The future of interventional cardiology lies in the left atrium.

The historic focus of interventional cardiology has been on treating atherosclerotic coronary artery disease. Balloons, stents, and debulking devices have been the mainstay tools of the interventionalist. However, as engineering skills have developed and clinical techniques have improved, new clinical avenues for the interventionalist beyond the coronary vessels are starting to evolve. One of the most opportunistic areas in cardiology for the interventionalist is the left atrium where treatment of clinically important issues such as mitral regurgitation, left atrial thrombus, atrial fibrillati…

[The ECG--a basic examination method in the cardiology laboratory].

Electrocardiography entertains a centennial anniversary, but it still remains the basic and widely used examination method in heart diseases. Its important position remains even within the framework of basic clinical research, where it represents a starting parameter for the evaluation of other more sophisticated methods of examination. The strengthening of basic research and rapid application of the obtained knowledge in clinical practice were the main reasons for the foundation of the "Laboratory for Research of Blood Circulation Control". The laboratory is particularly engaged in investigat

Cardiologic problems in the post acute ventilated patient.

Chronically critically ill patients who develop acute respiratory failure commonly have complicating cardiac pathology that may or may not be evident at initial evaluation. The acute coronary syndromes should be excluded in all patients presenting with respiratory failure. Cardiac rhythm disturbances are common and should be actively investigated and treated in all critically ill patients. Heart failure is common in the chronically critically ill patient but usually responds to early diagnosis and prompt treatment. Finally, cardiogenic shock carries a poor prognosis in most patient subsets exc

[Identification of psychologically impaired patients in cardiologic rehabilitation].

As psychological factors have a substantial influence on the further medical, psychological and occupational career of a patient with somatic disease, early identification of patients with psychological disturbances is of special importance in medical rehabilitation. The share of psychologically disturbed patients in a cardiac rehabilitation clinic has been screened. Different methods for identification of anxious and depressive patients (by medical and psychological staff as well as a screening instrument) are compared. Concluding, the benefit of introducing a screening instrument as a routin

The impact of physical activity in preventive and rehabilitative cardiology.

Worldwide exercise training is used for purposes of the preventive and the rehabilitative cardiology. After some historical remarks the consequences of prolonged bed rest as a type of lack of exercise are described. It follows the recommendation of a suitable quality, quantity and intensity of exercise training for preventive medicine. Peripheral and central adaptations are described as well as the significance for hemodynamic and metabolic changes. Animal trials with primates give also a clear evidence about positive effects of exercise training. First convincing results are also delivered by…

[Preoperative cardiological evaluation. Report of 255 cases].

PURPOSE: To study preoperative cardiological consultations in a cardiology referral center in a general hospital. METHODS: Two hundred and fifty five patients were studied between June and December 1989; the ages ranged between 16 and 82 (mean 55) years, 120 (47%) were male and 135 (53%) female. A questionnaire was applied by the physicians during consultation. RESULTS: Main symptoms were thoracic pain in 30 (11.8%) cases, dyspnea in 57 (22.4%), palpitations in 13 (5.1%). Symptoms of cardiac disease were absent in 141 (55.3%) patients. On physical examination arterial hypertension was detect…

The role of interventional cardiology in pediatric heart transplantation.

Hypoplastic left heart syndrome (HLHS) is the most common cause of cardiac death during the first week of life. The prognosis of this fatal malformation has changed completely since the introduction of neonatal heart transplantation. However, patients awaiting a heart donor are at high risk for closure of the ductus arteriosus and are exposed to complications from indwelling central venous catheters and long-term use of prostaglandin E1. To maintain the ductal patency, which is crucial for the survival of these patients, we performed a stenting of the ductus arteriosus in six infants with HLHS

[Indications for emergency cardiovascular surgery at the Institute of Cardiology].

Between 1981 and 1989 73 patients (46 males, 27 females aged 15-69; mean 42) underwent emergency cardiac operation in the I Clinic of Cardiosurgery. There were 39 (53%) patients with valvular disease, 18 (25%) with prosthetic valve dysfunction and endocarditis, 9 (12%) with acute aortic dissection (2 of them had Marfan's syndrome), 5 (7%) with atrial myxoma and 2 (3%) with malignant pericardial mesothelioma. All of them were treated in C.C.U. before surgery. 27 urgent operations were performed within the first 24 hours after the admission to C.C.U. 30 of 73 patients died (41%). The IV NYHA fun

[Cardiologic findings in incidental heart sounds in childhood].

In a random sample of patients examined in the outpatient department of pediatric cardiology in Giessen, an innocent heart murmur was found in 1009 children. Results and methods of cardiological examination relevant to the practitioner were established by evaluating the individual diagnostic criteria. An anamnesis without any reference to heart disease is of special significance for the diagnosis of an innocent heart murmur. Another important factor is the experience of the examining doctor. Innocent murmurs are systolic and frequently of "musical quality", their intensity ranges from grade 1/…

[Cardiological problems in the neonatal period (author's transl)].

Knowledge of the changes during transition from fetal to neonatal circulation is useful for the understanding of physiologic and pathologic variations in the neonatal cardiovascular system. Cyanosis as well as clinical symptoms of congestive heart failure in the neonate are unspecific and may represent cardiac and noncardiac disease (primary pulmonary disease, septicemia, intracranial haemorrhage or meningitis, polyglobulism). Besides detailed analysis of clinical symptoms, the examination of arterial bloodgases when breathing from room air and 100% oxygen, the electrocardiogram, the chest-roe
