and amputation rates of patients who participated in TCP by completing at least one post-discharge follow-up phone call after undergoing LEB for revascularizati

A Population-Level Evaluation of Emergency General Surgery Models of Care and Clinical Outcomes.

IMPORTANCE: Outcomes in emergency general surgery vary between hospitals, and models with dedicated resources and personnel have been developed with the aim of improving care. Existing literature is limited in scope and often does not involve nonoperatively treated patients, reducing generalizability. OBJECTIVE: To use population-level data to determine whether treatment in an emergency general surgery model is associated with clinical outcomes. DESIGN, SETTING, AND PARTICIPANTS: This was a retrospective cohort study of adults in Ontario, Canada, diagnosed with 1 of 9 emergency general surge

Impact of Early Vasopressor Administration on Neurological Outcomes after Prolonged Out-of-Hospital Cardiac Arrest.

UNLABELLED: Introduction Vasopressors are associated with return of spontaneous circulation (ROSC), but no long-term benefit has been demonstrated in randomized trials. However, these trials did not control for the timing of vasopressor administration which may influence outcomes. Consequently, the objective of this study was to develop a model describing the likelihood of favorable neurological outcome (cerebral performance category [CPC] 1 or 2) as a function of the public safety answering point call receipt (PSAP)-to-pressor-interval (PPI) in prolonged out-of-hospital cardiac arrest. [The course of rehabilitation in patients with locked-in syndrome].

On the basis of observations of the course taken by 5 patients with so-called Locked-in Syndrome (LIS), the level of functional performance that can be achieved by long-term rehabilitation notwithstanding very severe initial symptoms is set out. The priority measures in the early phases of the condition and the particular problems of long-term rehabilitation are summarized in a checklist. Technical aids to ameliorate communication are dealt with in some detail. Special importance is attributed to patient-centered psychotherapeutic support and to family involvement.

Recognition of visual symptoms in stroke: a challenge to patients, bystanders, and Emergency Medical Services.

BACKGROUND: Identification of visual symptoms as a sign of acute stroke can be challenging for both first line healthcare professionals and lay persons. Failed recognition of visual symptoms by medical dispatchers at the Emergency Medical Dispatch Center (EMDC-112) or personnel at the Out-of-Hours Health Service (OOHS) may delay stroke revascularization. We aimed to identify correct system response to visual symptoms in emergency calls. METHODS: Phone calls from patient or bystander to the EMDC-112 or OOHS, which included visual symptoms on patients later verified with stroke/Transient ischem

Clinical Characteristics and Outcomes of Aneurysmal Subarachnoid Hemorrhage Patients Who Live Alone.

It has been shown that living alone is one of the risk factors for unfavorable outcomes in ischemic stroke patients, mostly due to delay in receiving appropriate treatment. A single-center retrospective observational study was conducted to evaluate whether living alone was associated with unfavorable outcomes in aneurysmal subarachnoid hemorrhage (SAH) patients. Among 451 SAH patients admitted to our institution between January 2013 and December 2022, 43 patients who lived alone had sustained SAH at home (group A) and 329 patients who lived with family had sustained SAH at home (group F). A Clinical Training Program for Hybrid Closed Loop Therapy in a Pediatric Diabetes Clinic.

BACKGROUND: Hybrid closed loop (HCL) therapy is now available in clinical practice for treatment of type 1 diabetes; however, there is limited research on how to educate patients on this new therapy. The purpose of this quality improvement project was to optimize a HCL education program for pediatric patients with type 1 diabetes (T1D). METHODS: Our multidisciplinary team developed a novel HCL clinical training program for current insulin pump users, using a quality improvement process called the Plan-Do-Study-Act model. Seventy-two patients participated in the HCL training program, which inc

A model for delivery of extracorporeal life support in a stand-alone veterans affairs medical center.

INTRODUCTION: For the Veterans Health Administration (VHA) to continue to perform complex cardiothoracic surgery, there must be an established pathway for providing urgent/emergent extracorporeal life support (ECLS). Partnership with a nearby tertiary care center with such expertise may be the most resource-efficient way to provide ECLS services to patients in post-cardiotomy cardiogenic shock or respiratory failure. The goal of this project was to assess the efficiency, safety, and outcomes of surgical patients who required transfer for perioperative ECLS from a single stand-alone Veterans Af

The Parkinson's Universe: A person-centered care model for Parkinson's disease.

Parkinson's disease is the fastest growing neurodegenerative disorder worldwide, yet care delivery remains fragmented and inequitable. We propose a new construct called the Parkinson's Universe. It is a person-centered model that reimagines care as a coordinated universe. This commentary summarizes the model's elements including the patient as the sun or center of the universe, the caregiver as Mercury, the social support and multidisciplinary healthcare professionals as the other planets, mission control as care coordination, stigma as Pluto, barriers as asteroids, supportive technology as sa

Life-saving fall detection by a smartwatch in a case of ventricular fibrillation.

New smart devices that have the potential to support the health and well-being of their owners have become available. In particular, smart watches are able to identify a fall by the person who is wearing the watch and report it to pre-defined contacts and the local emergency control center. Falls in older people are common and only rarely caused by malignant cardiac arrhythmia. The case of an elderly male whose smart watch automatically reported his fall due to ventricular fibrillation to the local emergency control center is described. Through the intervention of the wearer's device, the acti

Cell dynamics in the germinal center of the human tonsil.

Apoptosis of the human tonsillar germinal center was studied by means of immunocytochemical methods. It is generally accepted that the germinal center of the lymphoid tissues plays an important role both in cell proliferation and cell death. Although many studies on cell proliferation have been reported, the cell death mechanism is not yet well understood. According to recent studies, apoptosis is the result of programmed, not pathological cell death, and it plays an important role in the maintenance, immunity and development of life. The present study showed that the tingible body was a mass

Language considerations for children of parents with substance use disorders.

Parents with substance use disorders are highly stigmatized by multiple systems (e.g., healthcare, education, legal, social). As a result, they are more likely to experience discrimination and health inequities [1, 2]. Children of parents with substance use disorders often do not fare any better, as they frequently experience stigma and poorer outcomes by association [3, 4]. Calls to action for person-centered language for alcohol and other drug problems have led to improved terminology [5-8]. Despite a long history of stigmatizing, offensive labels such as "children of alcoholics" and "crack

[Anesthesia in malignant hyperthermia].

Malignant hyperthermia is a condition for which the anesthesiologist must be prepared to take specific measures. Associated mortality is high despite correct diagnosis and, occasionally, the right treatment. Malignant hyperthermia is rare in Spain, however, and perhaps for this reason the Spanish public health authorities have not made an effort to respond to persistent calls for a national reference center for study and diagnosis, such as can be found in other European countries. The problem of malignant hyperthermia may become worse in the near future due to changes in the population brought

TeLePhone Respiratory (TeLePoR) score to assess the risk of immediate respiratory support through phone call for acute dyspnoea: a prospective cohort study.

BACKGROUND: Acute dyspnea is a frequent cause to call the Emergency Medical Call Center (EMCC). The main challenge for EMCC dispatchers is to quickly identify patients that will require respiratory support in order to provide them with the most accurate prehospital response. Our main objective was to derivate a score assessable during the first call to detect the most severe patients needing medical assistance. METHODS: This prospective observational cohort study was conducted in four different French EMCC from January 22nd to March 7th 2024. Patients over the age of 18 years old that called

Exploring the Telephone Call Experience of Patients with Non-English Language Preference in Primary Care vs. Specialty.

BACKGROUND: Latine people, comprising 18.5% of the US population, constitute the largest ethnic minority group, with nearly one-third self-identifying as having non-English language preference (NELP). Despite the importance of the telephone in health care access, there is limited understanding of how NELP patients navigate telephone calls with primary and specialty care clinics. OBJECTIVE: This qualitative study aims to capture how Spanish speakers with NELP characterize their telephone call experiences with primary and specialty care clinics. Quitline Activity in the Republic of Korea.

To reduce tobacco use and related harm in Korea, telephone based cessation services (Quitlines) began full operation to provide regular behavioral counseling for smoking cessation in 2006. After registration in the cessation program, at least 21 calls per year are given to each client to help quit and encourage maintenance. Tailored programs for males, females, and adolescent smokers have been offered taking into account smokers' characteristics and smoking behavior. Mailing self-help quit packs and e-mail and SMS services are allowable as additional services.A total of 23,201 smokers were reg
