Continuous intravenous infusion of chlorpromazine (CPZ) was used for both sedation and after-load reduction in 13 Japanese patients (Group 1) supported by an intraaortic balloon pump (IABP) after open-heart surgery. CPZ was initiated at a dose of 0.05 mg/kg/hr and was maintained in an approximate dose range of 0.1-0.3 mg/kg/hr depending on individual patient needs. The results obtained from a 5-year follow-up of 124 patients suffering from chronic circulatory insufficiency and having an implantable pacemaker were studied during various cardiotonic therapies. A combination of cardiac glycosides in subtherapeutical doses with peripheral vasodilators or cardioselective stimulants was found to enhance the efficacy of cardiotonic therapy and to prevent digitalis intoxication in low glycoside tolerance during ventricular pacing. Predictive criteria for clinical effects of cardiotonic therapy were defined.

Milrinone is an inotropic agent of the phosphodiesterase inhibitor family. In common with all molecules of this class it has both positive inotropic and vasodilator effects. The haemodynamic effects of 3 dosages of milrinone were studied in 25 patients with low output states after open heart surgery. An intramural coronary artery in the setting of truncus arteriosus (common arterial trunk) is an uncommon association. Following an uneventful surgical repair, a neonate developed a low cardiac output state deteriorating into cardiac arrest shortly after arrival into the intensive care unit, requiring extracorporeal membrane oxygenation support. Echocardiography and angiography showed occlusion of the left coronary artery, prompting emergency surgical reexploration. We report a case of familial contracted endocardial fibroelastosis (EFE) in a young boy presenting at 14 months of age with severe heart failure. A previous echocardiogram showed normal left ventricular (LV) size and systolic function. The family history was suggestive of X-linked cardiomyopathy. These findings are assessed in light of earlier reports of contracted EFE.
