Nasopharyngeal airway pressures in bronchiolitis patients treated with high-flow nasal cannula oxygen therapy.

BACKGROUND: Heated, humidified, high-flow nasal cannula oxygen therapy (HHHFNC) has been used to improve ventilation in preterm infants. There are no data on airway pressures generated and efficacy in bronchiolitis. OBJECTIVE: The objective of this study was to determine nasopharyngeal (NP) pressures generated with HHHFNC therapy in bronchiolitis. METHODS: We conducted a prospective, observational study to measure NP pressures at varying flow rates of HHHFNC therapy in moderate to severe bronchiolitis. Vital signs, bronchiolitis severity scores, and oxygen saturation were also noted. RESULT…

Echocardiography, not chest radiography, for evaluation of cannula placement during pediatric extracorporeal membrane oxygenation.

OBJECTIVE: Optimal cannula position is essential during extracorporeal membrane oxygenation (ECMO). We hypothesize that echocardiography is superior to chest radiography in diagnosing abnormal cannula position during ECMO. DESIGN: Retrospective. SETTING: Pediatric hospital. PATIENTS: 100 pediatric patients requiring ECMO. MEASUREMENTS AND MAIN RESULTS: We reviewed the medical records of all ECMO patients (n = 100), including reports of all echocardiograms (n = 326), during the years 2002-2004. Of the 91 patients who had echocardiograms while on ECMO, 33 had at least 1 echocardiogram for ca…

Predictors for treating obstructive sleep apnea with an open nasal cannula system (transnasal insufflation).

BACKGROUND: Obstructive sleep apnea (OSA) is a disorder that is associated with increased morbidity and mortality. Although continuous positive airway pressure effectively treats OSA, compliance is variable because of the encumbrance of wearing a sealed nasal mask throughout sleep. In a small group of patients, it was recently shown that an open nasal cannula (transnasal insufflation [TNI]) can treat OSA. The aim of this larger study was to find predictors for treatment responses with TNI. METHODS: Standard sleep studies with and without TNI were performed in 56 patients with a wide spectrum …

Stereotactic breast biopsy: en bloc excision of microcalcifications with a large-bore cannula device.

OBJECTIVE: Breast calcifications pose a significant diagnostic and procedural dilemma. We evaluated en bloc stereotactic excision of indeterminate and suggestive microcalcifications for histologic diagnosis using a large-bore cannula biopsy device. MATERIALS AND METHODS: We retrospectively reviewed 61 groups of microcalcifications removed with a large-bore cannula biopsy device from 59 patients (age range, 35-72 years old). The cannula diameter was 20 mm in 47 cases, 15 mm in nine cases, and 10 mm in five cases. The median lesion diameter was 6.6 mm (range, 4-17 mm). The procedure was perform…

In Situ Venous Cannula Puncture: A Simplified Technique for Extracorporeal Membrane Oxygenation Blood Salvage.

Salvaging residual circuit blood upon extracorporeal membrane oxygenation (ECMO) discontinuation is a clinically valuable blood conservation strategy, mirroring standard cardiopulmonary bypass protocols. We describe a simplified technique using an 8F vascular sheath inserted into the venous cannula via the Seldinger technique. Post-weaning, crystalloid infusion displaces 400-600 ml of sequestered autologous blood back to the patient at low pump speeds. This "off-the-shelf" method requires no prior circuit modification, maintains system integrity, and effectively reduces transfusion needs. It o…

High Flow Nasal Cannula Use in Bronchiolitis: Predicting Need for Early Escalation of Care.

OBJECTIVES: High-flow nasal cannula (HFNC) use in bronchiolitis has increased substantially with wide variability, likely due to the lack of objective means of predicting clinical outcomes. Our objective was to identify features associated with care escalation for infants started on HFNC in the Emergency Department (ED), thereby assisting in disposition of patients and optimizing the utilization of limited resources. METHODS: This is a retrospective cohort study from 3 free-standing children's hospitals of infants younger than or equal to 12 months with bronchiolitis who were initiated on HFN…

Coarctation Stenting in a Preterm Neonate With Unrepaired Transposition Using ECMO Venous Cannula Access.

BACKGROUND: Cardiac catheterization is essential for diagnosing and treating critically ill infants with congenital heart disease who require venoarterial extracorporeal membrane oxygenation (VA-ECMO). In neonates, cervical cannulation is often preferred because of small femoral vessels. CASE SUMMARY: A 3-week-old premature male infant with dextro-transposition of the great arteries and coarctation of the aorta (CoA) developed cardiogenic shock and acute kidney injury after atrial septostomy and prostaglandin discontinuation, requiring VA-ECMO. A benchtop model identified the 8-F venous cannu…

Performance of an indigenously developed high-flow nasal cannula device in healthy human volunteers.

BACKGROUND: High Flow Nasal Cannula (HFNC) therapy provides clinical and physiological benefits, particularly in the management of acute respiratory failure. However, the performance of indigenously developed HFNC systems remains underexplored. This study evaluates the safety and efficacy of oxygenation with a novel Indian-manufactured HFNC device in healthy adult volunteers. METHODS: In this single-centre, participant-blinded, randomised, phase 0 study, 21 healthy volunteers, predominantly male participants; findings may not generalise. They were assigned to receive oxygen via an HFNC device …

How High-Flow Nasal Cannula Is Impacted by Nasogastric/Esophageal Tube: A Bench Study.

High-flow nasal cannula (HFNC) is a common noninvasive respiratory therapy for respiratory failure, offering positive airway pressure and dead space clearance. In critically ill patients, additional nasal tubes for feeding or monitoring are often required, but their effect on HFNC performance is not well understood. This bench study evaluated the impact of nasal tube placement on dead space clearance, nasopharyngeal pressure, and airway resistance using standard and asymmetrical cannulas. Inserting a 5-Fr tube with a standard cannula had very little effect, whereas bigger sizes (12-Fr and 16-F…

Reducing Hormonal Inputs in Rabbit Reproduction: Physical Ovulation Induction with a 3D-Printed Cannula.

This study evaluated the effectiveness of a 3D-printed cannula in inducing ovulation in female rabbits without exogenous GnRH analogues. A total of 325 inseminations were performed across different physiological stages: nulliparous, multiparous lactating, and multiparous non-lactating does. Two insemination methods were compared: a physical stimulation technique using the 3D-printed cannula and a control method involving hormonal induction with buserelin acetate. Females were either synchronized with eCG or left untreated. Delivery rates and litter sizes were recorded to assess fertility outco…

Cannula-Associated Deep Vein Thrombosis Following Extracorporeal Membrane Oxygenation Decannulation: Incidence and Risk Factors.

Thrombotic complications during and after extracorporeal membrane oxygenation (ECMO) are commonly observed clinically. The incidences of cannula-associated deep vein thrombosis (CaDVT) post-ECMO support have predominantly focused on Caucasian demographics. This study aims to determine the incidence and risk factors for CaDVT in Vietnamese patients following ECMO decannulation. The retrospective study from January 2019 to February 2020 observed ECMO weaning patients and screened for CaDVT using Doppler ultrasonography. Data were collected on patient demographics, ECMO parameters, and transfusio…

Tailoring left ventricular assist device cannula implantation using coupled multi-scale multi-objective optimization.

BACKGROUND: The frequent occurrence of thromboembolic cerebral events continues to limit the widespread implementation of Ventricular Assist Devices (VAD) despite continued advancements in VAD design and anti-coagulation treatments. Recent studies point to the optimal positioning of the outflow graft (OG) as a potential mitigator of post implantation thromboembolism. OBJECTIVE: This study aims to examine the tailoring of the OG implantation orientation with the goal of minimizing the number of thrombi reaching the cerebral vessels by means of a formal shape optimization scheme incorporated in

Lung Volume and Ventilation Distribution After Bariatric Surgery: High-Flow Nasal Cannula Versus CPAP.

BACKGROUND: Patients with obesity are at increased risk of postoperative pulmonary complications. CPAP has been used successfully to prevent and treat acute respiratory failure, but in many clinical scenarios, high-flow nasal cannula (HFNC) therapy is emerging as a possible alternative. We aimed to compare HFNC and CPAP in a sequential study measuring their effects on gas exchange, lung volumes, and gas distribution within the lungs measured through electrical impedance tomography (EIT). METHODS: We enrolled 15 subjects undergoing laparoscopic bariatric surgery. Postoperatively they underwent…
