Kidney transplantation is associated with catastrophic out of pocket expenditure in India.

Kidney transplantation (KT) is only viable renal replacement option for most patients in India. Most patients do not have health insurance and meet treatment expenditure from their own resources. We prospectively evaluated the expenses associated with KT and its impact on the socioeconomic status of families in a public hospital. All direct and indirect expenses incurred by the patients from the time of diagnosis of chronic kidney disease to KT were recorded. Direct expenses included physician fees, cost of drugs and disposables, dialysis, and expenses on investigations and hospitalization. In

Hip sonography update. Quality-management, catastrophes - tips and tricks.

AIM: Hip sonography is today considered to be the diagnostic standard tool for developmental dysplasia of the hip (DDH) diagnosis. In Germany special commissions control the quality of the sonograms according to checklists to keep the standard high and to avoid over- and under - treatment. This paper lists tips and tricks to avoid typical mistakes according to these results. MATERIAL AND METHOD: In 8 German countries the quality of sonograms used to diagnose DDH was checked according to special checklists. An additional 250 medical doctors (pediatricians, radiologists, orthopaedic surgeons) w

Pain-related fear of movement and catastrophizing in whiplash-associated disorders.

OBJECTIVE: The main aim of this study was to test whether pain-related fear of movement and catastrophizing were predictors of pain intensity and disability 6 months after a whiplash injury. The mediator role of fear of movement was also explored. A sample of 147 whiplash patients with neck pain for less than 3 months participated in the first assessment, and 123 of them were interviewed again at a 6-month follow-up. Multiple regression analyses were performed. RESULTS: Fear of movement, initial pain intensity and initial disability were independent predictors of disability at the 6-month fol

Contribution of kinesophobia and catastrophic thinking to upper-extremity-specific disability.

BACKGROUND: Upper-extremity-specific disability correlates with mood and coping strategies. The aim of this study was to determine if two psychological factors, kinesiophobia (fear of movement) and perceived partner support, contribute significantly to variation in upper-extremity-specific disability in a model that included factors known to contribute to variation such as depression, pain anxiety, and catastrophic thinking. METHODS: We performed an observational cross-sectional study of 319 patients who each had one of the following conditions: trigger finger (n = 94), carpal tunnel syndrome

Understanding catastrophic worry in eating disorders: process and content characteristics.

BACKGROUND AND OBJECTIVES: The majority of people with eating disorders (ED) experience high levels of comorbid anxiety and depression, yet the maintenance processes of these in ED remain largely unknown. Worry, a defining cognitive feature and important maintenance factor of anxiety, has not been well-studied amongst people with ED. This is the first study to explore both the process and content characteristics of catastrophic worry in ED. METHODS: Twenty-nine patients with anorexia nervosa (AN), 15 patients with bulimia nervosa (BN) and 37 healthy controls (HC) completed measures assessing

PP068. Catastrophic antiphospholipid-syndrome (CAPS) - A severe pregnancy complication.

INTRODUCTION: Antiphospholipid syndrome (APS), is an autoimmune, hypercoagulable state caused by antibodies against cell-membrane phospholipids provoking arterial and venous thromboses as well as pregnancy-related complications such as miscarriage, stillbirth, preterm delivery, or severe preeclampsia. The syndrome occurs due to the autoimmune production of antibodies against phospholipid (aPL), a cell membrane substance. In particular, the disease is characterised by antibodies against cardiolipin (anti-cardiolipin antibodies) and β2 glycoprotein I. In rare cases, APS can lead to rapid organ f

Pain catastrophizing and pain coping among methadone-maintained patients.

OBJECTIVE: The aim of this study was to examine the association of pain catastrophizing and pain coping strategies with characteristic pain intensity (an average of worst, least, and typical pain intensity in the past week) and recent pain-related disability (an average of three measures of past week pain interference) in opioid-dependent patients enrolled in a methadone maintenance treatment program (MMTP) who reported recent pain. DESIGN: Cross-sectional survey. PATIENTS: One hundred and eight MMTP patients who reported recent pain. MEASURES: Participants completed measures of demographic

Pain-related fear, catastrophizing and pain in the recovery from a fracture.

Background and aims Pain-related fear and catastrophizing are prominently related to acute and persistent back pain, but little is known about their role in pain and function after a fracture. Since fractures have a clear etiology and time point they are of special interest for studying the process of recovery. Moreover, fracture injuries are interesting in their own right since patients frequently do not recover fully from them and relatively little is known about the psychological aspects. We speculated that catastrophizing and fear-avoidance beliefs might be associated with more pain and po

Cyclopia with shoulder dystocia leading to an obstetric catastrophe: a case report.

INTRODUCTION: Cyclopia is a rare fetal malformation characterized by a single palpebral fissure and a proboscis associated with severe brain malformations. Approximately 1.05 in 100,000 births including stillbirths are identified as cyclopean. The prevalence is about one in 11,000 to 20,000 in live births and one in 250 during embryogenesis. CASE PRESENTATION: A 30-year-old Indian woman of Asian origin, sixth gravida, was referred to the labor room of our hospital. There were no ultrasound examinations performed during this pregnancy as our patient had not received regular antenatal care. We

Recurrence of primary hyperoxaluria: an avoidable catastrophe following kidney transplant.

Primary hyperoxaluria is a rare autosomal recessive disease due to deficiency of an oxalate-metabolizing liver enzyme, which results in nephrolithiasis and renal failure. Concomitant liver and kidney transplant is recommended as isolated kidney transplant is inevitably complicated by recurrence of the disease. We present a 25-year-old man with end-stage nephrolithiatic renal disease who underwent bilateral nephrectomy, followed by kidney transplantation. There was progressive worsening of kidney function two weeks post transplant. Review of nephrectomy and transplant kidney biopsy showed abund

Sensitization of catastrophic cognition in cognitive-behavioral therapy for panic disorder.

BACKGROUND: Cognitive model of panic disorder have proposed that panic attacks result from the catastrophic misinterpretation of certain bodily sensations. Cognitive-Behavioral Therapy (CBT) for panic disorder aims to change these catastrophic cognitions. CBT intervention successfully caused reduction of catastrophic cognitions and symptomatic improvement in the majority of cases. However there are some patients who fail to modify their catastrophic cognitions or rather experience an increase in them during CBT treatment. It is clinically and theoretically important to understand about cogniti

Diffuse large and small bowel necrosis in catastrophic antiphospholipid syndrome.

The catastrophic variant is an accelerated form of the antiphospholipid syndrome resulting in multiorgan failure because of multiple small vessel occlusions. We report a case of catastrophic antiphospholipid syndrome in a patient with subacute cutaneous lupus erythematosus and ischemic bowel, who presented with acute abdominal pain due to diffuse right colon and small bowel necrosis requiring large resection, associated with acute respiratory distress syndrome, thrombocytopenia and disseminated intravascular coagulation. Histopathological examination of resected tissues showed diffuse arteriol

Silo pouch stoma: a rescue procedure for intestinal catastrophe in gastroschisis.

Silo pouch formation is a standard procedure to prevent compartment syndrome in gastroschisis. Intestinal complications such as perforation and volvulus can occur and their management can be perplexing. We present three such patients in which we formed a stoma through the silo pouch owing to these complications. Creation of stoma through the silo is a novel, safe temporizing technique to decompress the bowel while delayed reduction continues. Subsequently, when the baby and the bowel improve, the stoma can be closed.

Multiorgan failure and antiphospholipid antibodies: the catastrophic antiphospholipid (Asherson's) syndrome.

A review of 250 patients with the catastrophic antiphospholipid (Asherson's) syndrome (CAPS) taken from the web site organized by the Europhospholipid Group (http://www.med.ub.es/MIMMUN/FORUM/CAPS.HTM) is presented in this paper. A short historical overview of the antiphospholipid syndrome (APS) is followed by a description of the "triggering" factors, associated autoimmune diseases, clinical presentation, presumed pathogenesis, prognosis, mode of death and suggested therapies. Triggering factors are present in approximately 50% of patients and consist predominantly of infections, trauma, incl
