Acoustic shocks and traumas sometimes result in a cluster of debilitating symptoms, including tinnitus, hyperacusis, ear fullness and tension, dizziness, and pain in and outside the ear. The mechanisms underlying this large variety of symptoms remain elusive. In this article, we elaborate on the hypothesis that the tensor tympani muscle (TTM), the trigeminal nerve (TGN), and the trigeminal cervical complex (TCC) play a central role in generating these symptoms. We argue that TTM overuse (due to the acoustic shock), TTM overload (due to muscle tension), and ultimately, TTM injury (due to hypoxi

OBJECTIVE: Cervical and ocular vestibular evoked myogenic potentials (cVEMPs and oVEMPs) are commonly used in evaluation of neurotologic disorders. We present a case of sudden bilateral hearing loss immediately after oVEMP and cVEMP testing. The hearing loss did not recover. To our knowledge, no previous case reports discuss sudden hearing loss, especially bilateral, associated with VEMP testing.
PATIENT: A single patient with sudden bilateral hearing loss that has persisted after cVEMP and oVEMP.
INTERVENTION: The patient had a history of chronic daily dizziness. She underwent vestibular fu

BACKGROUND: Arcuate foramen is an ossification of the posterior atlanto-occipital membrane, forming a bony opening through which the vertebral artery (VA) enters the vertebral canal. Block vertebra is a synostosis of at least 2 vertebral bodies that did not separate during the embryological development. It is worth distinguishing it from Klippel-Feil syndrome, as the latter oftentimes involves other abnormalities (namely skeletal) and is typically diagnosed in childhood. Both variants potentially lead to impairment of blood flow through the VA.
CASE REPORT: The following case report presents

OBJECTIVES: To introduce the trigeminal cervical complex (TCC) as a comprehensive framework for understanding the anatomical and functional scope of the afferences and efferences of the trigeminal nerve and explaining common otolaryngologic symptoms, including head and neck myofascial pain syndrome. Additionally, it explores how the bidirectional transmission of neurotransmitters contributes to the sensitization of the TCC and motor nuclei.
METHODS: The study was conducted as a narrative review. The authors performed a comprehensive search of multiple databases, including Medline/OVID, Embase

Over the last 2 decades, sports-related concussion (SRC) awareness and management have evolved from an emphasis on complete cognitive and physical rest to evidence-based protocols and interventions. Chiropractors are primary care providers with exposure to athletes and teams in collision sports and, in addition, manage patients with concussion-like symptoms including neck pain, dizziness, and headache. With SRC frequently occurring in the absence of a medical practitioner, the role of allied health practitioners like chiropractors should be emphasised when it comes to the recognition, assessme

Diseases that cause malfunction of the sympathetic nervous system provide insight into how the sympathetic nerves normally modulate responses to stress. This paper discusses insight from a number of such diseases. Transection of the cervical spinal cord leads to autonomic dysreflexia. This syndrome causes episodic hypertension in quadriplegic patients from excess sympathetic activity reflexly activated by bowel or bladder distention. These patients lack cerebral control of spinal sympathetic reflexes. Radiotherapy to the neck can destroy the arterial baroreceptors that monitor blood pressure f

This case series presents an alternative treatment approach for refractory epilepsy in the setting of anti-gamma-aminobutyric acid B receptor (GABA-B-R) encephalitis and examines current guidelines for long-term cancer screening. We present two cases of anti-GABA-B receptor encephalitis from Kansas City, Missouri. The first case describes a 53-year-old woman presenting with new-onset headache, seizures, and progressive encephalopathy, who was treated with corticosteroids and anti-seizure medications, followed by intravenous immunoglobulin (IVIG) and plasma exchange (PLEX). Cancer screening wit

A 41-year-old woman experienced back pain upon waking up. Immediately afterward, she experienced a continual orthostatic headache. Thereafter, right ear fullness and dizziness also occurred. One month later, she became aware of repeated numbness that started in the right hand and spread to the right half of the body and lower limbs and continued for repeated periods of approximately 20-30 min. Neurological examination revealed no abnormal findings except for orthostatic headache. Electroencephalography showed no epileptic discharge. Head MRI revealed left convexal subarachnoid hemorrhage (cSAH

BACKGROUND: Cervical transforaminal epidural steroid injections (CTFESI) are commonly used to treat cervical radicular pain, but concerns remain about their safety due to case reports of complications such as stroke, spinal cord injury and death. These complications have been associated with use of particulate steroids causing embolic infarcts from inadvertent intra-arterial injection. With the use of non-particulate steroids these complications have never been reported. Additionally, recent cohort studies have demonstrated safety. Yet many providers still consider these procedures to be unsaf

PURPOSE: To perform a Delphi consensus for on-field and pitch-side assessment of sports-related concussion (SRC).
METHODS: Open-ended questions in rounds 1 and 2 were answered. The results of the first 2 rounds were used to develop a Likert-style questionnaire for round 3. If agreement at round 3 was ≤80% for an item, if panel members were outside consensus, or there were >30% neither agree/disagree responses, the results were carried forward into round 4. The level of agreement and consensus was defined as 90%.
RESULTS: Loss of consciousness (LOC) or suspected LOC, motor incoordination/atax

BACKGROUND: Patients with cervicogenic symptoms following a concussion have shown a disproportionate rate of delayed symptom resolution. However, the prevalence of cervicogenic symptoms in the acute stages following a concussion and the percentage of those patients who continue on to suffer delayed symptom resolution is poorly described in the literature.
OBJECTIVES: To provide a comprehensive report on the clinical prevalence, diagnostic methods, and potential treatment options for cervicogenic symptoms that are elicited during acute and chronic phases following a concussion.

BACKGROUND: Spinal cord injury (CIS) is an exceedingly rare complication of cerebral angiography, with only nine cases since 1962. Pathogenesis remains poorly understood.
CASE PRESENTATION: A 67-year-old diabetic woman presented with dizziness and normal neurological examination. During cerebral angiography, repeated injections of ioversol contrast medium (total 100 mL; approximately 20 min at a single site) were performed into the right subclavian artery for vertebral artery imaging due to vascular tortuosity. The angiography showed normal cerebrovascular morphology, without emboli, stenosis

INTRODUCTION: More than 50% of people who are diagnosed with whiplash-associated disorder (WAD) will report symptoms 12 months and beyond after their initial onset. However, many signs and symptoms, such as dizziness, emotional lability, confusion, ocular movement abnormalities, and balance deficits, may not be directly attributed to the cervical spine and may be more consistent with post-concussive syndrome (PCS).
METHODS: A total of 15 people with chronic (>3 months) WAD and 15 age-sex match controls were recruited. They were evaluated on clinical tools commonly used to assess signs and sym

BACKGROUND: Follow-ups more than 20 years after neck surgery are extremely rare. No previous randomized studies have investigated differences in pain and disability more than 20 years after ACDF surgery using different techniques. The purpose of this study was to describe pain and functioning more than 20 years after anterior cervical decompression and fusion surgery, and to compare outcomes between the Cloward Procedure and the carbon fiber fusion cage (CIFC).
METHODS: This study is a 20 to 24-year follow-up of a randomized controlled trial. Questionnaires were sent to 64 individuals, at lea

We report the case of a 51-year-old woman who developed neuromyelitis optica spectrum disorders (NMOSD) associated with primary biliary cholangitis (PBC). When she was 38 years old, she subacutely developed headache and urinary retention. A diffusion weighted image (DWI) on brain MRI showed high signal intensity in the left temporal white matter, and T2 weighted image (T2WI) on spine MRI showed high signal intensities in the spinal cord. After the initial event, follow-ups at 2, 6 and 9 months revealed that she developed neurological symptoms, and T2WI on spine MRI showed high signal intensiti

BACKGROUND: Aneurysms arising from the PICA are infrequent and those arising from an extracranially originating PICA in the extracranial intradural region are even rare. We report a case of bilaterally extracranial extradurally originating PICAs, each of which had an aneurysm, both being situated below the foramen magnum. This to the best of our knowledge is the first of its kind to be reported.
CASE DESCRIPTION: A 35-year-old woman presented with giddiness and headache of 2-day duration. CT scan head revealed blood in the 4(th) ventricle. 4 vessel angiogram revealed bilateral extracranial, e

This case report illustrates the difficulties that can arise when diagnosing a CNS infection. Following an episode of gastroenteritis, our patient developed dizziness, a fever up to 39 °C, photophobia, double vision, multiple cranial nerve deficits, and a resting nystagmus with alternating directions, and presented to the emergency department. Blood inflammatory markers were unremarkable, and a contrast-enhanced head CT scan revealed no abnormalities. The patient was discharged home with a provisional diagnosis of Meniere's disease. Two days later, the patient developed increasing lethargy and
